Az akut (AP) és krónikus (KP) hasnyálmirigy gyulladás továbbra is kihívást jelent mind a klinikusok mind pedig a kutatók számára. A súlyos AP mortalitása a mai napig elfogadhatatlanul magas (30-40%), míg a KP komoly rizikója a hasnyálmirigy rák kialakulásának, melynek mortalitása az egyik legrosszabb a ráktípusok között. Az elkeserítő klinikai adatok tükrében még kevésbé elfogadható, hogy a betegségek kialakulásának kezdetéről és kifejlődéséről ismereteink hiányossak. Munkacsoportunk kimutatta, hogy mind az AP mind pedig a KP korai fázisában a CFTR klorid csatorna gátlása fontos szerepet játszik (Gastroenterology 2011, Gastroenterology 2015-ez utóbbival a Nature Rewievs kiemelt cikként foglalkozott). Mindemellett, irodalmi adatok utalnak arra, hogy a KP és rekurráló AP kezdeti szakaszában a cisztás fibrózishoz hasonló protein gél ill. nyákdugó jelenik meg. Ezen két mechanizmus kapcsolatának megértése döntő fontosságú a fent említett betegségek megértéséhez.
Jelen pályázatunkban célul tűztük ki, hogy megfigyeljük a nyákszekréció változását pancreas betegekben, majd ezután magas szintű orvosbiológiai kutatási módszerek segítségével (sejtes kísérletektől a gén módosított egér modell kialakításáig) választ találjunk kérdéseinkre. A projekt a klinikum és alapkutatás közötti transzlációt kívánja megteremteni a legmodernebb technológiát használva eszközként. Ezáltal ez a projekt magában hordozza a lehetőségét egy új famakológiai célpontnak és annak, hogy csökkentsük az akut és krónikus pancreatitis morbiditását és mortalitását

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.
Hipotézisünk, hogy a pancreatitist indukáló faktorok, úgy mint alkohol, dohányzás, epesavak az epitheliális ion- és folyadék szekréció gátlásán túl megváltoztatják a termelődött nyák összetételét és mennyiségét. Ez az összetett mechanizmus megnövelheti a hasnyálmirigy nedv viszkozitását, mely kóros epitheliális nyák fenotípus kialakulásához vezethet. Ebben a tanulmányban azt szeretnénk megvizsgálni, hogy a nyákszekréció zavar milyen szerepet tölt be a pancreatitis lefolyásában. Ebben a 4 éves időtartamban a legmodernebb in vitro és in vivo technikákkal kívánjuk (i) megvizsgálni az akut és krónikus pancreatitises betegekben fellépő nyákszekréció defektust, (ii) állatmodellek segítségével megérteni a hasnyálmirigy által termelt nyák homeosztázisban fellépő változásokat,(iii) és megérteni a nyák szerepét a betegségek kialakulásának kezdetében és kifejlődésében.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. A heveny és krónikus hasnyálmirigygyulladás pathogenezisének, főleg annak kedzeti szakaszának megértése kulcsjelentőségű az új terápás célpontok felismeréséhben. Projektünkben a nyákszekréció zavarának átfogó vizsgálatát tűztük ki célul, ami az előzetes irodalmi adatok és eredmények alapján fontos szerepet játszhat az AP és KP kialakulásában. Megfigyeléseink új gyógyszercsoportok kifejlesztésének alapját is képezhetik, mely a a humán egészség fejlesztéséhez nagymértékben járulna hozzá.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.
A heveny és krónikus hasnyálmirigy gyulladás súlyos emésztőszervi megbetegedések, melyekre nem rendelkezünk oki kezeléssel, mivel mai napig hiányosak az ismereteink a betegség kialakulásáról és lefolyásáról. A betegágy mellett dolgozó orvosok és alapkutatók közötti információ áramlás (transzláció) biztosítja a problémák felvetését, a kísérletek kidolgozását, mely jelentős eredményt hozhat a hasnyálmirigy gyulladás megelőzésben és terápiájában, melynek következtében a betegek élettartama megnövelhető, életminőségük javítható. A pályázatunk célja, hogy a betegágy mellett dolgozó orvosok és alapkutatók (biológusok és orvosok) fogjanak össze, hogy (i) karakterizáljuk a hasnyálmirigy hámejtjeinek működését és nyáktermelésének szerepét az emberi hasnyálmirigy gyulladásos megbetegedéseiben (ii) kísérletes módszerekkel modellezzük a betegséget, hogy megértsük a hasnyálmirigy nyák termelésében fellépő változásokat és annak jelentőségét (iii) és az eredmények bírtokában új terápiás útmutatásokat adjunk. Megfigyeléseink új gyógyszercsoportok kifejlesztésének alapját is képezhetik.

Acute and chronic pancreatitis (AP and CP) are the two major inflammatory diseases of the pancreas, that can lead the patients to serious, life threatening condition.The mortality rate of severe AP is still unacceptable high (30-50%) whereas CP can turn to pancreatic carcinoma, the most malignant cancer type.Moreover, none of the above mentioned diseases have specific pharmacological treatment.Despite the frightful clinical situation, our current understanding of the initiation, development and progression of AP and CP is largely deficient. We have recently shown that disrupt level and function of cystic fibrosis transmembrane conductance regulator (CFTR) chloride channel plays a crucial role in the early phase of AP and apparent in CP as well (Gastroenterology 2011, Gastroenterology 2015–the latter one is highlighted by Nature Reviews HG). One of the leading histological features of CP and recurrent pancreatitis is the formation of a protein gel in the fine pancreatic ducts, while the main pathological feature of cystic fibrosis is the accumulation of thick and sticky mucus.Therefore understanding the mucus homeostasis of the pancreas is highly important both in physiological and pathophysiological aspects. In this project we will first plan to understand the characteristics of mucin homeostasis in human and after that, we will go to the bench side and will use sophisticated, new generational biomedical techniques to get answers for our questions.The project focuses on cross-functional collaborations between clinicians and researchers.The results of this project may open up the possibility of pharmacological therapy of AP and CP leading to reduced morbidity and mortality

What is the major research question?
Describe here briefly the problem to be solved by the research, the starting hypothesis, and the questions addressed by the experiments.
Our hypothesis is that pancreatitis causing toxic factors, such as alcohol, tobacco and bile acids in addition to having a deleterious effect on ductal electrolyte transport also alter the regulation of mucus secretion. The combined effect will elevate the viscosity of pancreatic juice, create a pathological epithelial mucus phenotype leading to ductal cell damage, acinar cell atrophy and progressive tissue damage. In this study we wish to prove our hypothesis that the imbalance of mucus homeostasis is a key and early event of the pathogenesis of pancreatitis. In this 4-year-project we propose to employ both in vitro and in vivo cutting-edge physiological, biochemical and molecular biological techniques to (i) understand the alteration of mucus homeostasis in patients suffering in acute and chronic pancreatitis, to (ii) develop animal models to investigate the importance of mucus changes in the pancreas, to (iii) understand the role of mucus in the initiation, development and progression of pancreatitis.

Understanding the pathomechanism, especially the initial steps of the pathogenesis of acute and chronic pancreatitis is highly important for developing new therapies. The scientific literature and our preliminary data indicated that disturbances in mucus secretion is highly important, therefore in this project we plan to conduct a comprehensive study on the role of mucus secretion in health and disease.
Importantly, the study is based on the most recent technical and conceptual advances in pancreatitis research and, as far as we are aware, there are no similar projects in progress within the international pancreatic research community. The results of this project may open up the possibility of pharmacological therapy of acute and chronic pancreatitis

Summary and aims of the research for the public

Acute and chronic pancreatitis are severe gastrointestinal diseases, for which no specific pharmacological therapy exist. This is due to the lack of understanding of the development and progression of these diseases. The communication between doctors working at the bedside and researchers working at the bench side helps to focus on important research questions and improve experimental design. This translational approach may bring advances in pancreatitis prevention and therapy, ultimately increasing the life expectancy and life quality of patients. The aim of this study is to (i) characterize the mucus secretion of the epithelial cells of the pancreas in human health and disease, (ii) to experimentally examine the mechanism of the disturbances in mucus secretion and (iii) according to the results of the study develop new therapeutic guidelines. The results of this project may open up the possibility of pharmacological therapy of acute and chronic pancreatitis

Krónikus hasnyálmirigy gyulladás (CP) esetében megnövekedett mukusz mennyiséget detektáltunk kis méretű hasnyálmirigy duktuszokban. MUC6 és MUC5B (szekrécióban résztvevő) mucinok megnövekedett termelése volt megfigyelhető humán mintákban, egérben pedig Muc6 esetén. In vivo és in vitro folyadék szekréció csökkent mennyiségét tapasztaltuk cerulein indukálta CP esetén. Az idő-esemény változások vizsgálata kimutatta, hogy a duktális ion szekréció károsodása parallel a megnövekedett Muc6 expresszióval. Projektünk következő lépésében, azonosítottunk új variánsokat a humán kationos tripszinogén génben; p.P17T és p.L104P. Kísérleteink rávilágítanak arra, hogy az új p.P17T variáns megemelkedett tripszinogén aktivációhoz vezet, míg a p.L104P mutációja genetikaliag kódolt endoplazmatikus retikulum stresszt eredményez ami a hasnyálmirigy gyulladásnak egy tripszin független útvonala. Elsőként mutattuk ki, hogy az endoplazmatikus stressz indukáló p.L104P mutáció az öröklődő hasnyálmirigy gyulladás okozója. Bemutattuk a tripszinogén aktiváció fontos szerepét a hasnyálmirigy gyulladás pathofiziológiájában. Továbbá bemutattuk az AQP1 (aquaporin 1) expresszióját és szerepét a hasnyálmirigy fiziológiában és pathofiziológiában. Eredményeink rámutatnak arra, hogy az AQP1 interakcióban van a CFTR (cisztás fibrózis transzmembrán regulátor) csatornával és részt vesz a hasnyálmirigy folyadék szekréciós működésében.

Results in English

We demonstrate increased mucus content in the small pancreatic ducts in CP. Secretory mucins MUC6 and MUC5B were upregulated in human, Muc6 in mouse CP. In vivo and in vitro fluid secretion was decreased in cerulein-induced CP. Analysis of time-course changes showed that impaired ductal ion transport is paralleled by increased Muc6 expression. At the next part of our project we identified novel variants in human cationic trypsinogen gene ; p.P17T and p.L104P. Our experiments revealed that the novel mutation p.P17T causes accelerated trypsinogen activation, while mutation p.L104P encodes genetically determined endoplasmatic reticulum stress which is a trypsin-independent pathway of pancreatitis. We were the first who described that the endoplasmatic reticulum stress inducing p.L104P mutation cause hereditary pancreatitis. We have shown the crucial role of trypsinogen activation in the pathophyisiology of pancreatitis. Henceforth, we provided evidence regarding the expression and role of AQP1 (aquaporin) in the physiology and pathophysiology of the pancreas. Our data indicate that AQP1 interacts with the CFTR (cystic fibrosis transmembrane conductance regulator) Cl- channel and takes part in the formation of pancreatic fluid.
