A HMGB1 ( High Mobility Group Box 1) protein egy kromatinnal asszociált nukleáris fehérje, mely fontos szerepet játszik a transzkripcióban, DNS rekombinációban. Ugyanakkor extracelluláris formáját mint proinflammatorikus folyamatokat fenntartó késői cytokint fedezték fel újra. Cytokineket indukáló hatása elsősorban a RAGE (receptor for advanced glycation end products) receptoron elindított szignál utakon keresztül valósul meg. A RAGE számos más ligand receptora; fokozott expressziója figyelhető meg egyes neurodegeneratív betegségekben is. Kutatásainknak kettős célja 1.) A HMGB1 felszabadulásának vizsgálata, 2.) a RAGE genetikai polimorfizmusok tanulmányozása. Kidolgozzuk a HMGB1 koncentrációk meghatározásának optimális módszerét a SIRS -be tartozó szepszis szindrómában és acut pancreatitisben. A hazánkban úttörő jellegű klinikai tanulmányokat állatkísérletes modell vizsgálatával egészítjük ki , törpe sertésben létrehozott endotoxin shockban. A HMGB1 termelést in vitro sejtkulturákban is tanulmányozzuk, különböző bakteriális inducerek hatására. Vizsgáljuk , hogy hogyan csökkenthető a HMGB1 felszabadulás ethyl pyruváttal vagy pentoxifyllinnel. A RAGE gén polimorfizmus vizsgálatokat Huntington kórban, Parkinson kórban, és sclerosis multiplexben végezzük el, mivel ezen neurodegeneratív betegségekben is feltételezhető a fokozott RAGE expresszió szerepe.
Summary
The HMGB1 protein (High Mobility Group Box 1) is a non-histone chromatin component, which participates in DNA replication and transcription. It’s extracellular form has been rediscovered as a late cytokine , initiating proinflammatory processes. HMGB1 dependent signalling through RAGE (Receptor for Advanced Glycation End - products) leads to transducing cellular signals, which promote further cytokine production . As RAGE is a multiligand receptor, an increased expression of RAGE has been observed also in neurodegenerative disorders. Our research plan is focused in two directions: 1.) Examination of the HMGB1 release 2.) Investigation of RAGE gene polymorphisms in neurodegenerative diseases.. HMGB1 concentrations will be determined in sepsis and in acute pancreatitis, as in prominent representatives of SIRS These will be pioneer studies on this field in Hungary. Clinical studies will be supplemented with animal experiments, eg. endotoxin shock in mini pigs. Additionally, in vitro experiments will be performed to investigate the regulation of HMGB1 secretion from different cell types, to investigate the effects of different bacteria on the HMGB1 induction. .The inhibitory effects of ethyl pyruvate and pentoxifylline on HMGB-1 secreation will be tested in vivo and in vitro. The frequencies of the 28S and the -374 and -429 polymorphisms of RAGE will be investigated in patients with Huntington’s disease, Parkinson’s disease and with sclerosis multiplex

Megállapítottk, hogy a humán monocyták, különböző bakteriális inducerek-így M.bovis (BCG) hatására is képesek HMGB1( High mobility group box 1 protein) termelésére. HMGB1 súlyos acut pancreatitiseben egyrészt a gyulladásos folyamatok, másrészt a necrosis miatt emelkedik, és így, mint késői citokin , a patogenezis lényeges tényezője. A kinurénsav, mely in vitro gátolta a TNF-alfa termelést, szignifikánsan csökkenti a monocyták HMGB1 szekrécióját is. Állatkísérletekben igazoltuk a high mobility group box 1 (HMGB1) gyulladásos mediátor szint emelkedése és a szöveti oxigénfelvétel, valamint a GI mikrokeringés romlása közötti összefüggést, amely alapján egyrészt, a HMGB1 prognosztikai jelentőségű lehet, másrészt farmakológiai terápiás célpont lehet szepszisben. A RAGE (Receptor for Advanced Glycation End products) a HMGB-1 egyik receptorának egy nukleotidos polimorfizmusát vizsgálva szignifikáns eltérést találtunk a genotipusok vonatkozásában sclerosis multiplexben szenvedő betegek esetében. Az alarminok másik csoportját képezik a defenzinek. A humán defenzin beta DEFB4 három promoter polimorfizmusa rizikó tényezőként szerepelhet súlyos acut pancreatitisben. Azt is kimutattuk, hogy az inducibilis beta defenzin-2 copiaszám variációit is érdemes vizsgálni, ugyanis az alacsony (< 4 ) copia szám hajlamosít az acut pancreatitis súlyosabb formájára. Az inducibilis defenzin beta-2 csökkent kopia száma nagyobb százalékban figyelhető meg strokos betegekben is.

Bacterial inducers, such as M.bovis (BCG) are able to augment the production of High mobility group box 1 protein (HMGB-1) from monocytic cells. In clinical studies it was observed, that the circulating HMFB1 level was significantly higherin patients with severe acute pancreatitis. In severe acute pancreatitis HMGB1 secretion can be induced by inflammatory cytokines, and can be released from necrotic cells, and can be a reliable marker in the pathomechanism. Kynurenic acid, which attenuated not only the the TNF-alpha production from monocytic cells, but also diminished the HMGB1 secretion. In experimental animal sepsis model an increased HMGB1 level was determined together with the impairment of tissue oxigenisation, and the impairment of the microcirculation. HMGB1 has not only a prognostic value, but might be a tool of therapeutical intervention in sepsis. RAGE(Receptor for Advanced Glycation End products) is the receptor of HMGB1 Our results revealed an association between the promoter polymorphism of RAGE and sclerosis multiplex. Concerning the results of another alarmin, the defensin, an association between three promoter polymorphisms of defensin beta 1 (DEFB1) and acute pancreatitis was obsereved, similarly with copy number variation of DEFB4 and severe acute pancreatitis. The low copy numbers(<4) might be a risk factor in severe acute pancreatitis. Additionally the low copy number variation of DEFB4 is significantly frequent in ischaemic stroke.
