Az endokrin orbitopathia (EO) az orbiták autoimmun betegsége, mely a Graves-Basedow kóros betegek 10-30%-nál alakul ki. Az exophthalmus, és valamennyi társuló tünet oka az orbita kötőszövet tömegének megnövekedése, a fibroblastok proliferációja és fokozott glycosaminoglycan (GAG) termelése, és a külső szemizmok oedemás duzzanata. Az orbita kötőszövetében lymphocytás infiltráció van jelen. Feltételezzük, hogy az orbita-fibroblastokra ható pathogenetikai tényezők endokrin orbitopathiában endokrin eredetűek (hormonhatások), immunológiaiak (cytokinek) és lokális tényezők (nyomás) lehetnek. Az EO betegek orbita decompressios műtéte során nyert mintákból készített fibroblast kultúrákat a beteg orbitájában találhatóhoz hasonló hatásoknak tesszük ki in vitro: bovin TSH, rekombináns humán TSH, TSHα, TSHβ, LH, hCG, IL-1β, IFNγ, magasabb CO2, alacsonyabb oxigén koncentráció, a légkörinél magasabb légnyomás. A fibroblastok válaszát a glycosaminoglycan szintézis és a PAI-1 szekréció mérésével állapítjuk meg.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Hipotézisünk szerint az EO patogenezisében három, részben független, de egyidejűleg mindig jelenlévő tényező játszik szerepet: 1. az autoimmun folyamat (CD4+, CD8+, CD20+ lymphocyták valamint macrophagok és az általuk termelt cytokinek); 2. humorális faktorok, melyek a vér- és nyirokkeringéssel jutnak az orbitába (TSH receptor antitestek, TSH, T4); 3. lokális tényezők, melyek csak az orbitára jellemzőek, a szervezet más helyein található kötőszövetre nem (fokozott intraorbitális nyomás, csökkent vénás keringés következményes hypoxiával, a fibroblastok által termelt feltételezett paracrin faktorok). Mindhárom tényezőre szükség van az EO kialakulásához és fennmaradásához, ezt az is bizonyítja, hogy bármelyikük kedvező irányú befolyásolása a tünetek javulásához vezet. Jelen munkánkkal tisztázni szeretnénk egyes hormonális, immunológiai és lokális tényezők szerepét a betegség pathogenezisében. Előzetes eredményeink arra utalnak, hogy a fibroblast eredetű PAI-1 paracrin regulátora az orbita-kötőszövetnek.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

Az EO elsősorban 30-50 éves nőket érintő betegség (nő:férfi arány 3:1). A legsúlyosabb esetekben vakság is kialakulhat egyik vagy mindkét szemen. A betegek életminősége mérhetően romlik, az arc előnytelen megváltozása családi konfliktusokhoz, a munkahely elvesztéséhez vezethet. A betegség pathogenezise csak részben tisztázott. A betegek orbitájában számos cytokin jelenlétét kimutatták az elmúlt 16 évben, ennek ellenére ma sincs olyan hatásos kezelési eljárás, amely teljes gyógyuláshoz vezetne. Az általunk vizsgált hormonok, cytokinek és lokális körülmények hatással lehetnek a fibroblastok glycosaminoglycan és PAI-1 szintézisére. A fibroblastok hormonális és immuntényezőkre, valamint hypoxiára és nyomásra adott válaszának tanulmányozása elősegítheti az orbitában zajló pathogenetikai folyamat megértését, és új terápiás beavatkozások lehetőségét teremtheti meg.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

Az endokrin orbitopathia a pajzsmirigy túlműködéséhez társuló szembetegség, mely súlyos esetben kezelés nélkül vaksághoz is vezethet. A szemgolyó mögötti kötőszövet felszaporodik, a szemmozgató izmok megvastagodnak. A betegség oka csak részben tisztázott, így a kezelés lehetőségei is korlátozottak. A kutatás a kóros mechanizmusok megismerésére irányul, ami lehetőséget biztosíthat hatásosabb beavatkozásra a betegség megelőzése, gyógyulása érdekében. A legsúlyosabb esetekben műtétre van szükség a szemüreg nyomásának csökkentésére. Kísérleteinket az ilyen műtétek során eltávolított kötőszövetben végezzük a kiváltó okként szóbajövő hormonok, immunológiai tényezők és egyéb faktorok hatásának laboratóriumi tanulmányozásával.

Graves' orbitopathy (GO) is an autoimmune disease of retroocular orbital components which accompanies Graves' hyperthyroidism in 10-30 % of the cases. Eye protrusion in patients with GO is the result of retroocular connective tissue expansion and extraocular muscle swelling. Mononuclear cell infiltration of the orbits results in enhanced glycosaminoglycan (GAG) synthesis by fibroblasts. The increased orbital volume is the proximate cause of many of the clinical manifestations of GO. We assume that orbital fibroblasts in Graves' orbitopathy are under distinct types of regulation: endocrine (hormonal), immune (cytokines), and pressure. The patients' orbital fibroblasts, obtained during orbital decompression surgeries, will be exposed to stimuli mimicking the microenvironment in the orbit. The effect of the following will be tested in vitro: TSH bovine, TSH recombinant human, TSHα, TSHβ, LH, hCG, IL-1β, IFNγ. The effect of higher CO2, lower O2, and higher than atmospheric pressure mimicking local orbital GO environment will also be tested. Fibroblast responses will be estimated by the measurement of glycosaminoglycan and PAI-I synthesis.

What is the major research question?
Describe here briefly the problem to be solved by the research, the starting hypothesis, and the questions addressed by the experiments.

According to our hypothesis, at least three different basic mechanisms are concomitantly present in the involved orbits: 1. Presence of immune cells (CD4+, CD8+, CD20+ cells and macrophages) and their cytokine production; 2. Humoral factors delivered by the orbital blood flow and/or lymphatics of the orbits (TSH receptor antibodies and/or TSH and T4); 3. Local factors which are unique to the orbit compared to connective tissue at other sites (increased intraorbital pressure due to bony orbit anatomy, decreased venous outflow with consequent hypoxia, regulatory factors secreted by fibroblasts themselves). All three are needed for the perpetuation of the orbital process in GO, as therapeutic interventions interfering with any one of them are comparably effective in alleviating the symptoms. In the present series of investigations, we aim to clarify the putative effect of hormonal, immune and local orbital factors, including PAI-1 on orbital fibroblasts will be tested. We assume, based on our preliminary data, that PAI-1 is a regulator of tissue remodeling in the orbit, originating from the fibroblasts.

What is the significance of the research?

GO is a disfiguring disease of women (female:male ratio 3:1) in the age range of 30-50 years. In severe cases, it may lead to sight loss on one or both eyes. Quality of life of the patients is diminished by family and workplace conflicts, as their facial appearance changes profoundly even in less severe cases. Although a series of different cytokines has been found to be present in the orbits of GO patients, details of the pathogenesis are rather obscure. At present, no effective therapy is known which could result in complete cure of GO. Characterization of fibroblast responses to hormonal and immune mediators, as well as local factors as pressure and hypoxia may facilitate our understanding of the orbital process and finding new therapeutic approaches. Some of the hormones, cytokines and conditions tested may influence the glycosaminoglycan and PAI-I production of fibroblasts and we may identify putative therapeutic targets.

Graves’ orbitopathy (GO), which may lead to sight loss in severe cases, is an eye disease accompanied by overproduction of thyroid hormones. The connective tissue behind the eye expands, and the eye muscles become swollen. The cause of the disease has not been completely clarified, thus treatment options are limited. In our research we aim to clarify the pathomechanism of GO, which may provide more effective treatment for prevention and cure of the disease. Orbital decompression surgery is necessary in some patients with more severe and active disease. In the experiments orbital connective tissue samples collected during surgeries will be exposed to hormonal, immune and other factors with putative role in the pathogenesis of GO.

A projekt keretében olyan modellt hoztunk létre, melynek segítségével in vitro vizsgálható az orbita fibroblastok viselkedése, és annak változása az endokrin orbitopathia (EOP) pathomechanizmusában szerepet játszó faktorok hatására, valamint összehasonlíthatóak az EOP és nem-EOP orbitából származó fibroblastok. A sejtvonalak növekvő sejtdenzitáson történő vizsgálatával az EOP lefolyása során változó proliferációs ráta hatását modelleztük. Megfigyeltük, hogy az egységnyi sejtre eső plasminogen aktivátor inhibitor 1 (PAI-1) termelés csökken a proliferáció mértékének csökkenésével, viszont a sejtek hyaluronan (HA) termelése nem változik. Feltételezzük, hogy az orbita fibroblastok PAI-1 szintézise a fokozott proliferáció támogatásával szerepet játszhat az EOP pathogenesisében. Igazoltuk, hogy a TGF-β a PAI-1 és a HA szintézis pozitív regulátora, és a hatás sejtdenzitás függő orbita fibroblastokban. Az orbita eredetű sejtek viselkedése eltért a nem orbitából származó sejtekétől, de az EOP és nem-EOP sejtvonalak között különbséget nem találtunk. A további vizsgált faktorok (citokinek – TNFα és IFNγ, hypoxia, TSH) közül a TNFα PAI-1 szintézis stimuláló hatását igazoltuk orbita fibroblastokban.
Results in English
Within the scope of this project an in vitro model has been established to study the behavior of orbital fibroblasts under the effects of factors contributing in the pathomechanism of Graves’ orbitopathy (GO), and to compare GO and non-GO orbital fibroblasts. For modelling altered proliferation rate characteristic during the course of GO, cell lines were cultured at increasing cell densities. We found that the plasminogen activator inhibitor type 1 (PAI-1) production, but not hyaluronan (HA) production per cell, declined with decreasing proliferation rate. We assumed that the PAI-1 synthesis of orbital fibroblasts supported proliferation, thus it might have a role in the pathogenesis of GO. We found that TGF-β induced PAI-1 and HA production in a cell density dependent manner. The effect of TGF-β on PAI-1 synthesis is specific to fibroblasts of orbital origin, and we did not found differences between GO and non-GO fibroblasts in this respect. Among the other factors examined (cytokines - TNFα és IFNγ, hypoxia, TSH) stimulating effect of TNFα on PAI-1 production was found in orbital fibroblasts.
