Kutatásunk célja a transzplantált vese antitestek közvetítette kilökődésének (ABMR) vizsgálata volt. Meg kívántuk határozni, hogy a komplementrendszer egyes genetikai tényezőinek (C4 kópiaszám variációk, és a komplotípus meghatározói) illetve a plazma vagy vizelet komplement biomarker profiljának milyen kapcsolata van az ABMR előfordulásával, és annak fontos szövettani jelével, a peritubuláris kapillárisok C4d jelölődésével. Eredményeink arra utalnak, hogy a vesetranszplantált betegek C4 CNV, és a komplotípust meghatározó gyakori polimorfizmusok allélfrekvenciái nem térnek el jelentősen a kontroll értékeitől. Megállapítottuk, hogy a donor-specifikus antitestek (DSA) mennyisége és kötőképessége a legfőbb meghatározója az ABMR kialakulásának, és ehhez a komplement biomarker profil mutatói nem képesek hozzátenni diagnosztikai vagy prognosztikai értelemben. Ehhez hasonlóan a komplement biomarkerek nem szolgáltattak plusz információt a C4d-pozitív ABMR megállapításához. Ugyanakkor vizsgálatainkból kiderült, hogy a vizeletben mérhető komplement markerek nem specifikus jelzői a glomeruláris károsodásnak, hiszen összefüggést mutattak a proteinura mértékével. Megállapítottuk, hogy a komplement biomarkerek mérésével nem javítható a DSA mérésével megállapított rejekció jelenléte, mértéke vagy minősége, így megerősíthető az a feltételezés, hogy a DSA elsődlegesen nem a komplementaktiváció által vezetnek el a vesegraft károsodásához.
Results in English
The aim of our research was to investigate antibody mediated rejection (ABMR) of kidney allografts. We aimed to determine whether the relationship between the individual genetic factors of the complement system (C4 copy number variants and the complotype determinants), and the complement biomarker profile of the plasma or urine are related to the occurrence of ABMR, and to its important histological (C4d stain of peritubular capillaries). Our results suggest that the renal transplant patients' C4 copynumbers and allelic frequencies of common polymorphisms that determine the complotype do not differ significantly from the control values. We have found that the amount and binding capacity of donor-specific antibodies (DSA) is the main determinant of the development of ABMR, and the complement biomarker profile components are unable to improve the diagnostic or prognostic power. Similarly, complement biomarkers did not provide additional information to determine C4d-positive ABMR. However, our investigations revealed that complement markers that are measurable in the urine are nonspecific indicators of glomerular damage as they correlated with the degree of proteinuria. We have found that by using complement biomarkers, the presence, degree or quality of the rejection - as determined by DSA - can not be improved, so we confirmed that DSA mediated kidney graft damage is caused by other mechanisms, than complement activation.
