A decorin az extracelluláris mátrix egy kis leucinban gazdag proteoglikánja, mely több, különböző szöveti eredetű tumor proliferációját gátolja. Azonban a decorinról a májrákok vonatkozásában alig található adat az irodalomban. Bebizonyították, hogy a decorin képes különböző tirozin kináz receptorok (RTK) gátlására, pl. az EGFR, IGF-IR, Met, VEGFR, PDGFR melyek mindegyikének fontos szerep jut a daganatok kialakulásában és progressziójában. E receptorok mesterséges vegyületekkel való gátlása a mai kemoterápiás kezelések integráns része. A májrákok esetében alkalmazott Sorafenib (Nexavar) célpontjai a VEGFR és PDGFR. Ennek alapján a decorin elméletileg alkalmazható nemcsak e receptorok gátlására, de az EGFR és Met receptorokéra is, melyek szintén szerepet játszanak a máj daganatainak kialakulásában és progressziójában. Vizsgálataink célja a decorin szerepének és tirozin kináz receptorok aktivitására kifejtett hatásának három szinten történő tanulmányozása primer hepatocellularis rákokban és colon carcinoma májmetastasisaiban: a.) in vitro modellrendszeren, ahol hepatoma és colon carcinoma sejtvonalakat kezelünk humán rekombináns decorinnal és vizsgáljuk proliferációra, valamint a RTK-ok aktivitására gyakorolt hatását; b.) in vivo, a decorin védő szerepét decorin knockout és vad típusú egerek segítségével, primer hepatocarcinogenesis és colon carcinoma-májmetastasis modellekben. Itt lehetőség nyílik exogén decorin bejuttatás hatásának vizsgálata e pathologiás folyamatokra, illetve a RTK-ok gátlására; c.) humán HCC és colon carcinoma májmetastasis mintákon a decorin mennyiség és RTK aktivitás összefüggésére.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Kiinduló hipotézisünk, hogy a decorin tumorszuppresszorként viselkedik a primer és metastaticus májrákokban. Az irodalmi adatok alapján a decorin hatásos tirozin kináz receptor gátló, így joggal merül fel a kérdés, vajon használható-e a decorin mint fiziológiás RTK gátló a máj daganatos megbetegedéseiben. In vitro és in vivo kísérleteink, valamint humán biopsziás mintákon történő vizsgálataink válasszal szolgálnának arra, hogy a decorin védő szereppel bír-e primer hepatocellularis carcinomákban illetve colon carcinomák májmetastasisai esetében. Ezen felül megtudhatjuk, hogy a decorin mesterséges bejuttatása lassítja vagy gátolja-e a fent említett daganatok növekedését illetve megjelenését, blokkolja-e az EGFR, Met, VEGFR és PDGF receptorok aktivitását, így alkalmazható lehet-e fiziológiás RTK inhibitorként.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. A kutatás eredményei bővítenék a decorin tumorszuppresszor szerepéről eddig megjelent ismereteket azzal, hogy jelentőségét a májrákok esetében tisztázzuk. In vivo rendszerünk, a decorin knockout egerek használatával és hepatocarcinogének segítségével indukált cirrhotikus és nem cirrhotikus környezetű primer májrákok vizsgálatával, a humán megbetegedéshez közelibb, pontosabb modellül szolgál, mint az általában használatos SCID egerekbe oltott xenograft modellek, lehetőséget teremtve az immunrendszer meghagyására és a betegség kialakulásának vizsgálatára. Az állatok rekombináns decorinnal történő kezelése és annak hatása a tirozin kináz receptorok aktivitására előrevetíti esetleges terápiás felhasználás lehetőségét. A decorin kemoterápiás alkalmazása elméletileg előnnyel bír a jelenleg gyakorlatban alkalmazott Sorafenib (Nexavar) kezeléssel szemben, mivel a VEGFR és PDGFR mellett további célpontjai az EGFR és Met, melyek kiemelten fontosak a májdaganatok kialakulásában és progressziójában. Emellett a decorin a szervezetben természetesen is előforduló molekula, mely mennyisége előzetes megfigyeléseink alapján a májdaganatok körül felszaporodik, így feltehetően a test természetes védekező mechanizmusának része. Mindezek alapján gyakorlati alkalmazása májrákokban szenvedő betegeknél akár egyedül, akár neoadjuvánsként esetleg prevenciós céllal nem megalapozatlan.

A decorin egy kisméretű, cukor oldallánccal rendelkező fehérje, mely a sejtek közötti térben található. Képes többféle daganat növekedését gátolni, azonban a májrákok vonatkozásában alig található adat az irodalomban. A decorinról bebizonyosodott, hogy olyan, a tumorsejtek felszínén lévő molekulákat (receptorokat) gátol, melyek a daganatok kialakulása és növekedése szempontjából fontos szabályozó faktorok hatását közvetítik. E receptorok gátlása a mai személyre szabott kemoterápiás kezeléseknek is része. A májrákok esetében használatos ilyen típusú gyógyszer a Sorafenib (Nexavar), mely kétféle receptort blokkol. Ezzel szemben a decorinnak több célpontja is van, így elméletileg alkalmas lehet a májrákok kemoterápiás kezelésére. Kiinduló hipotézisünk, hogy a decorin daganatgátlóként viselkedik májrákok esetében. Májrák és vastagbél rák sejtvonalakon, decorin génkiütött állatokon és emberi mintákon végzett kísérleteink választ adhatnak arra a kérdésre, hogy a decorinnak van-e védő szerepe primer májrákokkal illetve vastagbélrák májáttéteivel szemben. Ezen felül megtudhatjuk, hogy a decorin mesterséges bejuttatása lassítja, vagy megakadályozza-e az előbb említett daganatok megjelenését, illetve növekedését, képes-e gátolni a daganat sejtek felszínén lévő receptorokat, így alkalmazható-e kemoterápiás szerként.

Summary of the research and its aims for experts

Decorin is a small leucine-rich proteoglycan of the extracellular matrix, which is capable of inhibiting proliferation of tumors with various tissue origin. However, regarding liver tumors, there is hardly any data on the role of decorin in the literature. Decorin is proved to bind and block the activity of different receptor tyrosine kinases (RTK) such as EGFR, IGF-IR, Met, VEGFR, and PDGFR all known to have a crucial role in development and progression of tumors. Inhibition of RTKs by artificial compounds is part of the recent chemotherapeutic treatments. At present, Sorafenib (Nexavar) targeting VEGFR and PDGFR is applied for treating liver cancer. Based on this knowledge, theoretically, decorin blocking not only these two, but also EGFR and Met both receptors relevant in formation and progression of liver tumors could provide a new therapeutic approach. The aim of our research project is to study the applicablilty of decorin and its effect on RTK activity in pimary hepatocellular cancers and in liver metastases of colon carcinomas on three levels: a: in an in vitro model system, where hepatoma and colon carcinoma cell lines will be treated with recombinant decorin and their proliferation and activation state of their RTKs will be tested; b: in vivo testing the protective role of decorin in an experimental hepatocarcinogenesis and colon carcinoma-liver metastasis model using decorin knockout and wild type mice. Here, the system allows us to assess the effects of exogenous decorin on the pathological processes and on inhibition of RTKs; c: investigating the correlation between decorin level and RTK activity on samples of human HCCs and liver metastases of colon cancer.

What is the major research question?

Our hypothesis is that decorin acts as a tumor suppressor in primary and metastatic liver cancers. Based on data of the literature decorin is a potent tyrosine kinase receptor inhibitor, thus raising the question whether decorin can be utilized as a physiological RTK inhibitor in cancers of the liver is well-established. Our in vitro and in vivo experiments and studies on human biopsy samples will provide an answer as to whether decorin exerts a protective effect against primary hepatocellular carcinomas and liver metastases of colon carcinomas. Moreover we intend to find out if exogenous decorin delivery is capable to slow down or prevent tumor formation and to block EGFR, Met, VEGFR and PDGF receptor activity, thus serving as a physiological RTK inhibitor.

What is the significance of the research?

Results of our project would improve the knowledge already published in the literature on the tumor suppressor role of decorin with adding its action and importance in liver cancers. Our in vivo system with use of decorin-null mice and with induction of primary hepatocarcinogenesis in cirrhotic and non-cirrhotic livers serves as a more accurate model of human diseases than utilization of xenograft models implanted in SCID mice. This system allows us to investigate the development of the disease without omitting the immune system. Treatment of the animals with recombinant decorin and follow its effect on activity of receptor tyrosine kinases predicts its possible therapeutical benefit. Use of decorin and a chemotherapeutic agent ideally holds advantages against Sorafenib (Nexavar) applied in the present routine as decorin inhibits not only VEGFR and PDGFR targets of Sorafenib, but also EGFR and Met both important in development and progression of liver cancers. Besides, decorin naturally occurs in the body, and its accumulation around liver tumors was observed in our previous studies suggesting that it might take part in the natural defense mechanism of the human body. Based on these, practical application of decorin in treatment of patients with liver cancer either alone or as a neoadjuvant, or maybe with a preventional approach is justified. Thus, appearance of decorin in clinical treatment of cancers is not far ahead in the future.

Decorin is a small protein containing a sugar side chain and found in the space among cells. It is capable to retard growth of various tumors; however there is only a few data available in the literature on the effect of decorin in liver cancer. It is proved that decorin can block molecules on the surface of tumor cells (called receptors) that transmit effect of regulatory factors participating in the formation and growth of cancers. Blockade of these receptors is an integral part of personalized chemotherapy used at present. A type of these medicines is Sorafenib (Nexavar) utilized in treatment of liver cancers and blocks two of these receptors. We hypothesize that decorin acts as a tumor inhibitor in liver cancers. Our researches on liver cancer and large intestine cancer cells, on mice lacking decorin and on human samples would answer the question whether decorin has a protective role against primary liver cancers and metastates of colon cancers. In addition we learn if artificial delivery of decorin is able to slow down or prevent the formation or growth of tumors mentioned above, and to block the receptors on the tumor cell surface, hence could be applied as a chemotherapeutic compound.

E receptorok gátlása a mai személyre szabott kemoterápiás kezeléseknek is része. Kiinduló hipotézisünk, hogy a decorin daganatgátlóként viselkedik primer és áttétes májrákok esetében. Tumor sejtvonalakon, decorin génkiütött állatokon és emberi mintákon végzett kísérleteink alapján a decorinnak védő szerepe van primer májrákokkal illetve vastagbélrák májáttéteivel szemben. A decorin mesterséges bejuttatása lassítja, ill. megakadályozza e daganatok megjelenését, növekedését, és hatásosan gátolta a tumorsejtek felszínén lévő tirozin-kináz receptorokat. Mindezek alapján gyakorlati alkalmazása májrákokban szenvedő betegeknél akár egyedül, akár neoadjuvánsként esetleg prevenciós céllal nem megalapozatlan.

Blockade of these receptors is an integral part of personalized therapy introduced for the treatment of cancer in the last decade. We hypothesized that decorin acts as a tumor inhibitor in primary and metastatic liver cancers. Our studies on liver cancer cell lines, on mice lacking decorin, as well as on human specimens proved that decorin has a protective role against primary liver cancers and metastases of colon cancers. In addition, artificial delivery of decorin is able to slow down or prevent the formation and growth of tumors mentioned above. Moreover, decorin effectively inhibited various tyrosine kinase receptors on the tumor cell surface. Based on our results, application of decorin in treatment of patients with liver cancer either alone or in combination with other compounds might be considered.
