Az emberi bőr fizikai, kémiai és mikrobiológiai hatásoknak ellenálló, komplex barrier. A bőr barrier funkcióinak több szintje létezik és ezek szoros együttműködése elengedhetetlen. A két legfontosabb barrier elem a fiziko-kémiai barrier és az immunológiai barrier. A gyulladásos, immun-mediált bőrbetegségek közül kiemelkedő jelentőségű az atopiás dermatitis (AD) és a psoriasis vulgaris. Mindkét betegség nagyon gyakori, krónikus, jelenleg csak tünetmentessé tehető, nem gyógyítható. Ezen kórképek esetében az utóbbi években egyre nagyobb figyelem fordult a bőr barrierjének tanulmányozása felé. Munkánk során részben leíró jellegű információkat szeretnénk nyerni az AD-re és a psoriasisra jellemző barrier eltérésekről, részben pedig modell rendszerekben alapkutatási módszerekkel vizsgáljuk, hogy a különböző típusú barrier károsodások hogyan hatnak a barrier más elemeire. Kutatásaink központi sejtjei a keratinocyták, mivel ezen sejtek kiemelt jelentőségűek mind a fiziko-kémiai barrier, mind az immunológiai barrier működésében. A kutatások során nyert új adatok, a betegségek patomechanizmusának részletesebb feltárása jelentősen befolyásolhatja terápiás algoritmusainkat és hatékonyságunkat. Míg az elmúlt évtizedekben elsősorban a gyulladáscsökkentés (immunológiai barrier javítása) volt a fő terápiás eszköz, az új kutatási eredmények egyre inkább előtérbe helyezik a fiziko-kémiai barrier helyreállítását célzó kezeléseket. Ugyancsak reméljük, hogy kutatásaink segítségével a gyulladásos bőrbetegségeken belül alcsoportokat tudunk elkülöníteni és az alcsoportok ismerete pozitívan befolyásolhatja a diagnosztika, a terápia és a további kutatások alakulását.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Az egészséges, gyulladásmentes bőr esetén a bőr fiziko-kémiai és immunológiai barrierje egy stabil egyensúlyi helyzetben, egymást támogatva és kiegészítve működik. Irodalmi adatokra alapozott hipotézisünk szerint gyulladásos bőrbetegségek esetén, akár a betegség iniciáló faktoraként a két komponens közül valamelyik működése károsodik, és emiatt az egyensúly fenntartása érdekében kompenzációs folyamatok indulnak be a másik oldalon. Amennyiben a folyamat progrediál, az egyensúly végül teljesen megbomlik és a túlműködő faktorok miatt gyulladás alakul ki, szubklinikai majd manifeszt klinikai tünetek jönnek létre. A kezdeti barrier károsodás lehet genetikai vagy szerzett, érintheti elsősorban a fiziko-kémiai barrier elemeit (keratinocyta, corneocyta, filaggrin, egyéb struktúrfehérjék, corneodesmosomák, tight junction, lipid réteg, proteázok, inhibitorok stb.) vagy az immunológiai barrier összetevőit (keratinocyta, dendritikus sejt, T sejt, Toll receptorok, NOD receptorok, citokinek, kemokinek stb.) Munkacsoportunk alapvetően arra keresi a választ, hogy a fiziko-kémiai barrier változásai módosítják-e a bőr immunológiai működését, illetve fordítva, az immunológiai barrier eltérései hogyan hatnak a fiziko-kémiai barrierre, elsősorban a keratinocyták defense mechanizmusaira. Az alapkutatási módszerek alkalmazása mellett munkánkat teljessé teszi, hogy immun-mediált bőrbetegségekben (atopiás dermatitis, psoriasis) is tanulmányozzuk a fenti összefüggéseket, a bőr fiziko-kémiai barrierje és immunológiai defense mechanizmusainak eltéréseit, ezek szerepét a betegségek kialakulásában és ezen eredmények alapján új diagnosztikai, későbbiekben terápiás eljárások kifejlesztése indulhat el.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

Az atopiás dermatitis (AD) és a psoriasis vulgaris két nagyon gyakori, immun-mediált, gyulladásos bőrbetegség, melyek együttesen hazánk felnőtt lakosságának kb. 3%-át érintik, gyermekkorban az AD előfordulása megközelíti a 20%-ot. Az életminőséget súlyosan károsító hatásukon túli jelentőségükre az utóbbi évek kutatásai hívták fel a figyelmet. A súlyos gyermekkori AD fokozott rizikót jelent asthma bronchiale, illetve rhinitis allergica későbbi kialakulására a bőrön át történő szenzitizáció miatt. Psoriasis esetében pedig feltételezhetően a kezeletlen, súlyos formáknál fennálló szisztémás gyulladás az egyes komorbiditások (pl. kardiovaszkuláris betegségek) progresszióját fokozhatja, így hozzájárulhat a betegek élettartamának rövidüléséhez. Kutatásaink fontos célja és jelentősége részben, hogy leíró jellegű információkat nyerjünk az AD-re és psoriasisra jellemző bőr barrier eltérésekről, melyek jelentősen befolyásolhatják terápiás algoritmusainkat és hatékonyságunkat. A barrier kóros működésének korai felismerése és kezelése ugyanakkor a primer prevencióban is jelentős lehet. Kutatásaink másik jelentősége, hogy modell rendszerekben alapkutatási módszerekkel vizsgáljuk a különböző típusú barrier károsodások egymásra kifejtett hatását. Az ennek során általunk kidolgozott és beállított modell rendszerek (külső gyökérhüvely keratinocyta tenyésztés, gyulladásos dendritikus sejt modell előállítása in vitro) könnyebben tanulmányozhatóvá tehetik a gyulladásos bőrbetegségeket, melyeknél a bőr biopsziás minták nyerése bizonyos határokba ütközik. Eredményeink ugyancsak segíthetnek a gyulladásos bőrbetegségeken belüli alcsoportok felismerésében és azok karakterizálásában, mivel meggyőződésünk szerint sem az AD, sem a psoriasis nem egy egységes betegség, hanem egy-egy betegség csoport. Ezeken belül mind a kutatómunkát (genetikai analízisek), mind a terápiák hatékonyságát nagyban növelné az altípusok ismerete. Végül reményeink szerint a follicularis T helper sejtek vizsgálatával olyan diagnosztikai módszert is ki tudunk dolgozni, mely alkalmas lehet az immunterápia hatékonyságának monitorizálására atopiás betegségekben.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

Bőrünk számos feladata közül az egyik legismertebb, hogy ellenáll a szervezetet érő fizikai, kémiai és fertőző ágensek által kifejtett káros hatásoknak, ugyanakkor meggátolja a kóros mértékű víz és elektrolit vesztést is. Ezt a funkciót „barrier” működésnek nevezzük, melyet a bőr sejtjeinek, enzimjeinek, egyéb fehérje és lipid molekuláinak szigorúan irányított közös munkája biztosít. A barrier működés két alapvető eleme az immunológiai barrier és a fiziko-kémiai barrier, melyek károsodását több gyulladásos bőrbetegségben is leírták az utóbbi években. Saját kutatásainkban két bőrgyógyászati kórképben, atopiás dermatitisben és psoriasis vulgarisban vizsgáljuk a bőr immunológiai és fiziko-kémiai barrierjének változásait, eltéréseit és ezek egymásra kifejtett hatásait. Ez a két immun-mediált kórkép nagyon gyakori mind Magyarországon, mind Európában és mivel tüneteiket ugyan már jelentősen lehet javítani, de végleges gyógyulást nem tudunk elérni a betegeknél, így minden kutatási eredmény, ami közelebb visz a betegség okainak megismeréséhez, újabb és hatékonyabb terápiák kifejlesztéséhez vezethet. Véleményünk szerint a bőr barrier eltéréseinek kiemelkedő szerepe van az atopiás dermatitis és a psoriasis kialakulásában és ezek feltárása illetve tanulmányozása új terápiás eljárások alapját teremheti meg. Ugyanakkor vizsgálatainkkal új diagnosztikai módszert is tervezünk fejleszteni.

The human skin is a complex barrier, resistant to physical, chemical and microbiological effects. Multiple levels of the skin barrier system exist that require close cooperation. The most important barriers are the physico-chemical and the immunological barriers. Among the immune-mediated skin diseases atopic dermatitis (AD) and psoriasis are extremely important. Both are very common, chronic, incurable, and only symptomatic treatment is available. In the recent years the attention has been focused on the investigation of barrier functions in these two diseases. In our work we are planning to gain descriptive information about the barrier impairments in AD and psoriasis, and furthermore by using basic molecular biological methods and new model systems we are preparing experiments to investigate the influence of one barrier component on other barrier elements. The central interest of our research will be the keratinocyte, since this cell play a crucial role both in the physico-chemical as well as in the immunological barrier. The gathered new data may help to reveal more information on the pathomechanisms of these diseases and they may also promote the modifications of the therapeutical algorithms. While in the recent decades the main therapeutic tool was the anti-inflammatory treatment (improvement of the immunological barrier), new results promote the treatments that restore the physico-chemical barrier. Furthermore we hope that our data will help us to distinguish certain subgroups in these inflammatory skin diseases and this distribution may affect the diagnostic, therapeutic interventions as well as the further investigations in clinical research.

What is the major research question?

In healthy conditions the physico-chemical and immunological barrier functions of the skin are in a harmonic equilibrium, supporting and substituting each other. According to our hypothesis and also based on the literature, when one of these barriers is altered, the other counterpart tries to compensate in order to restore the fine homeostatic balance. If this compensatory mechanism is further progressing, an imbalance will develop resulting in inflammation of the skin, and the persisting inflammation on a given genetic background will lead to the manifestation of the characteristic clinical symptoms. The initiating barrier dysfunction may be genetic or acquired, and may affect either the elements of the physico-chemical barrier (keratinocyte, corneocyte, filaggrin, other structural proteins, corneodesmosomes, tight junctions, lipid layer, proteases, inhibitors, etc) or that of the immunological barrier (keratinocyte, dendritic cell, T cells, Toll-like receptors, NOD-like receptors, cytokines, chemokines etc). Our research group is basically inquiring whether the alterations in the physico-chemical barrier may modify the immunological processes in the skin, or if the changes in the immunological barrier may affect the physico-chemical barrier. Beside applying the basic scientific research approaches we are also able to directly study these relationships in immune-mediated skin diseases (atopic dermatitis, psoriasis) as well as the role of barrier dysfunctions in the development of these skin conditions. Based on the new results we may be able to develop new diagnostic and therapeutic methods in the near future.

What is the significance of the research?

Atopic dermatitis (AD) and psoriasis are very common immune-mediated inflammatory skin diseases that together affect about 3% of the entire adult population of our country, and AD also affects about 20% of the children population. These diseases also have a significant negative effect on the quality of life. Furthermore the presence of childhood AD is a strong predisposing factor for the development of bronchial asthma, or allergic rhinitis, because sensitization may be possible through inflamed skin. In psoriasis it has been postulated that the severe, untreated forms of the disease will generate a systemic inflammation that further promote the progression of other co-morbidities (cardiovascular diseases) with a decreasing life expectancy. Important goals of our studies are to gain descriptive information about the skin barrier alterations in AD and psoriasis with significant new results regarding the therapeutic algorithms. The early recognition of the discrepancies in the skin barrier function and their immediate treatment may help in the primary prevention of these diseases. Another important aspect of our research is that different types of barrier dysfunctions are examined by basic research tools in model systems. During this innovative step we create and apply new model systems, such as outer root sheath keratinocyte culture and inflammatory dendritic cell model in vitro. These methods can help us to study inflammatory skin diseases, especially when skin biopsy specimens are difficult to collect. Our results will also help us to recognise and categorise new subtypes within these two diseases, while we believe that none of these diseases are homogenous but rather they are a spectrum of different subgroups. The efficacy of genetic analyses as well as that of treatment modalities may be increased by the knowledge of these subtypes within the diseases. We also hope that by the investigation of follicular T helper cell population we will be able to develop a diagnostic tool that enables us to monitor the efficacy of allergen specific immunotherapy in atopic diseases.

Among the many functions of our skin one of the most respected one is that it resists physical, chemical, and microbial agents that may directly threaten our organism. Furthermore our skin is able to protect us from excessive water and electrolyte loss. This task is called barrier function. The cells, enzymes, lipids, proteins of the skin are participating in an orderly fashion to maintain this very sensitive skin barrier function. The two basic elements of skin barrier functions are the immunological and the physico-chemical barriers that may be altered in many inflammatory skin diseases as it has been described in the recent years. In our own studies we will investigate the changes of the immunological as well as the physico-chemical barrier both in atopic dermatitis (AD) and in psoriasis. These two immune-mediated diseases are very common in Hungary. Until now only symptomatic treatments have been available in these diseases and definite therapy with total recovery is not known. We believe that skin barrier alterations play major role in the development and maintenance of AD and psoriasis. Every new detail about the barrier dysfunctions and the pathomechanism of these diseases will help to develop new treatment strategies and more effective medications. Furthermore, based on our results we are willing to describe new diagnostic techniques as well.

Kutatómunkánk során a bőr immunológiai és fiziko-kémiai barrierjének jellemzőit és ezek összefüggéseit vizsgáltuk egészséges bőrben és bőrbetegségekben. Munkacsoportunknak elsőként sikerült bizonyítani, hogy a különböző bőr régiókra eltérő veleszületett és adaptív immunológiai miliő és fiziko-kémiai barrier jellemző, mely magyarázatot adhat arra, hogy bizonyos immun-mediált bőrbetegségek miért lokalizálódnak egy adott bőr régióra. Ezen elgondolás jelentősen befolyásolhatja a barrier helyreállító terápiák fejlesztését. Ugyancsak felhívjuk a figyelmet arra, hogy az „egészséges kontroll bőrminta” fogalma csak abban az esetben helytálló, ha a kutató pontosan leírja a bőr régiót, ahonnan a minta származik. Atópiás dermatitisben sikeresen detektáltunk eltérő adaptív (Treg, follikuláris T sejt) és veleszületett (dendritikus sejt) immunválaszbeli, valamint fiziko-kémiai barrier defektusokat (filaggrin mennyiség), melyek mind összefüggést mutattak a bőr gyulladásának mértékével. Ezek AD specifikusnak mondhatóak, ugyanis HIES (Hyper-IgE szindróma)–ben szenvedő betegeknél, az AD-hez hasonló bőrtünetek ellenére nem találtunk barrier eltéréseket vagy allergiás érzékenységet. Psoriasis esetében kimutattuk, hogy a súlyos bőr eltéréseket szisztémás gyulladás (aktivált dendritikus sejtek) valamint társbetegségek (alacsony D3 vitamin szint, gyakori szubklinikus kardiovaszkuláris károsodás, életminőség-romlás) kísérik, mely rávilágít arra, hogy a psoriasis több mint bőrbetegség.

In this research project we investigated the characteristics and the connections between the immune and the physico-chemical skin barriers in health and in immune-mediated skin diseases, like atopic dermatitis (AD) and psoriasis. We provided the first evidences that different skin regions have region specific innate and adaptive immune and barrier milieu, which may explain the preferred localization of some immune-mediated and autoimmune skin disorders and can influence future barrier repair therapeutic concepts. We also proposed that the term „healthy skin control sample”, should only be accepted if researchers carefully specify the exact region of the healthy skin. In AD we could detect different adaptive (Treg, follicular T cell), innate (dendritic cell) and barrier defects (filaggrin content), which all were closely connected to the severity of the skin inflammation, and seemed to be AD specific, since in patients suffering from HIES (Hyper IgE Syndrome), in spite of the AD-like skin lesions, no barrier defects or allergic sensitizations were found. In psoriasis we could demonstrate that severe skin immune alterations are accompanied also by severe systemic inflammation (activated blood dendritic cells) and comorbidities (low vitamin D3 levels, frequent subclinical cardiovascular disease, diminished quality of life), which reflect that psoriasis is more than just “skin deep”.
