A pályázat célja a XIII-as véralvadási faktor (FXIII) B alegységének (FXIII_B) lényeges struktúrális, funkcionális és klinikai aspektusainak feltárása. A pályázaton belül célunk:
• A FXIII-B 3 dimenziós szerkezetének homológ modellezése és a modell kísérletes validálása.
• Meghatározni, hogy a FXIII-B a plazmában monomer vagy dimer formában van-e jelen.
• Azonosítani a FXIII-B FXIII-A-val való kapcsolódásában szerepet játszó epitópo(ka)t.
• Karakterizálni a FXIII-B fibrinogénhez való kötődését.
• Megvizsgálni, hogy a FXIII-B hogyan szabadul fel a fibrinogénről a fibrin képződés során.
• Felfedni a szénhidrát oldalláncok szerepét a FXIII-B funkciójában, különös tekintettel a FXIII keringésben való fél-életideje szempontjából.
• Megvizsgálni a szénhidrát oldalláncok szerepét a FXIII-B S. aureus protein A-hoz való kapcsolódásában.
• Kifejleszteni egy, a plazmában lévő szabad FXIII-B meghatározásra alkalmas immunassay-t és kidolgozni annak alkalmazását rekombináns FXIII-A2 terápián lévő FXIII-A deficiens betegek plazmájában a szabad FXIII-B meghatározására.
• Meghatározni a FXIII-B polimorfizmusok és a FXIII szintek hatását a myocardiális infarctus rizikójára idősebb és relatíve fiatalabb populációban.
• -Megvizsgálni a FXIII-B polimorfizmusok és a FXIII szintek hatását a vénás tromboemboliák rizikójára

A véralvadás XIII-as faktora (FXIII) a hemosztázis egyik kulcsfontosságú tényezője, hiánya súlyos vérzékenységhez, rossz sebgyógyuláshoz vezet, nők esetén pedig a terhesség kihordása nem lehetséges. A plazma FXIII két FXIII-A és két FXIII-B alegységekből áll (FXIII-A2B2). A FXIII-B nélkülözhetetlen a potenciális katalitikus aktivitással rendelkező FXIII-A2 keringésben tartásához. A keringő FXIII-A2B2 féléletideje kb. 11-12 nap, mely a FXIII-B hiányában néhány órára csökken. A FXIII-B-vel való komplex képződés azt is megakadályozza, hogy a FXIII-A2 spontán, szükségtelenül aktiválódjon. Bár a FXIII-B a plazma FXIII nélkülözhetetlen komponense, szerkezete és funkciója több szempontból máig felderítetlen. Vizsgálataink eddig ismeretlen struktúrális/funkcionális molekuláris tényezők feltárása irányulnak (3 dimenziós struktúra, monomer-dimer állapot, szénhidrát-oldalláncok szerepe, a FXIII-A2-vel való komplex képződésének struktúrális elemei). Az alegységek interakciójában szerepet játszó epitóp identifikálása lehetővé teheti egy olyan peptid szintézisét, mely gátolhatja a két alegység kötődését. Egy ilyen peptidben jelentős terápiás potenciál rejlik, hiszen a FXIII szintet olyan szintre csökkentheti amely már antitrombotikus, de nem fokozza a vérzés kockázatát. A fibrinogén a szabad és a komplexben lévő FXIII-B szállítója. A FXIII-B-fibrinogén interakció jelentősége és ennek hirtelen megszűnése a fibrin képződése során nem ismert. E folyamat struktúrális követelményeinek megismerése ezen a területen új utakat nyithat a kutatások számára. A plazmában lévő szabad FXIII-B mérésére alkalmas módszer lehetővé teszi a szabad FXIII-B monitorozását; a szabad FXIII-B ahhoz szükséges, hogy a terápiásan alkalmazott rekombináns FXIII-A2 a keringésben maradhasson.
Az emelkedett FXIII szinteket összefüggésbe hozták a miokardiális infarktus (MI) rizikójával nőkben, és a FXIII-A p.Val34Leu polimorfizmus védő hatást nyújt MI-al és vénás tromboembóliákkal (VTE) szemben. A FXIII-B polimorfizmusok szerepe a MI és a VTE rizikójában nem ismert, erre a pályázat eredményei adhatnak választ. Azt is megtudhatjuk, hogy amennyiben van hatás, az a FXIII szinteken keresztül érvényesül-e.

Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

A vérlalvadás XIII-as faktora (FXIII) kulcsszereplő a vérzés csillapításban. A FXIII két A (FXIII-A) és két B (FXIII-B) alegységből áll. A FXIII hiány súlyos vérzékenységet, rossz sebgyógyulást és nők esetében spontán abortuszt okoz. A FXIII-A alegység a felelős a vérzés elállításáért, a sebgyógyulás gyorsításáért és a magzat elvesztésének megakadályozásáért. A FXIII-B a FXIII-A-hoz kötődik és azt a keringésben tartja. A FXIII hiányos betegek élethosszig tartó pótló terápiát igényelnek. Erre a célra legújabban virusmentes rekombináns FXIII-A koncentrátumot vezettek be. A rekombináns FXIII-A a beteg saját FXIII-B-jét használja fel a keringésben maradáshoz. Ismerve a FXIII-B fontosságát és azt, hogy funkciója sok tekintetben ismeretlen, ennek a molekulának a tanulmányozását tűztük ki célul. Célunk eleddig ismeretlen strukturális vonatkozások, a FXIII-A hoz való kötődés mechanizmusának, a fibrinogénnel való kölcsönhatásnak a felderítése. Egy új módszert fogunk kifejleszteni a szabad FXIII-B mérésére, mely hozzájárul a rekombináns FXIII terápia biztonságához. Míg a FXIII hiány vérzékenységgel jár, a FXIII szintek változásai és mutáns FXIII befolyásolhatják veszélyeztetettséget az artériás és vénás trombozisokra. Vizsgálni fogjuk, hogy a FXIII szintek és a népesség körében gyakran előforduló FXIII-B mutánsok (polimorfizmusok) hogyan függenek össze a miokardiális infarktus és a mély vénás trombozis rizikójával.

The proposed research intends to explore important structural, functional and clinical aspects of the B subunit of coagulation factor XIII (FXIII). Within the project we intend:
• To construct a model for the 3 dimensional structure of FXIII-B by homology modeling and validate it experimentally.
• To determine if free FXIII-B in the plasma exists in monomeric or dimeric form.
• To identify the epitope involved in the interaction of FXIII-B with FXIII-A.
• To characterize the binding of FXIII-B to fibrinogen.
• To find out how FXIII-B is released from fibrinogen during its transformation into fibrin.
• To reveal the function of carbohydrate side chain present in FXIII-B with special reference to the lifespan of FXIII in the circulation.
• To reveal the role of carbohydrate side chain present in FXIII-B in its interaction with S. aureus protein A.
• To develop an immunoassay for the determination of free FXIII-B in human plasma and make it applicable for monitoring free FXIII-B in the plasma of FXIII-A deficient patients being on recombinant FXIII-A2 therapy.
• To assess the effect of FXIII-B polymorphisms and FXIII levels on the risk of myocardial infarction in elderly and in relatively young population.
• To assess the effect of FXIII-B polymorphisms and FXIII levels on the risk of venous thromboembolism.
• To determine the effect of FXIII-B polymorphisms on plasma FXIII-B and FXIII-A2B2 levels.

What is the significance of the research?

Coagulation factor XIII (FXIII) is a key player in hemostasis, its deficiency results in severe bleeding diathesis, impaired wound healing and, in women, inability to carry out pregnancy. Plasma FXIII is a heterotetramer of FXIII-A and FXIII-B subunits (FXIII-A2B2). FXIII-B is highly important for keeping FXIII-A2, the subunit with potential catalytic activity, in the circulation. The half-life of circulating FXIII-A2B2 is approximately 11-12 days; in the absence of FXIII-B it is decreased to a few hours. Complex formation with FXIII-B also prevents the spontaneous undesired activation FXIII-A2 in plasmatic conditions. Although FXIII-B is an essential component of plasma FXIII its structure and function in many aspects is still unexplored. Our study will shed light on so far unknown structural/functional features of the molecule (3 dimensional structure, monomeric-dimeric state, the role of carbohydrate side chains, the structural entities involved in complex formation with the FXIII-A2). Identifying the epitope involved in subunit interaction may lead to the synthesis of a peptide that inhibits the interaction of the two subunits. Such peptide could have therapeutic potential by decreasing FXIII level to a range where it is antithrombotic without risking spontaneous bleeding. Fibrinogen act as a carrier for FXIII-B whether it is free or in complex. The importance of FXIII-B fibrinogen interaction and its sudden disruption during fibrin formation is not known. Revealing the structural requirements opens up a new way for research on this area.
A method to be developed on the measurement of free FXIII-B in the plasma makes it possible to monitor the concentration of free FXIII-B required for keeping therapeutically administered recombinant FXIII-A2 within the circulation.
Elevated FXIII levels have been associated with increased risk of myocardial infarction (MI) in women, and FXIII-A p.Val34Leu polymorphism confers a protective effect against MI and venous thromboembolism (VTE). The association of FXIII-B polymorphisms with the risk of MI and VTE is an unexplored area on which the proposed study could shed light. It will also show if such effect, if any, is exerted through the regulation of FXIII levels.

Blood coagulation factor XIII (FXIII) is a key player in arresting bleeding. FXIII consists of two A (FXIII-A) and two B (FXIII-B) subunits. FXIII-A deficiency results in severe bleeding complications, impaired wound healing and, deficient women cannot carry out pregnancy. FXIII-A exerts the effect that arrests bleeding, improves wound healing and prevents pregnancy loss. FXIII-B binds to FXIII-A and this way keeps it within the circulation. Deficient patients should be on life-long substitution therapy. For this purpose a virus-safe recombinant FXIII-A product was introduced. The recombinant product should utilize the patients own FXIII-B to remain in the plasma and be effective. Considering the important and, to a considerable extent, unexplored role of FXIII-B we set out to study this molecule. So far unknown structural features, the mechanism of binding to FXIII-A, its interaction with the clotting protein fibrinogen will be discovered. A new method for the measurement of free FXIII-B in the plasma will be designed to ensure the safety of recombinant FXIII therapy. While the lack of FXIII results in bleeding tendency, changes in its level or mutant FXIII-B might increase or decrease the risk of arterial and venous thrombosis. It will be investigated how FXIII level and mutant FXIII-B molecules, frequently occurring in the population (common polymorphisms), influence the risk of myocardial infarction and deep vein thrombosis.

1/ Megkonstruáltuk a FXIII-B két első sushi doménjének a 3D szerkezetét A két sushi domént meg is szintetizáltuk. A sushi domén 2 nem kötődött a FXIII-A2-höz. 2/ Az intron K nt29756 C>G polimorfizmusnak megfelelő variáns kevésbé kötődött a FXIII-A2-höz mint a vad típusú FXIII-B és csökkentette a FXIII szintet. 3/ A FXIII-B kötődött egy mindkét fibrinogen γ-lánc variánsban (γA and γ’) meglévő a rövid peptid szekvenciához és a γ’ lánc C-terminális peptid szekvenciájához is. 4/ Felderítettük a FXIII-B szénhidrát oldalláncának a kémiaI szerkezetét. 5/ Kidolgoztunk ELISA technikát a totál és a szabad FXIII-B meghatározására és segítségével monitoroztuk a FXIII-B szinteket szerzett FXIII deficienciában. 6/ A FXIII-B intron K nt29756 G allél védett a koronária szklerózis és a miokardiális infarktus ellen magas fibrinogén szintek esetén, a FXIII-A Leu34 allél jelenlétében. A védőhatás összefüggött a FXIII szintek csökkenésével. 7/ Az intron K nt29756 G allél fiatal felnőtteket is védett a miokardiális infarktus ellen. A dohányosok emelkedett FXIII szintje hozzájárult fokozott rizikójukhoz. 8/ A FXIII szint felső harmadában lévő nőkben fokozódott a venás tromboembolia kockázata; az emelkedett FXIII-B szint férfiakban védő hatással bírt.
Results in English
1/ The 3D structure of the first two sushi domains of FXIII-B were constructed and used for blind docking to FXIII-A dimer. The two sushi domains were chemically synthesized and used for binding studies. Sushi domain 2 did not bind to FXIII-A2. 2/ FXIII-B variant due to intron K nt29756 C>G polymorphism bound to FXIII-A2 with less affinity than wild type FXIII-B, and decreased FXIII-A2B2 level. 3/ FXIII-B bound to a short peptide sequence present in both fibrinogen γ-chain variants (γA and γ’) and also to the C-terminal peptide of the γ’ chain. 4/ The chemical structure of FXIII-B carbohydrate side-chains was revealed. 5/ ELISAs for the determination of total and free FXIII-B in the plasma was designed and used for monitoring FXIII-B levels were in patients with acquired FXIII-A deficiency. 6/ The FXIII-B intron K nt29756 G allele provided significant protection against coronary artery sclerosis and myocardial infarction in patients with a fibrinogen level in the upper tertile in the presence of the FXIII-A Leu34 allele The protection was related to decreased FXIII level. 7/ Similar protective effect FXIII-B intron K nt29756 G allele against myocardial infarction was observed in young adults. The elevation of FXIII levels in smokers contributed to the increased risk of myocardial infarction in this population. 8/ FXIII-A2B2 antigen levels in the upper tertile increased the risk of venous thromboembolism in females; elevated FXIII-B antigen level had a protective effect in males.
