A rheumatoid arthritis (RA) krónikus autoimmun betegség, melyre jellemző a proinflammatorikus citokinek, többek között a tumor necrosis factor (TNF) és az interleukin 17 (IL-17) fokozott termelődése. A T limfociták központi szerepet játszanak a RA pathogenezisében. A nitrogén monoxid (NO) egy rövid féléletidejű, számos szabályozó szabályozó funkciót betöltő molekula, immunmoduláns hatása jól ismert. Korábbi adataink szerint NO szabályozza a fiziológiás T sejt aktivációt és hozzájárul szisztémás autoimmun betegségek kialakulásához is. Legújabb adataink szerint RA-es betegek T limfocitáinak NO termelése jelentősen fokozott. Vizsgálataink szerint továbbá NO szabályozza a TNF hatásait, a T limfocita aktivációt és a Th1/Th2 differenciálódást, mindezek alapján központi szerepe feltételezhető a RA pathogenezisében.

Az utóbbi években azonosított interleukin 17 termelő CD4+ T limfociták (Th17 sejtek) és a CD4/CD25+ természetes reguláló T sejtek (nTreg) jelentősen hozzájárultak a RA pathomechanizmusának megértéséhez. A Th17 és az nTreg sejtek potenciális terápiás célpontok RA-ben. NO termelés jelentősen fokozott RA-ben, ugyanakkor a NO Th17 és Treg sejtek differenciálódására és szabályozására kifejtett hatása nem ismert. Szintén nem ismert részleteiben, hogyan szabályozza a NO proinflammatorikus citokinek hatásait. Tervezett munkánk során vizsgálni fogjuk NO szerepét RA-ben. Jelentősen fokozott NO termelés hipotézisünk szerint hozzájárulhat a T limfocita jelátviteli folyamatok, differenciálódás zavarához és a megváltozott citokin termeléshez RA-ben.

Rheumatoid arthritis (RA) is a common chronic autoimmune disease, characterized by the overproduction of proinflammatory cytokines such as tumor necrosis factor (TNF) and interleukin-17 (IL-17). The central role of T lymphocytes in the pathogenesis of RA is widely accepted. A strong immune regulatory role of nitric oxide (NO), a pleiotropic short-lived free radical, has also long been recognized. Our previous data indicate that NO regulates physiological T cell activation and may contribute to the pathogenesis of systemic autoimmune diseases as well. According to our recent data, the NO production of the T lymphocytes of RA patients is markedly increased. Furthermore, we have also showed that NO modulates the effects of TNF, regulates T cell responsiveness and Th1/Th2 differentiation, strongly supporting the principal role of NO in the regulation of immune response in RA.

The recent discovery of the interleukin IL-17 secreting CD4+ T cells, (Th17 cells) and the naturally occurring immunosuppressive regulatory CD4/CD25+ T cells (nTreg), has a major impact on our understanding about the pathomechanism of RA. Both TH17 and nTreg cells are potential therapeutic targets. Although NO is overproduced by T cells in RA, the effects of NO on the development or maintenance of Th17 and T reg cells has not been investigated yet. Furthermore, the precise role of NO in regulating the effects of proinflammatory cytokines is not known in details. Our proposed work will uncover the important immune regulatory role of NO in the pathogenesis of RA. We suggest that increased levels of NO in T cells may contribute to perturbations of cytokine production, T cell signal transduction and differentiation, characteristic of patients with RA.

Humán T-lymphocyták TNF kezelése (15-40 ng/ml) szelektíven és dózisfüggő módon csökkenti a CD3 ζ-lánc kifejeződését (p<0,05) és gátolja az aktiváció indukálta IL-2 termelést (p<0,01). A TNF kezelés fokozza az Src-like adaptor protein (SLAP) kifejeződését és a CD3 ζ-lánc-SLAP kolokalizációt (p<0,05 és p<0,01). SLAP siRNS gátolja a TNF által indukált ζ-lánc csökkenést. Rheumatoid arthritises (RA) betegek CD4 T-sejtjeinek SLAP expressziója több mind kétszerese az egészséges kontrollokénak (p<0,05). TNF kezelés nem befolyásolja a TNF-blokkoló biológiai terápiával kezelt betegek CD4-lymphocytáinak SLAP kifejeződését. A anti-CD3/CD28 kezelés fokozza az IL-17 termelést, de nem befolyásolja a RORγt kifejeződést. Az anti IL-4 és az anti-IFNγ antitest kezelés fokozza az anti-CD3/CD28, TGFβ, IL-6, és IL-1 kezelés által indukált RORγt expressziót. Az IL-17 termelés hasonló volt a differenciálódás ötödik és tizedik napján, ugyanakkor az IL-22 termelést gátolta a TGFβ, IL-6, IL-1 citokin és anti-IL-4, anti-IFNγ blokkoló antitest kezelés. RA-s betegek TH-17 sejtjei az egészséges kontrollokénál több IL-17 et termelnek. Proinflammatorikus citokinek (IL-1, IL6, IL-17, IL-18), nitrogén monoxid (NO) és NO szintetáz gátló LNMMA nem befolyásolta az iTreg differenciálódást és funkciót.

TNF treatment of human T lymphocytes (15-40 ng/ml) selectively downregulates CD3 ζ-chain expression in a dose-dependent manner (p < 0.05) and decreases activation-induced IL-2 expression (p < 0.01). Both Src-like adaptor protein (SLAP) expression and the colocalization of SLAP with CD3 ζ-chain was enhanced by TNF treatment (p < 0.05 and p < 0.01, respectively), whereas TNF-induced ζ-chain downregulation was inhibited by gene silencing of SLAP with small interfering RNA. SLAP levels of the CD4+ T-lymphocytes isolated from patients with rheumatoid arthritis (RA) were more than 2-fold higher than that of the healthy donors' (p < 0.05); moreover, TNF treatment did not alter the SLAP expression of the CD4+ cells of anti-TNF therapy-treated patients. Anti-CD3/CD28 treatment increased the IL-17 production, but did not alter the RORγt expression. The anti-IL-4 and anti-IFNγ antibody treatment significantly increased the anti-CD3/CD28, TGFβ, IL-6, and IL-1 induced RORγt expression. The IL-17 production was similar in the fifth and tenth day of the treatments by contrast the IL-22 production was greatly reduced by TGFβ, IL-6, IL-1, cytokines, anti-IL-4 and anti-IFNγ blocking antibodies. The differentiated TH-17 cells of patients with RA produce more IL-17 than that of the healthy donor’s. Proinflammatory cytokines (IL-1, IL6, IL-17, IL-18), nitric oxide (NO) or NO synthase inhibitor LNMMA didn’t alter the iTreg differentiation or function.
