A védőoltások nagyon hatékonyak a járványok megakadályozásában, megszámlálhatatlan életet mentettek meg mióta a gyermekkori védőoltások elterjedté váltak világszerte. A sikeres védőoltás humorális immunválaszt indukál és a termelődő elleanyagok a komplementrendszerrel együtt védenek a behatoló kórokozóval szemben. Ideális esetben minden egyes beoltott személyben ellenőrizni kellene, hogy hatékony volt-e az immunizálás, amit azonban gazdaságossági szempontok miatt sajnos nem végeznek el. A microarray technika ideális lenne ezen mérések költség- és időhatékony végrehajtására. Ezen módszer hatékonysága az antigén specifikus ellenanyagok mérésében már széleskörűen bizonyított. Általában csak IgG izotípusú ellenanyagválaszt vizsgálnak és csak viszonylag kevés kutatócsoport tesztel páhuzamosan több izotípust. A projektem fő célja a komplement aktiváció mérésére alkalmas technika alkalmazása a védőoltások hatékonyságának vizsgálatára. Ezt a technikát munkacsoportunk fejlesztette ki az antigén specifikus ellenyagok szintjének és komplementaktiváló képességének párhuzamos vizsgálatára. Munkacsoportunk széleskörűen alkalmazta ezt a technikát autoimmun betegségek jellemzésére; a jelen projekt keretében a módszer hatékonyságát szeretném megvizsgálni fertőző betegségek immunizálásának területén. Ezen technika segítségével feltárható és vizsgálható a védőoltásban részesült egyének komplex immunprofilja.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Teljes vagy alegység vakcinával történő sikeres aktív immunizálás immunválaszt vált ki. A termelődött ellenanyagok mennyisége és minősége határozza meg az oltás sikerét. Az ellenanyagok izotípusa, affinitása és glikoziláltsági foka szabja meg az általa kiváltható effektor hatások körét. A komplementaktiváció az ellenanyagok által kiváltott egyik nagyon fontos effektor funkció. A komplementrendszer lényeges szerepet tölt be a kórokozókkal szembeni védekezésben; részt vesz gyulladás indukálásában, pathogén lizálásában, opszonizációjában és neutralizációjában. A projekt fő kérdése, hogy mi az összefüggés az oltóanyagok által indukált különböző izotípusú ellenanyagok mennyisége és komplement aktiváló képessége között.

Mi a kutatás jelentősége?

A védőoltás által kellett humorális immunválasz nagyon fontos része a immunizálásnak. Az immunválasz minősége és mennyisége határozza meg, hogy sikeres volt-e az immunizálás. Így a termelődött ellenanyagok mennyiségének mérésén túl azok minőségének – izotípusának, általuk keltett effektor funkció természetének - meghatározása is fontos. A projeket időtartama alatt szeretném bizonyítani, hogy a chipen történő komplementrendszer aktiváció mérés egy kitűnő módszer a védőoltás hatékonyságának részletes jellemzésére. Ez a technika nem csak az antigén specifikus ellenanyagválasz erősségét – különböző izotípusokra párhuzamosan- hanem az általuk kiváltott komplement aktiváció mértékét is meghatározza. A projekt további bizonyítékul szolgálhat a chipen történő komplement aktiváció technikájának előnyeiről. Ez a módszer sokat segíthet új oltóanyagok fejlesztésében, mivel a vakcinajelöltek kompex hatása vizsgálható vele, így segít kiválasztani a leghatékonyabb vakcina összetevőt. Microarray technikával nagyon sok párhuzamos mérés végezhető el, így számos védőoltás hatása ellenőrizhető költséghatékony módon. A technika ezen tulajdonságai ideálissá teszik olyan nagy mintaszámú mérések elvégzésésre, melyek már reprezentálni tudják a teljes populációt. Hatékony védelme az adott populációnak a fertőző betegségekkel szemben mindig egy nagyon fontos megválaszolandó kérdés. A védelem elvesztése, vagy a nem megfelelő immunizálás rejtve maradhat. Ha egy immunizált személy olyan környezetben él ahol nem található meg az adott kórokozó, akkor a védelem hiánya nem derül ki. A turizmus növekedésével megnő egyben annak a lehetősége, hogy az utazók olyan területeket látogatnak meg ahol az illető kórokozó még mindig őshonos. Ezen utazások során előfordulhat, hogy az utazó megfertőződése után elszállítja a kórokozót olyan területre ahonnan korában már kiírtották. Így mindig készülni kell arra, hogy rég elfeledett betegségek újra megjelennek, ezért fontos ismerni egy adott populáció immunológiai állapotát. A projektem során nyert eredmények képezhetik egy későbbi populáció szintű kísérletsorozat alapjait, mely felméri a magyar populáció betegségek iránti fogékonyságát, megmutatva ezzel a vakcinációs protokoljaink esetleges gyenge pontjait.

A védőoltások nagyon hatékonyak járványok megelőzésében. Nem feltétlenül derül ki azonban, ha az oltás hatására nem alakul ki megfelelő védelem vagy ha ez az idő múlásával eltűnik. Ha egy védőoltásban részesült személy olyan környezetben él ahol nem található meg az adott kórokozó, akkor a betegség iránti fogékonysága rejtve marad. A turizmus növekedésével megnő egyben annak a lehetősége, hogy az utazók olyan területeket látogatnak meg ahol az illető kórokozó még mindig őshonos. A megfertőződött turista elszállíthatja a kórokozót olyan területre ahonnan korában már kiírtották. Fontos felderíteni az adott területen élő közösségek betegségek iránti fogékonyságát, hogy felkészüljünk rég elfeledett betegségek ismételt megjelenésére. A sikeres védőoltás hatására az ember szervezetében speciális molekulák, ellenanyagok termelődnek. Ezen ellenanyagok nagyon erősen kötődnek a behatoló kórokozóhoz, megakadályozva ezzel szaporodását. Képesek a vérben található molekulák egy csoportját, a komplementrendszert aktiválni. A komplementrendszer elősegíti a gyulladást, közvetlenül elpusztíthatja vagy elkülönítheti a kórokozót. A munkám során apró pontokban kórokozókat nyomtatok speciális üveg lemezekre –chipekre- és vizsgálom a vérben található kórokozókat felismerő ellenanyagok mennyiségét és komplementrendszert aktiváló képességét, felmérve így a védőoltások hatékonyságát. A mérés során, láthatóvá tehetők azok a pontok melyek olyan kórokozót tartalmaznak ami ellen sikeres védelem alakult ki. Ezen munka eredményei biztos alapot nyújthatnak egy későbbi, nagy szabású kísérlet sorozatnak amelyben a magyarországon alkalmazott védőoltási séma hatékonyságának felmérése a cél.

Vaccination is a very efficient way to prevent outbreak of epidemics and since childhood vaccination become common procedure worldwide, uncountable life have been saved. A successful active immunization would induce humoral immune response and the produced antibodies together with the complement system protect against invading pathogens. Ideally all immunized person should be tested whether effective protection has developed, but for economical reasons this is not accomplished. Microarrays are ideal technique for multiplex, cost and time efficient measurements. Applicability of this technique on assessment the antigen specific antibody in serum has been widely proved. Usually only IgG antibodies are measured with this technique and few groups measure several isotypes of antibodies simultaneously. Key goal of this project is the application of the on-chip complement measurement method for testing the result of immunization. This method, which was developed by our group, lets the simultaneous assessment of the level of antigen specific antibodies with various isotypes along with the measurement of their complement activating properties. Our group extensively used this technique for characterization of autoimmune diseases; during this project I will test its applicability in the field of immunization against infectous agents. Using this method a very complex immune profile of the immunized person is depictable and analyzable.

What is the major research question?

A successful active immunization with the whole or subunit of a pathogen will induce immune response. The amount and type of the produced antibody will determine the success of vaccination. Isotype, affinity and glycosylation of antibodies define the circle of effector functions that can be initiated by the given antibody. An important effector function of antibodies is their complement activating property. Complement system is vital in the protection against pathogen; it takes part in induction of inflammation, pathogen lyses, opsonization and neutralization. The question that this project addresses is what the connection between the levels of different vaccine induced antibody isotypes and they effectiveness in activation of the complement system is?

What is the significance of the research?

Vaccination induced humoral immune response is crucial component of immunization. The immune system of the immunized recipient responds to the components of vaccine. The quality and quantity of the immune response will determine if suitable protection is occurred. So beyond measurement the level of induced antibody, also its quality - isotype, nature of antibody induced effector functions - is important. During this project I would like to prove that on-chip complement activation assay is an excellent method for detailed characterization the effectiveness of human vaccines. This method measures not only the amount of antigen specific antibodies - various isotypes simultaneously - but also their effector function in terms of the complement activation. Results of this project could give further prove of the advantage of on-chip complement activation test. This technique could be a versatile tool during development of new vaccines, as it enables assessment the complex effect of vaccine candidates and help to choose the most effective part of a vaccine formula. Microarray technique carries out numerous measurements in small so various immunizations can be validated simultaneously in a cost efficient way. These properties of the technique take it ideal for assessment such a numerous samples that can represent the whole population. The effective protection of the population against various infections is always a key question that has to be answered. The lost of protection or inadequate immunization is not necessary rised into view. If the immunized person live in an environment that absent the given pathogen then the lacking immunity is unrevealed. Nowadays the increasing volume of tourism raises the possibility that travelers take a trip into regions of the world where the given pathogen is yet endemic,and then deliver pathogen to area where it was earlier eradicated. Therefore we always have to be prepared that pathogens reappear, so it is advantageous to know the immunological status of the given population. Results of this project can be basement of a later population scale experiment that unravels immunological status of Hungarian population, showing the weak points of our current vaccination regime.

Nowadays the increasing volume of tourism raises the possibility that travelers take a trip into regions of the world where the given pathogen is yet endemic and then can deliver pathogen also into an area where it was earlier eradicated. Therefore we always have to be prepared that pathogens reappear, so it is advantageous to know the immunological status of the given population. The result of vaccination is the production of specific molecule called antibody by the human organism. Antibody can bind strongly to the invading microbe and protect against the infection. Antibody can activate a group of molecules in blood, called complement system; those proteins induce inflammation, destroy invading microbes and enwrap then to be separated. Antibodies and complement system are important in protection against pathogens. I will study the effectiveness of immunization by measuring on a chip both the amount of pathogen specific antibodies and their complement activating property. The chip is a slide where various pathogens or their fragments are printed in less than a millimeter wide spots. After the blood sample from an immunized person is contacted with slide, then spots can be visualized containing a pathogen against which the patient is successfully immunized. This chip can be the basic of a future project that survey effectiveness of our immunization protocols.

Meghatároztuk 95 egyén komplex funkcionális immunprofilját fehérje microarray technika segítségével azáltal, hogy megmértük a kórokozókat felismerő IgM, IgG és IgA izotípusú ellenanyagok szintjét, valamint a képződő immmunkomplexek által indukált C3 és C4 fragmentum kötődés mértékét. Így nem csak az antigén specifikus ellenanyagok mennyiségéről, de az általuk elindított komplement aktiváció mértékéről is vannak ismereteink. A kereskedelemben kapható, diagnosztikában is alkalmazott ELISA kitekkel validáltuk a microarray technikával nyert eredményeinket. ELISA mérések alapján a vizsgálatba bevont egyéneknél a tesztelt oltások hatékonyság alapján három csoportba oszthatók: magas IgG szeropozitivitás (>90%) a rózsahimlő, diftéria és tetanusz, közepes (~60%) mumpsz, kanyaró és hepatitis B vírus, valamint alacsony szeropozitivitás (3%) a szamárköhögés toxinja ellen. Erős korrelációt (r~0.8) tapasztaltunk az ELISA és microarray mérések között szamárköhögés, tetanusz és diftéria IgG meghatározás során, ami bizonyítja az általunk beállított multiplex rendszer hatékonyságát. A kórokozó specifikus komplement aktiváció mérések során kiderült, hogy a komplement aktiváció nem feltétlenül mutat szoros korrelációt az ellenanyagszinttel minden általunk vizsgált kórokozónál, így az antigén által indukált komplement aktiváció mérése extra információval szolgálhat az immunizálás eredményességének megértésében.
Results in English
We determined the complex functional immune profiles of 95 immunized individuals on protein microarray platform by measuring pathogen specific IgM, IgG, IgA levels and the immune complex induced C3 and C4 fragment deposition. In this way information was gained not only about the level of antigen specific antibody but also about their complement activating properties. Commercially available diagnostic ELISA kits were applied to validate our microarray measurements and these results revealed that vaccination efficiency against the tested pathogens in our cohort can be sorted in three groups based on percentage of IgG seropositivity: high percentage of protected individuals (above 90%) for rubella, diphtheria, tetanus; medium percentage (around 60%) for mumps, measles, hepatitis B virus; low percentage for pertussis toxin (3%). There was strong correlation (r~0.8) between ELISA and microarray measurement of pertussis, tetanus and diphtheria pathogen specific IgG antibody levels that proved the good performance of this multiplex technique. Measurement of pathogen induced complement activation revealed that complement activation does not necessarily correlate strongly with the antibody levels by each pathogen so measurement of antigen specific complement activation can give extra information for understanding protective effect of immunization.
