Itt írja le a kutatás fő célkitűzéseit a témában jártas szakember számára.
A hirtelen szívhalál világszerte az egyik vezető halálok. Mivel a szív repolarizációja több káliumáram finom egyensúlyán alapszik, ezen áramok kölcsönhatása rendkívül fontos a normál repolarizáció fenntartásához. Ez azt is jelenti, ha egyetlen áram működése megváltozik, akkor a többi áram ezt még kompenzálni tudja (repolarizációs tartalék, RT). Az RT szerepének pontosabb megismerése ezért rendkívüli fontosságú, hogy megérthessük az aritmiák keletkezésnek mechanizmusát, valamint a hirtelen szívhalál és az aritmiák kivédésének lehetőségét. Sok antiaritmiás szer hatékony antiaritmiás hatása kifejtésekor maga is hozzányúl az RT-hez (gyengíti), ezért paradox módon újabb aritmiák forrása lehet, amit proaritmiás mellékhatásnak neveznek. Jelenleg gyógyszerbiztonsági kérdés megállapítani, hogy egy új gyógyszerjelölt rendelkezik a proaritmiás mellékhatással, amit kísérletesen úgy lehet megállapítani, hogy megvizsgáljuk a szer hatását az RT-re. Egy új gyógyszerjelölt kifejlesztéshez ezres nagyságú vegyületkönyvtár ellenőrzésére is szükség lehet, ezért felvetődött, hogy számítógépes szimulációval ezt a mechanizmust fel lehetne gyorsítani, és ezáltal költséghatékonyabb is lehetne. A jelen projekt célja, egyesíteni az Oxfordi Egyetem Számítógépes Tudományok tanszékének a szakembereit az SZTE Farmakológiai és Farmakoterápiai Intézetének elektrofiziológusaival, hogy együtt új pontos humán és kutya akciós potenciál modelleket fejlesztenek ki. Ez a munka a természetes folytatása lenne annak a 2009-ben kezdődött sikeres nemzetközi ko-operációnak, amelynek során a két csoport már eredményes együtt dolgozott a „preDict” nevű EU-PF7 projektben

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.
A projekt fő célja a repolarizációs tartalék (RT) aritmiák patomechanizmusában játszott szerepének a számítógépes modellezése. A cél eléréshez két technikai szakértelemmel rendelkező tapasztalt nemzetközi csapat kutató fogott össze, hogy munkájuk során megpróbáljanak olyan új humán és kutya akciós potenciált modelleket hozzanak létre, amely alkalmas lehetne új vegyületek proaritmiás hatásainak előrejelzésére. A munka nehézsége kettős. Egyrészt még mindig nem teljesek ismereteink minden transzmembrán ionáramról, másrészt a humán kamrai és pitvari sejtekben működő áramok ismerete még ennél is alacsonyabb. A szív akciós potenciáljának számítógépes szimulációja még hiteles adatok ismeretében is igen komplex bonyolult számítógépes feladat.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!
A jelen projektjavaslat olyan eredmények elérését tűzte ki célul, amely elősegíti az új gyógyszerek biztonsági farmakológia munkáját. A tervezett HERG1/a és HERG1/B izoformák vizsgálata segíthet megismerni számos olyan váratlan proaritmiás gyógyszermelléhatás mechanizmust, amely jelenleg világszerte megnehezíti a biztonság farmakológiai vizsgálatokat. Új kísérletes adatokkal hitelesített megfelelő akciós potenciál számítógépes modellek kifejlesztése elősegítheti több új, eddig nem ismert ioncsatorna leírását, vagy ismereteink pontosítását eddig ismert ioncsatornákról. Ezek a modellek segíthetnek a gyógyszeriparnak is azon gyógyszerfejlesztési programban, amelyeket genetikailag módosított sejtvonalakon történő voltage-clamp technikákra alapoznak, mert csökkenthetnék az állatkísérleten alapuló kísérletek számát. A számítógépes modellezés speciális elméleti és gyakorlati elektrofiziológiai, de ugyanakkor magas számítógépes, bioinformatikai ismereteket igényel, amely ritkán adottak egy intézetben. A jelen projekt nemzetközi kooperációban kívánja áthidalni ezt a nehézséget. Az együttműködés során a szegedi és oxfordi intézetek PhD hallgatóinak ez kiváló lehetőség lesz, hogy a projekt végrehajtása során tanulmányi úton vehessenek rész a kooperációs partnernél, és ezáltal megismerkedhessenek a modellezéshez szükséges "idegen" technikával.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.
A hirtelen szívhalál a legtöbbször az aritmia következménye és jelentős mértékben hozzájárul a kardiovaszkuláris mortalitáshoz. Új hatékony antiaritmiás gyógyszerek tervezése idő és pénzigényes feladat. A kísérletek során modern egész állaton végzett illetve sejtszintű és molekuláris szintű vizsgálatokat végeznénk a hirtelen szívhalál mechanizmusának megismerése céljából. A már több éve közös kutatómunkát végző nemzetközi csapat komplex munkával olyan számítógépes modellt szeretne kifejleszteni, amely elősegíti új hatékony vegyületek kifejlesztését, valamint az olyan nem kardioaktív hatású gyógyszerjelölteket, amelyek nem rendelkeznek életet veszélyeztető mellékhatásokkal. Az eredmények kibővíthetik ismereteinket az életet veszélyeztető szívritmuszavarok mechanizmusairól, és segítenének egyaránt megismerni a kockázati tényezőket illetve ezen veszélyes betegségek megelőzési vagy terápiás lehetőségeit. Ezek a modellek segíthetnek a gyógyszeriparnak is azon gyógyszerfejlesztési programban, amelyeket genetikailag módosított sejtvonalakon történő voltage-clamp technikákra alapoznak, mert csökkenthetnék az állatkísérleten alapuló kísérletek számát.

Summary of the research and its aims for experts

Sudden cardiac death (SCD) is one of the leading causes of mortality worldwide. This includes that impairment of one single type of ion channel can be compensated by the function of the others (repolarization reserve), and it seems that the repolarization process and the electrophysiological function of the heart is stable, so the abnormality is often remained unnoticed. However, further hits in the cardiac ventricular channel system can lead to manifest and robust changes in repolarization, and provide a substrate for life threatening cardiac arrhythmias and SCD. Many drugs when exert their strong antiarrhythmic effect paradoxically may be a source of new arrhythmias, which is called proarrhythmic effect. Safety pharmacology prescribe that all new drug candidate should be tested for their proarrhythmic effect. This very time and money consuming, therefore, would be very helpful if by this process could be accelerated by computer simulation. The aim of the present research proposal is to combine the scientific knowledge of the experimental experts of the department of Pharmacology and Pharmacotherapy, University of Szeged, and of the computer experts of the Department of Computer Science, University of Szeged. This research work is the continuation of the fruitful collaboration started in 2009, when the same groups were core members of the EU-FP7 grant called „preDict”.

What is the major research question?

The major goal of the research project is to elucidate the role of repolarization reserve (ReR) in the pathophysiology of arrhythmias. To achieve this goal the proposal offers an cooperative effort of two international research groups in order to develop new reliable human and dog cardiac action potential models, which may be suitable to predict the proarhythmic potency of new drug candidates. The work is hampered by two reasons. In one hand the knowledge regarding the transmembrane atrial and ventricular currents especially in human are far to be complete. In the second hand even with the help of reliable current data the construction of a cardiac action potential model is complex computational work.

What is the significance of the research?
Describe the new perspectives opened by the results achieved, including the scientific basics of potential societal applications. Please describe the unique strengths of your proposal in comparison to your domestic and international competitors in the given field.
The proposed research proposal suggests getting new results which would help the safety pharmacology work when designing new drugs. Elucidating the physiology and pharmacology of the HERG1/A and 1/B ion channel isoforms would help to improve our understanding regarding unexpected proarrhythmic drug adverse effects, which is currently one of the major problem of the safety pharmacological investigations worldwide. The development of new cardiac ventricular action potential models with proper and systemic validation would provide proper future research to better identify possible so far unrecognized ion channels and change our existing view of the functions of the cardiac ion channels. These new models also would offer more focused drug development programs for the drug industry using voltage clamp results obtained in transfected cell lines to predict potential results and may help to avoid complicated animal welfare issues. The computational modelling work requires special theoretical and practical electrophysiology knowledge, but also complex computer and bioinformatical sciences skilfulness also, which rarely is present in one academic institute. The present research proposal aims to resolves this problem in a common international co-operation research networking. During implementing the research project the PHD students from both Szeged and Oxford groups will be offer the unique possibility to perform shirt scientific trips in the other group and to get familiarize with the different research technique for modelling work.

Summary and aims of the research for the public

Sudden cardiac death is most often a consequence of cardiac arrhythmias and contributes significantly to cardiovascular mortality. The development of new antiarrhythmic drugs is a time and money consuming process. In this project unified the research potency two academic international co-operation in order to develop a computational model, which helps and improve the developing of new antiarrhythmics, and also to develop new non cardiac drugs avoiding life threatening cardiac side effects. During their research they would apply different modern whole animal, cellular and molecular biological experimental models to gain useful information regarding the basic mechanism of sudden cardiac death. The obtained results can be expected to improve our existing knowledge of the mechanisms of the life threatening cardiac arrhythmias causing sudden cardiac death, help to identify those of having enhanced risk and improve our tools to prevent or cure this dangerous disease. These new models also would offer more focused drug development programs for the drug industry using voltage clamp results obtained in transfected cell lines to predict potential results and may help to avoid complicated animal welfare issues.

A jelen projektnek célja volt, hogy az Oxfordi Egyetem Számítógépes Tudományok tanszékének a munkatársaival olyan komplex in sillico akciós potenciál modellek kifejlesztése, amelynek segítségével meg tudjuk állapítani, hogy az adott ionáram gátló szerek rendelkeznek vagy sem proaritmiás mellékhatásokkal. A projekt során 3 altémában végeztünk kísérletes munkát, amely során jelentős eredményeket értünk el. 1. Kifejlesztettünk olyan új in sillico akciós potenciál modelleket, amely segítségével meg tudjuk állapítani: i) hogy egy vegyület gátló hatása miként változtatja meg a proaritmiás hatásért felelős repolarizáció instabilitását („beat-to-beat” variabilitás); ii) milyen szerepet játszik a nátrium-kálium pumpa a szív aritmogenézisében. 2. Megvizsgáltuk a nátrium-kalcium cseremechanizmus (NCX) élet és kórtani szerepét. Új saját fejlesztésű és szabadalmaztatott NCX gátlók segítségével kimutattuk, hogy az NCX gátlása jelentős antiaritmiás hatással rendelkezik. 3. Megvizsgáltuk milyen szerepet játszanak egyes csatorna géndefektusok olyan ismert kardiális hatással rendelkező kórképekben, mint az Andersen-Tawill és a hosszú QT5 (LQT5) szindrómák. Kidolgoztunk és szabadalmaztattunk egy új transzgenikus LQT5 nyúlmodellt. A projekthez kapcsolódó kísérletes munkát 12 teljes tudományos dolgozatban publikáltuk (kumulatív impakt faktor 36.29). A projekt során 3 PhD doktori értekezés és védés történt a kooperációs munka eredményeképpen.

The aim of the present project was to develop complex in silico action potential models, which may help to investigate the proarrhythmic potency of several ion channels blockers. The project was performed in an international cooperation with the researchers of the Department of Computer Sciences from Oxford University. The project consisted of 3 research subtopics. The following main results were obtained: 1. We have developed novel in silico action potential models, and by applying these models we have investigated that: i) how can an ion channel blocker modulate or modify the instability of the repolarization (beat-to-beat variability), which is known to be responsible for the proarrhythmic effect of the drugs; ii) what is the role of the sodium-potassium pump in the cardiac arrhythmogenesis. 2. We have investigated the physiological and pathophysiological role of the sodium- calcium exchanger (NCX). By applying some newly patented drugs developed by our group we demonstrated that the NCX-blocking effect has significant antiarrhythmic effect. 3. We have investigated the possible role of several ion channel gene mutations in some diseases having cardiac effects as the Andersen-Tawill and LongQT5 syndromes. The results obtained by implementing the present project have been published in 12 full lengths research papers having the cumulative impact factor of 36.29, and three PhD students defended their PhD degrees by presenting the research connected to this project.
