A klinikailag felismert terhességek 20%-a végződik vetéléssel, további 20%-ukban pedig szülészeti kórképek lépnek fel, melyek az anya és magzat egészségét és életét súlyosan veszélyeztethetik. E szövődmények hátterében sokszor az apai antigénekkel szemben kialakuló anyai-magzati immuntolerancia zavara, illetve lokális (in utero) vagy szisztémás anyai gyulladásos folyamatok állhatnak. Noha egyre több információ áll rendelkezésre a trofoblaszt/embrió eredetű extracelluláris vezikulák (tEV-k/eEV-k) és szülészeti kórképek összefüggéseiről, a tEV-célsejt interakciók molekuláris mechanizmusának jellemzése még nem történt meg. Ezen interakciókban kulcsszerepe lehet egyes, a placenta vagy embrió által termelt fehérjéknek, melyek az anya-embrió kapcsolatot szabályozzák, és mennyiségi változásaik megfigyelhetők egyes terhességi kórképekben. E pályázat célja olyan még egyáltalán nem vagy csak kevéssé jellemzett placenta/embrió fehérjék vizsgálata, melyek tEV-k/eEV-kben történő expressziója, a tEV/eEV-k mediálta anyai immunmodulációban játszott szerepe koraterhességben még nem ismert. Célunk e fehérjék overexpresszálása EV-kben új in vitro sejtmodell-rendszerben, valamint EV-khez kapcsolódó fehérjék anyai immunsejt interakcióinak molekuláris jellemzése és rendszerbiológiai módszerekkel történő vizsgálata. Ez a jelátviteli hálózat felépítése és működése szempontjából jelentős kulcsfehérjék, szövet- és betegség-specifikus „cross-talk” fehérjék, valamint gyógyszer-hatáspontok azonosításához vezethet. A pályázati támogatással az anyai-embrió immunológiai kapcsolatának olyan új útvonalait ismerhetjük meg részletesen, melyeknek diagnosztikus és terápiás jelentősége lehet.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.
Probléma:
Egyre nagyobb számú kísérletes adat utal arra, hogy az embrió illetve a méhlepény trofoblaszt sejtjei által termelt extracelluláris vezikuláknak (EV-k) fontos fiziológiás szerepe van az anyai immunrendszer modulációjában, a terhesség létrejöttében és fenntartásában. Az embrió/méhlepény fehérjéknek az EV-k terhesség-specifikus immunmoduláló hatásában betöltött szerepét még sem fiziológiás sem pedig patológiás terhességekben nem vizsgálták.

Hipotézis:
Az embrióból/méhlepényből az anyai keringésbe illetve a deciduába jutó, EV-khez kapcsolt fehérjék meghatározó szerepet játszanak az anyai immunrendszer működésének - lokális és szisztémás - szabályozásában a terhesség korai szakaszában. Az EV-k megváltozott (mennyiségi és összetételi) mintázata az anyai keringésben a vetélések jó biomarkere lehet.

Kérdések:
1) Rendelkeznek-e immunmoduláló hatással az embrió/méhlepényi fehérjéket expresszáló EV-k?
2) Milyen moduláló hatásai vannak az EV-knek az immunsejtek válaszára a veleszületett (neutrofilek, NK sejtek) és adaptív (limfociták) immunrendszer esetében?
3) Milyen moduláló hatásai vannak egyes - az EV-kben overexpresszió útján feldúsult – embrió/méhlepényi fehérjéknek a veleszületett és adaptív immunválaszban részt vevő immunsejtekre? Milyen jelátviteli útvonalakon keresztül fejtik ki hatásaikat?
4) Van-e diagnosztikus jelentősége az embrió/méhlepényi fehérjéket expresszáló EV-knek, mint potenciális biomarkereknek, vetélésekben?

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!
Új perspektívák az alapkutatásban:
1) A tervezett kutatások olyan in vitro modellrendszer létrehozását eredményezik, amellyel adekvát módon lehet majd vizsgálni az embrió/méhlepényi fehérjék EV-k közvetítette terhesség-specifikus immunmoduláló hatásait.
2) Ezáltal először válik majd lehetővé az embrió/méhlepényi fehérjéket expresszáló EV-k sejtszintű hatásmechanizmusainak részletes jellemzése.
3) Az EV-célsejt interakciók molekuláris jellemzése és rendszerbiológiai módszerekkel történő vizsgálata lehetővé teszi új szövet- és betegség-specifikus „cross-talk” fehérjék, jelátviteli fehérjék és hálózatok azonosítását és jellemzését.
4) Az embrió/lepényi fehérjéket expresszáló EV-k kimutatása az anyai keringésben és ezek vizsgálata vetélésekben alapkutatási jelentőséggel bír.
5) A keringő embrió/lepényi fehérjéket expresszáló EV-k és a terhesség klinikai paramétereinek korrelációs vizsgálata egyrészt diagnosztikus és prognosztikus biomarkerek, másrészt új terápiás célpontok felfedezését eredményezheti.

Társadalmi hasznosíthatóság:
1) Az anyai vérben keringő, embrió/méhlepényi fehérjéket expresszáló EV-k vizsgálatának fontos szerepe lehet a vetélések kialakulásának előrejelzésében. Az anyai vér EV-mintázatának vizsgálata, a terhesgondozás során rutinszerűen alkalmazott vizsgálati módszerek kiegészítéseképpen, érzékenyebbé teheti a terhességi rizikóbecslést.
2) Az embrió/méhlepényi fehérjéket expresszáló EV-k és az anyai immunrendszer kapcsolatának rendszerbiológiai módszerekkel történő vizsgálata olyan új jelátviteli útvonalak, betegség mechanizmusok és gyógyszer-célpontok azonosításához vezethet, melyeknek komoly terápiás jelentősége lehet a terhesség során.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.
A sikeres terhesség alapfeltétele az anya és embrió/magzat között kialakuló folyamatos immunológiai párbeszéd a fogantatástól a születésig. E kommunikáció zavara vetélésekhez és szövődményes terhességek kialakulásához vezethet. Ez a klinikailag felismert terhességek esetén 20%-ban vetélést, további 20%-ban pedig szülészeti kórképek kialakulását jelenti, melyek az anya és magzata egészségét és életét súlyosan veszélyeztethetik. Ez különös jelentőséggel bír az elöregedő társadalom és a csökkenő születésszámok mellett hazánkban. Az anya és embrió közti kommunikáció napjainkban felismert új útvonalát jelentik az embrió vagy méhlepény sejtjei által az anyai keringésbe bocsátott apró hólyagocskák, úgynevezett „mikrovezikulák” közvetítette üzenetek az anyának. A pályázatban vizsgálni kívánt méhlepény vagy embrió eredetű fehérjék fő üzenethordozói lehetnek e mikrovezikuláknak. Kérdésfeltevésünk arra irányul, hogy mennyire és milyen módon kapcsolódnak az embrió/méhlepény fehérjék a mikorvezikulákhoz és hogyan szabályozzák az anyai immunrendszer működését koraterhességben. Feltételezéseink szerint az anyai vérben kimutatható mikrovezikulák embrió/méhlepényi fehérjéinek vizsgálata előre jelezheti a vetélések kialakulását. A terhesgondozás során rutinszerűen alkalmazott vizsgálati módszerek kiegészítése az anyai vér mikrovezikuláinak vizsgálatával érzékenyebbé teheti majd a terhességi kockázatbecslést. Az alapkutatás szempontjából a mikrovezikulák és az anyai immunrendszer kapcsolatának vizsgálata új betegség mechanizmusok és gyógyszer-célpontok azonosításához vezethet. Összességében vizsgálatainknak komoly diagnosztikus és terápiás jelentősége lehet.

Twenty percent of clinically recognized pregnancies end with miscarriage, and 20% of pregnant women develop various pregnancy complications, which may cause severe maternal and fetal morbidities and death. The origin of these pregnancy complications is often rooted in the failure of maternal-fetal immune tolerance against paternal antigens, or local (uterine) or systemic maternal inflammatory conditions. Although trophoblast/embryo-derived extracellular vesicles (tEVs/eEVs) have been implicated in several pregnancy complications, the molecular mechanisms of EV-target cell interactions remain elusive. Proteins produced by the placenta or embryo, which regulate maternal-fetal connection and have altered expression in pregnancy complications, may play a crucial role in these molecular interactions. The goal of this grant proposal is to investigate such poorly or not yet characterized placental/embryonal proteins, which have unexplored expression in tEVs/eEVs and participation in tEV/eEV-mediated maternal immune modulation in early pregnancy.
Our aim is to overexpress these proteins in EVs via a new cellular model as well as to perform the molecular and systems biology characterization of the interactions between these EV-associated proteins and maternal immune cells. This will support the identification of key proteins in signaling networks, tissue- and disease-specific “cross-talk” proteins, as well as pharmaceutical targets. Altogether, with the support of the grant proposal we will unravel novel maternal-fetal immune-interactions and signaling pathways that have diagnostic and therapeutic significance.

What is the major research question?
Describe here briefly the problem to be solved by the research, the starting hypothesis, and the questions addressed by the experiments.
Problem:
Accumulating experimental evidence suggests that extracellular vesicles produced by the embryo or trophoblastic cells of the placenta (EVs) have physiological role in the modulation of the maternal immune system, in the establishment and maintenance of pregnancy. Proteins produced by the embryo/placenta have not been characterized for their EV-associated pregnancy-specific immunoregulatory functions in physiological or pathological pregnancies.

Hypothesis:
Embryo/placental proteins associated with EVs are released into the maternal circulation or deposited into the decidua and play significant roles in the regulation of the maternal immune system in early pregnancy. The altered EV pattern (quantities and compositions of these EVs) in the maternal circulation may be good biomarker for miscarriages.

Questions:
1) Do EVs carrying embryo/placental proteins have immunoregulatory properties?
2) What are the modulatory effects of EVs on various cellular responses in the innate (neutrophils and NK cells) and adaptive (lymphocytes) immune system?
3) What are the modulatory effects of EV-associated embryo/placental proteins on various cellular responses in the innate (neutrophils and NK cells) and adaptive (lymphocytes) immune system? Which signal transduction pathways are modulated in these immune cells by embryo/placental proteins?
4) Is there a diagnostic significance of EVs expressing these embryo/placental proteins in miscarriages?

What is the significance of the research?
Describe the new perspectives opened by the results achieved, including the scientific basics of potential societal applications. Please describe the unique strengths of your proposal in comparison to your domestic and international competitors in the given field.
New perspectives in basic research:
1) The proposed study will result in the establishment of an in vitro model system, which will enable the adequate studies of the pregnancy-specific immunomodulatory effects of embryo/placental proteins mediated through EVs.
2) This will enable, for the first time, the detailed assessment of the molecular effects of EVs expressing embryo/placental proteins at the cellular level.
3) The molecular biological characterization of EV-target cell interactions along with systems biology analyses support the identification of novel tissue- and disease-specific “cross-talk” proteins, signaling proteins and networks.
4) The detection of EVs expressing embryonic/placental proteins in the maternal circulation and testing their differential abundance in miscarriages has impact on basic research.
5) The correlation analysis of embryo/placental protein expressing EVs and the clinical parameters of pregnancy may lead to the discovery of diagnostic and prognostic biomarkers as well as therapeutic targets.

Social utilization:
1) The investigations of EVs expressing embryo/placental proteins in the maternal circulation may have important contribution to the prediction of miscarriages. Testing EV-patterns in maternal blood as an addition to routine prenatal tests may increase the sensitivity of risk assessment algorithms in pregnancy.
2) The systems biology analyses of the interactions of EVs expressing embryo/placental proteins with the maternal immune system may support the identification of novel signaling pathways, disease mechanisms and pharmaceutical targets which may have therapeutic significance during pregnancy.

Summary and aims of the research for the public
Describe here the major aims of the research for an audience with average background information. This summary is especially important for NRDI Office in order to inform decision-makers, media, and others.
The ultimate requirement for a successful pregnancy is the continuous immunological cross-talk between the mother and her embryo/fetus from conception till parturition. The disturbance of this communication may lead to miscarriages or pregnancy complications. This means miscarriages in 20% of clinically recognized pregnancies and various obstetrical syndromes in another 20%, which latter may severely impact maternal and fetal health and life. This has a critical importance regarding the ageing society and decreasing birth-rate in Hungary. Being recognized these days, the embryonic messages to the mother carried by the so-called “microvesicles”, miniature particles, mean a novel way of communication between mother and her offspring. The unique proteins of the embryo or placenta that we aim to investigate in the proposed study may be major messengers of these microvesicles. We aim to examine how and in what extent these embryonic/placental proteins may be associated with these microvesicles and regulate the maternal immune system in early pregnancy. According to our hypothesis, the investigations of embryonic/placental proteins in microvesicles detected in maternal blood may help in the prediction of miscarriages. The testing of microvesicles in maternal blood as an addition to routine prenatal tests may increase the sensitivity of risk assessment in pregnancy. The basic research of the interactions of microvesicles and the maternal immune system may support the identification of novel disease mechanisms and pharmaceutical targets. Altogether, our proposed study may have important diagnostic and therapeutic significance.

Huszonegy kéziratot publikáltunk (össz-IF: 104), szerkesztettünk egy „research topic”-ot a Frontiers in Immunology-ban, a terület egyik legjobban hivatkozott folyóiratában, amely összesen több mint 330.000 megtekintést kapott és e-könyvként is megjelent. Egy másik publikációval a Placenta címlapjára kerültünk. Számos fiziológiás és patofiziológiás jelátviteli útvonalat fedeztünk fel a szolubilis és/vagy extracelluláris vezikulákhoz kötött placenta/embrió fehérjékkel kapcsolatban. Legfontosabb eredményeink közé tartozik a méhlepény fejlődését és az anya-magzat immun-kölcsönhatásait szabályozó kulcsfontosságú jelátviteli útvonalak felfedezése, ezen jelátviteli útvonalakban - szolubilis és/vagy extracelluláris vezikulákhoz kapcsolt formában - központi szerepet játszó placenta fehérjék jellemzése, valamint ezen fehérjék és/vagy jelátviteli útvonalak szabályozási zavarának feltárása terhességi kórképekben, beleértve a preeclampsiát, vetéléseket és trofoblaszt betegségeket. Eredményeink hozzájárulhatnak e fehérjék hatásának/szerepének jobb megértéséhez az anyai-magzati immuntolerancia kialakításában és fenntartásában egészséges terhességekben, és abban, hogy egyensúlyhiányuk hogyan járulhat hozzá a súlyos terhességi kórképek patofiziológiás eseményeihez. Eredményeink a terhességi kórképek új biomarkereinek alkalmazásához is vezethetnek, amelyek javíthatják a patológiás terhességek jelenlegi szűrési módszereinek pontosságát.

We published 21 manuscripts (total-IF: 104), co-edited a highly cited research topic in Frontiers in Immunology, one of the best cited journals in the field, which received >330,000 views in total and was published as an e-Book, published top downloaded/cited papers in this prestigious journal, got on the cover of Placenta with another publication, and discovered several physiological and pathophysiological signaling events related to placenta- and embryo-derived soluble and extracellular vesicle-bound proteins. Our most important findings include the discovery of key signaling pathways which regulate placental development and maternal-fetal immune interactions in pregnancy, the characterization of placental proteins central to these signaling pathways in soluble and/or extracellular-vesicle mediated forms, and the dysregulation of these placental proteins and/or signaling pathways in pregnancy complications including preeclampsia, miscarriages and trophoblastic diseases. Our findings may contribute to the better understanding of the effects/roles of these placental proteins in the establishment and maintenance of maternal-fetal immune tolerance in healthy pregnancies and of how the imbalance of these proteins contributes to the pathophysiological events in severe pregnancy complications. These results may also lead to the implication of new biomarkers of pregnancy complications, which may improve the accuracy of the current screening methods for pathological pregnancies.
