A hosszú szénláncú, többszörösen telítetlen zsírsavakat (LCPUFA) legalább 25 éve az egyik legfontosabb étrendi kiegészítőként tartják számon, mivel az n-3 LCPUFA csökkent arányának szerepet tulajdonítanak számos gyulladással kapcsolatos klinikai állapot kialakulásában. Korábbi saját vizsgálataink eredményei alapján nagy biztonsággal állítható, hogy a) transz izomér telítetlen zsírsavakat a vizsgálatok eredményét befolyásoló zavaró tényezőnek kell tekinteni minden az LCPUFA ellátottságot tanulmányozó kutatásban és b) a vizsgált csoportok nemi megoszlása ugyancsak zavaró tényezőként szerepelhet az LCPUFA metabolizmusának vizsgálataiban. Ezen saját korábbi megfigyelésink alapján tervezzük az LCPUFA ellátottság további vizsgálatát olyan klinikai kórképekben, ahol gyulladásos folyamatok szerepelnek kóroki tényezőként. Tervezett nemcsak az LCPUFA metabolizmus jobb megértéséhez járulhatnak hozzá, hanem alapjául szolgálhatnak étrendi kiegészítő készítmények gyermekkori alkalmazásának is. Ezen gondolatkörön belül randomizált kontrollált LCPUFA szupplementációs vizsgálatot tervezünk diabetes gyermekben. Az elmúlt időszakban a gyermekgyógyászati érdeklődés homlokterébe került étrendi kiegészítők a prebiotikumok. Ezen a kutatási területen korábban Magyarország hat városára kiterjedő vizsgálatot szerveztünk, valamint rendszerezett áttekintő közleményben foglaltuk össze a kérdést a nemzetközi tudományos közvélemény számára. Jelen pályázat támogatásával pedig azt a hipotézist kívánjuk vizsgálni, hogy a prebiotikumok hatásosak lehetnek-e az egészséges gyermekekben kialakuló lázas epizódok gyakoriságának a csökkentésére.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.
Okkal vetődik fel a kérdés, hogy a korai perinatális LCPUFA szupplementációnak lehet-e a gyermekkor későbbi szakaszára áthúzódó hatása. A közelmúltban beszámoltunk az iskoláskor kezdetén tetten érhető hatásról, a jelen pályázat nyújtotta lehetőséggel pedig folytathatnánk a nyomon követést az iskoláskornak az iskolai teljesítmény jobb megítélését lehetővé tevő későbbi időszakában is. Feltételezzük, hogy mind a korai posztnataális, mind pedig az aktuális LCPUFA státusz összefüggésben állhat a gyermekkori allergiás tünetek kialakulásával. A Heidelbergben működő Német Rákkutató Centrum Klinikai Epidemiológiai Részlegével folytatott korábbi együttműködés továbbvitelével megvizsgálhatjuk a női tej zsírsavösszetétel r által jellemezett korai LCPUFA státusz és a későbbi gyermekkor allergiás jelenségeinek az összefüggését. A korábbi megfigyeléses vizsgálatainkban szerzett tapasztalatainkra alapozva tervezzük n-3 LCPUFA tartalmú étrendi kiegészítő alkalmazását diabeteses gyermekekben. Azon feltételezés alapján, hogy az inulin-típusú fruktánok csökkenthetik az infekciós hátterű lázas epizódok kialakulásának gyakoriságát 3-6 éves gyermekekben, randomizált kontrollált klinikai vizsgálatot tervezünk. Ki kívánjuk használni azt a körülményt, hogy a jelen pályázat vezető kutatója egyben a Magyar Cochrane Tagozat igazgatója is. Ezzel a háttérrel jó lehetőség nyílhat nemcsak hagyományos rendszerzett áttekintő közlemények létrehozására, hanem adott esetben Cochrane Review összeállítására is.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!
Miután az anyatejpótló csecsemőtápszerek LCPUFA-val történő kiegészítése immár bevett gyakorlattá vált, napjainkban a szoptatott csecsemők LCPUFA ellátottságát befolyásoló tényezők vizsgálat került az érdeklődés homlokterébe. A korábbi vizsgálatunkban a várandós anya étrendjének kiegészítése útján LCPUFA szupplementációban részesült gyermekek késői fejlődésneurológiai nyomon követése nemcsak jól beilleszkedik munkacsoportunk nemzetközileg elismerti aktivitásába, de hozzájárulhat ennek a speciális nutríciós intervenciónak a jobb megértéséhez is. Az LCPUFA metabolizmus zavarainak a különböző kórképekben történő jobb jellemzése hozzájárulhat étrendi kiegészítőkkel folytatott klinikai vizsgálatok tervezéséhez ezekben a klinikai helyzetekben. A trsnz izomér zsírsavak zavaró szerepének és a vizsgált csoportok különböző nemi megoszlása befolyásoló szerepének pontosabb körvonalazása általában véve is javíthatja a zsírsav-analitikai mutatókat értékelő klinikai vizsgálatok hatékonyságát. A diabetese gyermekek étrendjének n-3 LCPUFA-val történő kiegészítése egyszerű eszköze lehet a lipid metabolizmus zavarának mérséklésében. A fiatal egészséges gyermek étrendjének prebiotikummal történő kiegészítése pedig hatásos eszköz lehet az infekciós eredetű lázas állapotok gyakoriságának mérséklésében. Bár ezek a hétköznapinak tartott lázas epizódok általában nem teremtenek egészségügyi veszélyhelyzetet, azonban összességükben jelentős társadalmi költségekkel járhatnak. Egy Magyarországon készített Cochrane Review jelentősen erősíthetné a bizonyítékokon alapuló orvoslás hazai helyzetének nemzetközi megítélését

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.
A linolsav és az alfa-linolénsav azon biológiailag fontos hosszú-szénláncú többszörösen telítetlen zsírsavak (LCPUFA) szintézisének a prekurzorai, melyek különböző prosztaglandinok, tromboxánok és leukotriének prekurzoraiként is szerepet játszanak. Az LCPUFA pedig a gyermekkori étrendi kiegészítő táplálás egyik legfontosabb eszköze. Az omega-6 és omega-3 LCPUFA megbomlott egyensúlyának szerepet tulajdonítanak számos gyulladással kapcsolatos klinikai állapot kialakulásában. A prebiotikumok olyan fermentált étrendi összetevők, melyek a gyomor-bélrendszer baktérium flórájának az összetételét jótékonyan befolyásolva ígérnek egészségnyereséget. A jelen pályázat keretében egészséges és beteg gyermekek jól meghatározott csoportjaiban kívánunk adatokat nyerni az LCPUFA és a prebiotikumok étrendi kiegészítőként történő alkalmazhatóságáról. Ennek keretében tervezzük a korábbi vizsgálatainkban részt vett gyermekek nyomon követését éppúgy, mint az allergiás jelenségek hátterében álló, az LCPUFA háztartás a megszületést követő vagy a megbetegedést közvetlenül megelőző időszakban megmutatkozó eltéréseink felmérését egyaránt. Tervezzük továbbá placébóval kontrollált klinikai vizsgálatokban mind az LCPUFA, mind pedig a prebiotikum étrendi kiegészítés az egészség különböző jellemzőire gyakorolt hatásának a megítélést.

Long-chain polyunsaturated fatty acids LCPUFA have been generally considered as one of the most important potential dietary tools in preventive nutrition for at least 25 years. Disturbed equilibrium of n-6 to n-3 LCPUFA has been suggested to play a role in the development of various clinical conditions with inflammation. On the basis of our previous observations it can be firmly concluded that a) trans fatty acids should be considered as confounding variables in any study on LCPUFA status b) gender might be significantly related to the availability of LCPUFA as well. Armed with these own experiences, now we plan to to investigate LCPUFA status in various paediatric diseases in that inflammatory processes play important pathophysiological role. Data obtained in these disease conditions should contribute not only to our better understanding the role of LCPUFA in the pathogenesis of inflammation, but may open the field for clinical studies on supplementing n-3 LCPUFA. We plan to organise randomised controlled clinical trial for testing LCPUFA supplementation in paediatric diabetes. Novel potential supplement to be investigated within paediatric nutrition is the prebiotic family of nutritional supplements. Our early research contribution to this topic so far was the organisation of a nation-wide collaborative study in Hungary, followed by systematic review of the field. Now we hypothesise that dietary supplementation with prebiotic inulin type fructans may significantly reduce the incidence of infectious diseases in healthy children and plan to organise placebo controlled randomised trial with this dietary supplement as well.

What is the major research question?
Describe here briefly the problem to be solved by the research, the starting hypothesis, and the questions addressed by the experiments.
We hypothesise that optimisation of prenatal LCPUFA supply might offer detectable benefits in later periods of childhood. We have recently reported our observations made at early school-age. The support of the present project proposal might allow us to continue the clinical follow-up and neurological testing up to the age of the children, when also school achievement are measurable. We hypothesise that both early postnatal and actual LCPUFA status might be related to the development of allergic symptoms in later periods of childhood. By maintaining the fruitful collaboration with the Division of Clinical Epidemiology and Aging Research, German Cancer Research Centre, Heidelberg, Germany, now we can revisit the question of the role of LCPUFA status characterised by fatty acid composition of human milk in allergy development. Based on our previous data obtained in observational studies, we aim to investigate the effect of n-3 LCPUFA supplementation in diabetic children on LCPUFA and lipid status as well as clinical parameters of diabetes control. We hypothesise that dietary supplementation with prebiotic inulin type fructans will significantly reduce the incidence of infectious diseases in healthy children aged 3 to 6 years during cold season when compared to control children as indicated by the reduced number of febrile infectious episodes during the 6 months study period. The principal researcher of the present project proposal is the head of the Hungarian Cochrane Centre. This background provides excellent opportunity not only for the composition of systematic review (meta-analysis), but to approach the Cochrane Collaboration with the idea of a Cochrane review as well.

What is the significance of the research?

After general acceptance of the potential need for LCPUFA supplementation to infant formulae, factors potentially influencing perinatal LCPUFA supply also in the breast-fed infant became the focus of interest. Follow-up of the neurodevelopment in children participating in our previous nutritional trial using n-3 LCPUFA supplements in pregnant women will fit well into the series of our observations describing the long-term effect of supplementation and contribute to our better understanding of overall effect of such unique nutritional intervention. Today, clear-cut delineation of altered LCPUFA status together with other putative disturbances in lipid metabolism should provide ground to plan clinical trials aimed to correct abnormalities in LCPUFA status. Analysis of the potential confounding effect of different level of exposure to trans isomeric fatty acids and different gender distribution on fatty acid composition of biological samples will contribute to the valuation of data obtained in any research carried out by using fatty acid analysis. Dietary supplementation of n-3 LCPUFA in diabetes may normalise non-physiological availability of LCPUFA and contribute with easily achievable means to the well-being of the patients. Similarly, dietary supplementation of prebiotic to young children might offer simple method to reduce the number of infectious episodes in healthy children. Though these minor infections usually do not cause severe medical concern, nevertheless they contribute substantially to the financial costs of the society. Composing Cochrane Review might strengthen the position of Cochrane Hungary within the international Cochrane community.

Linoleic and alpha-linolenic acids are the precursors of the synthesis of biologically important long-chain polyunsaturated fatty acids (LCPUFA) that play important biological roles also as precursors of various prostaglandins, thromboxans and leukotrienes. LCPUFA are generally considered as one of the most important potential dietary tools in preventive nutrition. Disturbed equilibrium of n-6 to n-3 LCPUFA has been recently suggested to play a role in the development of various clinical conditions with inflammation. Prebiotics are selectively fermented dietary ingredients, which promote specific changes in the composition and/or activity of the gastrointestinal microbiota and thus confer benefits on the host health. The present project aims to provide in clinically carefully characterised, sizeable groups of paediatric patients broad spectrum of data on these biologically active compounds. We plan to follow-up children who participated in our previous nutritional trials, but we also plan to initiate novel observational studies both in healthy infants and in children suffering from allergic symptoms. Moreover, in placebo controlled randomised clinical trials we aim to investigate the effects of relatively easily available dietary supplements like n-3 LCPUFA or inulin-type fructans to improve health both in sick children suffering from diabetes mellitus and in the healthy paediatric population.

Főzőolajok egymásutáni rövid hevítéseit követően a zsírsavösszetétel kedvezőtlen változásai szorosabb összefüggésben vannak az olaj minőségével, mint a hevítések számával (Foods, IF: 4,121). Az ejkozapenténsav és a dokozahexénsav a köldökzsinórvérben mért magasabb értékei a gyermekkori allergiás állapotok kisebb kockázatával hozhatók összefüggésbe (Life, IF: 3,817). Felhívtuk a figyelmet az ejkozapenténsav és a dokozahexénsav potenciális gyulladáscsökkentő hatására SARS-COV-2 betegségben (Frontiers of Physiology, IF: 3,160). Megállapítottuk, hogy a prebiotikus hatású fruktánok profilaktikus alkalmazásával csökkenthető a lázas epizódok és az arcüreggyulladás előfordulási gyakorisága 3-6 éves gyermekekben (The Journal of Nutrition, IF: 4,398). A prebiotikus fruktánok adásának az antibiotikus kezelést követő bél-diszbiózisra gyakorol kedvező hatását írtuk le 3-6 éves gyermekben (Beneficial Microbes, IF: 2,939). Rendszerezett áttekintő közleményben számoltunk be arról, hogy az inulin-szerű fruktánok a megszületéstől egészen 83 éves életkorig szignifikánsan fokozzák a Bifidobaktériumok jelenlétét a bélflórában (Crit Rev Food Sci Nutr, IF: 12,260). Magyarország összes orvoskarát és egészségtudományi karát felölelő felmérésünkben megállapítottuk, hogy az egészségügyi munkatapasztalat és a hallgatói kutatótapasztalat szignifikáns pozitív összefüggésben áll a bizonyítékokon alapuló egészségügy ismereteinek szintjével (PLOS One, IF: 2,740).
Results in English
We measured changes in fatty acid composition and in some health-related indices in edible oils after short-time heating and found that the type of oil has a greater effect on the changes of health-related indices than the number of heating sequences (Foods, IF: 4.121). In a systematic review we found that high cord blood eicosapentaenoic and docosahexaenoic acid values were significantly associated to clinically relevant reduction in allergy (Life, IF: 3.817). We drew the attention to the possible beneficial effect of eicosapentaenoic acid and docosahexaenoic acid supplementation in SARS-CoV-2 infection (Frontiers of Physiology, IF: 3.160). We found that prophylactic supplementation with prebiotic fructans was able to decrease the incidence of febrile episodes and sinusitis in 3 to 6 years old children (The Journal of Nutrition, IF: 4.398). We found that prebiotic supplementation attenuated antibiotic-induced disturbances in children aged 3 to 6 years (Beneficial Microbes, IF: 2.939). We reported in a systematic review that chicory-derived inulin-type fructans significantly increased Bifidobacterium abundance in participants with an age range from 0 to 83 years (Crit Rev Food Sci Nutr, IF: 12.260). We performed an online survey in all medical and health sciences faculties in Hungary and found that healthcare work experience and conducting student research were the most important factors significantly influencing evidence-based medicine knowledge (PLOS One, IF: 2.740).
