A melanomás megbetegedések okozzák a legtöbb, rosszindulatú tumorok által előidézett halálozást a 20-30 éves magyar nők körében. A tumoros megbetegedések előfordulása és az általuk előidézett halálozások száma világszerte növekvő tendenciát mutat. Az utóbbi évtizedben számos terápiát fejlesztettek ki rákbetegek kezelésére, a kemoterápiáktól kezdve a különböző alternatív gyógymódokon, őssejt kezeléseken, bakteriális terápiákon át az immunterápiákig. Az a rendkívül érdekes eredményünk, mely szerint a B16F1 sejtekkel előidézett tüdő metasztázisok minden egyes állatban visszafejlődtek kísérletes Chlamydia pneumoniae fertőzés hatására, vezettek munkahipotézisünkhöz:
Feltételezzük, hogy a C. pneumoniae ferőzés képes eltolni a makrofág polarizáció irányát a tumor ellenes M1 típus felé, elősegítve ezáltal hatékony tumoricid válasz kifejlődését. Korábbi adatainkra alapozva erőteljes M1 asszociált kemokin és citokin szekréciós profil kialakulását várjuk. Tisztázni kívánjuk a C. pneumoniae fertőzés hatására bekövetkező tumorregressziót, a kemoattraktáns hatású IL-24 jelenlétében. Részletes kísérletes állatmodell kidolgozását tervezzük úgy saját intézményi, mint kollaborációs felhasználásra, különböző gyógyszerfejlesztési kutatásokban. Várható eredményeink mélyebb betekintést nyújthatnak a tumorok mikrokörnyezetében lezajló makrofág polarizáció újabb, még ismeretlen, bakteriális fertőzésekkel összefüggő mechanizmusaiba.

Summary

The melanoma of the skin is associated with the highest death rate among malignant tumors of 20-30 years old women in Hungary (according to National Cancer Registry). The incidence of cancer and tumor related death shows a growing tendency worldwide. In the last decade numerous therapies were developed to cure cancer patients, including chemotherapies, supplementary alternative medications, stem cell therapies, bacterial therapies and immunotherapy. Our recent intriguing observations that experimental Chlamydia pneumoniae infection of mices followed by administration of B16F1 melanoma cells resulted in pronounced lung metastasis regression and growth suggested that:
C. pneumoniae infection can skew macrophage polarization toward the anti-cancer type M1 and elicitate an effective tumoricid responses. According to our previous data we are expecting an increased M1 profile-attached chemokine and cytokine secretion. Our objective is to clarify the role of C. pneumoniae infection in tumor regression and macrophage polarization, especially int he presence of the chemoattractant IL-24. We are planning to develop a detailed animal protocol for a melanoma metastatic model either for our utilization or for future collaboration in drug development studies. Our expected results may provide a deep insight to a novel regulation of macrophage polarization by a previously unrecognized mechanism, i.e. bacterial infection in tumor microenvironment.

A melanoma kimenetét erőteljesen meghatározza a betegség alatt lejátszódó immunfolyamat milyensége. Mivel az anti-tumorális mechanizmusok jelentős átfedést mutatnak az anti-bakteriális immunválaszokkal, ezért feltételezhető, hogy fertőzés generálásával vagy annak mimikálásával hasonló daganat ellenes hatás érhető el. A Chlamidophyla pneumoniae tüdőpatogént korábban nem vizsgálták ebből a szempontból. Kutatásaink során azt találtuk, hogy a kísérletes melanóma tüdő éttétek száma csökken, az állatok túlélése pedig megnövekszik hőinaktivált C. pneumoniae intranasalis adminisztrációja során. A jelenség elemzése során arra a következtetésre jutottunk, hogy a tumorok visszafejlődése javarészt az immunsejtek újra programozásának tudható be. Kísérleteink szorosan összekapcsolódnak egy IV. stádiumú melanomás beteg esetével. A páciens az alkalmazott BOLD kemoterápia első dózisa után iatrogen sepsis állapotába került. A metasztázisok visszafejlődtek, a beteg 2009 óta PET-CT megerősítetten daganat mentes. Tekintve a BOLD terápiára vonatkozó statisztikákat, kizártnak tekinthető, hogy a septikus folyamat adjuváns hatásának nem lett volna szerepe a beteg gyógyulásában. Mindezeket az eredményeket összegezve elmondhatjuk, hog ymind a beteg mind a kísérleti állatok gyógyulási folyamatában a C. pneumonie-nak szignifikáns szerepe lehet, amely új terápiás lehetőségek kifejlesztését eredményezheti.

Immune response seems to be a decisive factor in the outcome of melanoma malignum . It has been recognized for over 100 years that cancer patients might recover following bacterial infections. Our studies connected with a case of unexpected remission in a grade IV metastatic melanoma patient suffering sepsis during regular BOLD chemotherapy. The patient has been completely asymptomatic and PET/CT-verified metastasis free from 2009 to this date. Based on statistics, the complete recovery suggested an adjuvant effect of sepsis. Chlamidophyla pneumoniae (C. pneumoniae) has not previously been implicated in tumor regression. As a lung-specific intracellular pathogen, it seemed to be especially suitable for therapy of lung metastasis. Mice bearing melanoma lung metastases were treated with inactivated C. pneumoniae. The lung metastasis numbers significantly decreased, while the survival of the animals significantly increased. Studies of cytokine profile of the treated mice suggested that the tumor rejection might be caused by re-education of the tumor-infiltrating immune cells by the infection. Although the presence of C. pneumoniae was not initially investigated in the patient, it was retrospectively verified after recovery from the sepsis. Our results suggested that C. pneumoniae might be responsible for the unexpected tumor remission in humans and in experimental animals also, and attest the therapeutic potential of this approach.
