A korai és késôi utódepolarizációs folyamatok (EAD, DAD) olyan kóros membránpotenciál-változások, amelyek gyakran normális szívciklusok közé ékelôdve jelennek meg. Mindkét típusú utódepolarizáció életveszélyes szívritmuszavarok forrása lehet, mivel kórosan ingerelhetik a szomszédos szívizomsejteket. A területen eddig végzett intenzív tudományos kutatás ellenére is számos nyitott kérdés és ellentmondás található a konkrét utódepolarizációs mechanizmusok (pl. az aktuális trigger-mechanizmus, a töltéshordozó, valamint a frekvencia-függô sajátságok) vonatkozásában. Jelen kísérleti projekt céljául tûztük ki egy olyan átfogó utódepolarizációs modell kidolgozását, amely képes kiküszöbölni a jelenlegi elméletekben rejlô ellentmondásokat. Ezt úgy kívánjuk megvalósítani, hogy szisztematikusan megvizsgáljuk és összehasonlítjuk a különbözô farmakológiai módszerekkel kiváltható utódepolarizációk jellemzô sajátságait és meghatározzuk a legalapvetôbbnek tartott tényezôk (akciós potenciál idôtartam, nettó membránáram, intracelluláris kalcium-koncentráció, ingerlési frekvencia) szerepét az utódepolarizációk kialakulásában. Feltérképezzük a töltéshordozóként potenciálisan szóba jövô ionáramok (L-típusú Ca-áram, késôi Na-áram, Cl-áram, Na/Ca-csereáram) részvételét, valamint tanulmányozzuk az utódepolarizációk és az akciós potenciál idôtartam "beat-to-beat " variációinak kapcsolatát a különbözô eredetû utódepolarizációk esetén. Mindezt egy olyan emlôsfajon, a kutyán, kívánjuk megvalósítani, amelynek szívelektrofiziológiai sajátságai a legjobban hasonlítanak a humán szív tulajdonságaihoz. Várható eredményeink - reményeink szerint - hozzásegítenek az utódepolarizációs mechanizmusokon alapuló ritmuszavarok jobb megértéséhez és ezáltal ésszerûbb terápiás stratégiák ill. gyógyszerek kifejlesztéséhez.
Summary
Early and delayed afterdepolarizations (EADs and DADs, respectively) are pathological membrane potential changes usually associated with normally driven cardiac action potentials. Both are sources of life threatening cardiac arrhythmias, since are able to trigger extra action potentials in the neighboring cardiac cells. In spite of the intensive research of this field, several questions remained open or controversial, including the underlying triggering mechanisms, ionic nature and frequency-dependent properties of afterdepolarizations. This project aims to develop a comprehensive theory of afterdepolarizations, by applying systematical parametric description of afterdepolarizations induced by various pharmacological interventions. The role of the most important electrophysiological parameters related to cardiac action potentials (action potential duration, net transmembrane ion current, intracellular calcium concentration, and stimulation frequency) will be examined. Contribution of various ion currents as potential charge carriers (L-type Ca current, late Na current, Cl current, and Na/Ca exchange current) will be determined. Relation of beat-to-beat variation of action potential duration to afterdepolarizations of various origins will be studied. This research will be performed in a mammalian species, the dog, which displays cardiac electrophysiological characteristics most resembling those of the human heart. The results to be obtained according to this proposal may yield a better understanding of the mechanisms underlying afterdepolarization-based cardiac arrhythmias, and consequently to develop more adequate and more rational therapeutic strategies.

This project was designed to study some basic aspects of arrhythmia generation including the properties of ion currents involved in arrhythmia-genesis, the activation of the isoproterenol-induced currents which represent the normal way of responding to physical or psychical challenges, consequences of changes in intracellular calcium concentration which normally occurs during each cardiac cycle but is also an important proarrhythmic factor, and the role of beat-to-beat variability of action potential duration which is considered as the best proarrhythmic indicator. Beyond describing many important new mechanisms, results and details related to arrhythmia-genesis, our main conclusion is that the studied proarrhytmic mechanisms are so complex and specific, that there is no room for unifying theories. Accordingly, each type of afterdepolarizations is governed by its own specific mechanism requiring specific handling.

Kutatásaink során alapvetô arrhythmia mechanizmusokat vizsgáltunk, beleértve az arrhythmiák kialakításában vezetô szerepet játszó ionáramok sajátságait, az izoproterenol által aktivált áramok tulajdonságait (mely áramok a fizkai vagy psychés terhelésre adott válaszreakciók során mindig megjelennek), az intracelluláris calcium koncentráció változások következményeit (melyek minden szívverés során jelen vannak - ugyanakkor fontos proarrhythmiás mechanismusokat rejtenek) és a kardiális akciós potenciál idôtartamának “beat-to-beat” változásait, amelyet jelenleg a legmegbízhatóbb proarrhythmia indikátornak tartunk. Túlmenôen azon, hogy kutatásaink számos fontos új eredménnyel és mechanizmus feltárásával gazdagították ismereteinket a szívarrhythmiák vonatkozásában, fô konklúziónk abban összegezhetô, hogy nem lehet a különbözô mechanizmusok szerint kialakuló utódepolarizációkat egységes rendszer szerint kezelni azok egyedien specifikus sajátságaik miatt.
