A bélnyálkahártya barrier strukturális és funkcionális károsodása, valamint csökkent védekező képessége bakteriális transzlokáció (BT) kialakulását eredményezi. A BT a májcirrhosis jellegzetessége, súlyosbítja a proinflammatorikus folyamatokat elősegítve ezáltal a májkárosodás kialakulását. A gyulladás és a véralvadás szorosan összefüggő, egymással kölcsönhatásban lévő folyamatok a máj mikrovaszkulatúrájában és szerepet játszanak a májcirrhosis pathogenezisében. A gyulladás és a véralvadás folyamatainak kölcsönhatása azonban számos vonatkozásában csak kevéssé ismert. Jelen tanulmányban célul tűztük ki a BT folyamatának, valamint az immunrendszer és a véralvadás kölcsönhatásának pontosabb megismerését májcirrhosisban. Előzetes kutatási eredményeink alapján a különféle, IgA típusú antimikrobiális- és auto-antitestek – melyek képződése májcirrhosisban fokozott – egyrészt a bélnyálkahártya megnövekedett mikrobiális terheltségét tükrözhetik, másrészt egyes patogenetikai folyamatok aktív résztvevői lehetnek. A cirrhosishoz társuló IgA típusú antimikrobiális- és auto-antitesteket jellemezzük, meghatározzuk azok szubtípus megoszlását és antigén specificitását. Tisztázzuk pontos képződési mechanizmusukat és vizsgáljuk a bél asszociált immunrendszer közreműködését a fokozott IgA képződésben. Igazoljuk, hogy bizonyos bakteriális antigének a B-sejt proliferáció és differenciáció aktiválásán keresztül specifikus IgA antitest termelését váltják ki. In vitro modellrendszerben vizsgáljuk, hogy az IgA típusú antitestek képesek-e fokozni az intrahepatikus mikrovaszkulatúrában zajló patológiás folyamatokat a von Willebrand factor függő thrombocyta aktiváció befolyásolása révén.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

A szérum total immunoglobulin A (IgA) szintjének emelkedése a májcirrhosis jellegzetessége, jelentősége és mechanizmusa azonban nem ismert. Igazoltuk, hogy bizonyos IgA típusú auto- és antimikrobiális szérum antitestek előfordulása májcirrhosisban gyakori és feltételezzük, hogy képződésükben a bakteriális transzlokáció folyamata és a bélnyálkahártya immunrendszere fontos szerepet játszik. (I) Igazoljuk, hogy májcirrhosisos betegek bélnyálkahártyájában az IgA termelő plazmasejtek mennyisége felszaporodik, jellemezzük a lokális IgA alosztály típusokat és bizonyítjuk a szekretoros komponens fokozott epiteliális expresszióját. (II) Igazoljuk, hogy bizonyos bakteriális antigének a B-sejt proliferáció és differenciáció aktiválásán keresztül specifikus IgA antitest termelését váltanak ki. (III) Azonosítjuk az IgA típusú ANCA antigénjeit. Munkacsoportunk korábban igazolta, hogy elsősorban az IgA típusú szérum antitestek kapcsolatot mutattak a májcirrhosis progressziójával és a betegség specifikus szövődmények kialakulásával, amely alapján feltételezhető, hogy ezek az antitestek szerepet játszhatnak a májfibrosis pathogenezisében. A β2-GPI és a Saccharomyces cerevisiae esetén keresztreagáló epitópok ismertek, ami arra enged következtetni, hogy az ASCA antitestek önmagukban, vagy az anti-β2-GPI miatti keresztreakció révén felerősítik a máj mikrovaszkulatúrájában zajló patológiás folyamatokat, azáltal, hogy felfüggesztik a β2-GPI vWF-függő thrombocyta aktivációra kifejtett gátló hatását. (IV) Az ASCA IgA vizsgálata során meghatározzuk annak szubtípus megoszlását. Majd ezen antitest tisztítását követően megvizsgáljuk annak hatását a vWF-függő thrombocyta adhézióra és aggregációra.

Mi a kutatás jelentősége?

A májcirrhosis klinikai megjelenése és a betegség lefolyása rendkívül változatos. A hullámzó betegséglefolyás során alapvetően két különböző klinikai entitással kell számolnunk, melyek esetén eltérő immun patogenetikai folyamatok kerülnek előtérbe. A májműködés akut rosszabbodása során kialakuló állapot (ACLF, krónikus májelégtelenségre rakódott akut dekompenzáció állapot) sok esetben szervelégtelenséggel jár, míg a májműködés krónikus rosszabbodása esetén a májfibrózis progressziója és a szövődmények kialakulása uralja a kórképet. Májcirrhosisban, a fokozott bakteriális transzlokáció (BT) emelkedett szisztémás endotoxin szintet eredményez, és a szisztémás bakteriális fertőzések kialakulásának fontos kockázati tényezője. A BT fontos szerepet játszik a májműködés akut és krónikus rosszabbodásában is. A BT folyamatának pontosabb megismerése segíthet (1) a májcirrhosis progressziójának és a bakteriális infekciók kialakulásának szempontjából leginkább veszélyeztetett betegcsoport elkülönítésében (2) a folyamatot megbízhatóan azonosító diagnosztikus laboratóriumi módszerek kidolgozásában illetőleg (3) nem antibiotikum alapú, célzott terápiás eljárások kidolgozásában. Mindezen célok elérésével a klinikusok számára lehetővé válhat a mindennapi klinikai gyakorlatban olyan kezelési és gondozási stratégiák kialakítása májcirrhosisban, melyek alkalmazása elősegíti a szövődmények kialakulásának megelőzését és azok uralását, késlelteti a betegség progresszióját és csökkenti a halálozást.
Májcirrhosisban nemcsak a betegség klinikai megjelenése és lefolyása, hanem a háttérben meghúzódó patogenetikai folyamatok is összetettek, azonban még számos vonatkozásban nem tisztázottak. Részletes vizsgálatuk azonban jelentőséggel bír. Kitűntetett figyelmet érdemelnek azok a tényezők, melyek együttesen tartják fenn az intestinális traktus és a máj gyulladását. A véralvadási rendszer dinamikus részét képezi mind a bél, mind pedig a máj krónikus gyulladásos folyamatainak és fontos eleme a májcirrhosis patogenezisének. Mivel kevés adat áll egyelőre csak rendelkezésre a véralvadási rendszer és az immunrendszer sejtjei között létrejövő kölcsönhatásokról, így azok ígéretes célpontjai lehetnek a májcirrhosissal kapcsolatos kutatásoknak.

Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

A májzsugor klinikai megjelenése és a betegség lefolyása rendkívül sokszínű képet mutat. A háttérben meghúzódó kóroki folyamatok is összetettek és még számos vonatkozásban nem tisztázottak. Részletes vizsgálatuk kiemelt jelentőséggel bír, különösen a bélrendszer és a máj gyulladását együttesen fenntartó tényezőké. A véralvadási rendszer és az immunrendszer sejtjeinek kölcsönhatása fontos szerepet játszik mind a bélben, mind pedig a májban zajló gyulladásos folyamatokban, így fontos eleme a májzsugor kóroktanának és ígéretes célpontja lehet a májzsugorral kapcsolatos kutatásoknak. A bélnyálkahártya fokozott áteresztő képessége májzsugorban az immunrendszer zavarával együtt a baktériumok és bakteriális termékek megnövekedéséhez vezet a vérkeringésben. Ez a folyamat pedig, mely bakteriális transzlokáció (BT) néven ismert, súlyosbítja a már zajló gyulladásos folyamatokat, elősegíti a májkárosodás kialakulását, szerepet játszik a májműködés heveny és krónikus rosszabbodásában. Jelen tanulmányban célul tűztük ki a BT folyamatának, valamint az immunrendszer és a véralvadás kölcsönhatásának pontosabb megismerését májzsugorban, mely segíthet a leginkább veszélyeztetett betegcsoportok elkülönítésében, új diagnosztikus laboratóriumi módszerek kidolgozásában és alapul szolgálhat új gyógyszerek kifejlesztéséhez. A mindennapi klinikai gyakorlat számára így lehetővé válhat olyan kezelési és beteggondozási stratégiák kialakítása a májzsugorban szenvedő betegekben, melyek alkalmazása elősegíti a szövődmények kialakulásának megelőzését és azok uralását, késlelteti a betegség romlását és csökkenti a halálozást.

Enhanced bacterial translocation (BT) due to disruption of gut barrier and mucosal immunity is characteristic for cirrhosis leading to uncontrolled pro-inflammatory processes and tissue damage of the liver. Inflammation and coagulation are closely linked, interdependent processes in the hepatic microvasculature and thought to be involved in the disease pathogenesis and also progression (hepatic fibrosis). However, only limited data are available about this intimate relationship. In the present study, we aim to explore more precisely various pathogenic processes leading to BT and further clarify the link between immune system and coagulation in cirrhosis. Based on our previous results, enhanced formation of IgA type antimicrobial- and auto-antibodies might be the result of augmented microbial challenge to the gut. Moreover, these antibodies might actively participate in the pathogenesis of cirrhosis. We are to characterize cirrhosis associated IgA type anti-microbial and auto-antibodies more precisely determining their subtypes and their antigen specificity. We are to elucidate exact formation mechanisms and define the involvement of gut-associated lymphoid tissue (GALT) in the enhanced specific IgA production. We are to justify that bacterial constituents are able to induce specific IgA production effecting B-cell proliferation and differentiation in cirrhosis. Using in vitro model system, we are to examine whether IgA type antibodies exaggerate the pathologic intrahepatic microvascular processes interfering von Willebrand factor (vWF) dependent platelet activation.

What is the major research question?

Elevated total serum immunoglobulin A (IgA) level is a characteristic feature of cirrhosis however, the formation and importance of IgA is not clearly understood.Our group established that the presence of certain IgA type antimicrobial- and autoantibodies were common in the sera of patients with cirrhosis. It may be hypothesized that formation of these antibodies is due to the bacterial translocation and involves the gut mucosal immune system. (I) We are to justify the piling of IgA producing plasma cells in the gut mucosa of the patients with cirrhosis, characterize the type of the local IgA subclass and demonstrate the enhanced epithelial expression of secretory component. (II) We demonstrate that certain bacterial constituents are able to induce enhanced specific IgA production effecting B-cell proliferation and differentiation in cirrhosis. (III) We are to identify the antigens of IgA class ANCAs. In previous clinical studies of our group, mostly IgA class serum antibodies were associated to progression of cirrhosis and development of diseases specific complications. These findings imply that IgA antibodies have a role in the pathogenesis of fibrosis. Presence of cross-reactive epitopes on Saccharomyces cerevisiae and β2-GPI has been raised the possibility that ASCA alone or in cross-reactivity with anti-β2-GPI exaggerate the pathologic intrahepatic microvascular processes in cirrhosis interfering with inhibitory effect of β2-GPI on VWF-dependent platelet adhesion and aggregation. (IV) For studying IgA type ASCA, we determine its subclass distribution. After separation of this antibody, we are to examine its effect on VWF-dependent platelet adhesion and aggregation.

Clinical presentation and disease course of cirrhosis are heterogeneous and variable over time. During the evolution of the disease, acute decompensation events associated with organ failure(s) – so-called acute-on chronic liver failure episodes – and chronic decompensation with progression of liver fibrosis and development of disease specific complications comprise distinct clinical entities with different immunopathology mechanisms. Enhanced BT associated with systemic endotoxemia and increased occurrences of systemic bacterial infections has substantial impact on both clinical situations. Thoroughly understanding of BT (1) may help identifying patients at high risk for disease progression and developing systemic infections in cirrhosis (2) may promote the development of reliable diagnostic measures of the process and (3) may support the development of non-antibiotic based, targeted therapeutic approaches those we are really lack of up to now. Achievement of these goals will hopefully assist clinicians in everyday practical decision-making when establish treatment and care strategy for the patients suffering with end-stage liver disease surmounting complications, delaying progression and diminishing mortality.
Complexity of cirrhosis, however, is not limited to clinical manifestation, but extends to the underlying pathogenic mechanisms. Until exact mechanisms of cirrhosis are not fully understood, each pathogenic component should be pursued in detail. In view of the fact that cirrhosis is typically chronic in nature factors responsible for the maintenance of liver and gut inflammation together should attract special attention. The coagulation system is a dynamic participant in the multifaceted process of chronic inflammation of gut-liver axis and represents an important pathogenic component of cirrhosis. Only limited data are available about the intimate relationship between coagulation system and different cells of immune system have made it a promising target for research in cirrhosis.

Until exact mechanisms of cirrhosis are not fully understood, each pathogenic component should be pursued in detail. Especially factors responsible for the maintenance of liver and gut inflammation together should attract special attention. The coagulation system in relationship with different cells of immune system plays a pivotal role in chronic inflammation of gut-liver axis and represents an important pathogenic component of cirrhosis, therefore making it a promising target for research in cirrhosis. Disruption of gut barrier and mucosal immunity in cirrhosis leads to the increase of bacteria and bacterial products in systemic circulation. This process is called enhanced bacterial translocation (BT). BT leads to uncontrolled pro-inflammatory processes and tissue damage of the liver, and contribute to the acute or chronic worsening of liver function. In the present study, we aim to explore more precisely various pathogenic processes leading to BT and further clarify the link between immune system and coagulation in cirrhosis. Thoroughly understanding these mechanisms may help identifying patients at high risk, may promote the development of new diagnostic laboratory methods and therapeutic drugs. Achievement of these goals will hopefully assist clinicians in everyday practical decision-making when establish treatment and care strategy for the patients suffering with end-stage liver disease surmounting complications, delaying progression and diminishing mortality.

Az IgA típusú F-aktin antitest(AAA) primér szkerotizáló kolagitiszben(PSC) és alkoholos cirrózisban is a betegek azon alcsoportját azonosította, akiknél progresszív betegséglefolyás várható. Az AAA jelenléte összefüggött a különböző mikrobiális antigénekre és enterocita károsodásra adott fokozott mucosalis immunválasszal. A citoszkeletális F-aktin egy új extracelluláris DAMP szignál, mely a dendritikus receptor DNGR-1/CLEC9Ahoz kötődik. A Syk-SFK szignalizáció antigén kereszt-prezentációt eredményez a CD8+T-sejtek felé. A bélben aktiválódott CD8+T-sejtek képesek bejutni májba az enterohepatikus útvonalon keresztül és egerekben immunmediált kolangitiszt indukáltak. Az IgA típusú anti-glikoprotein2(GP2) antitest a PSC új szerológiai markere, mely képes azonosítani a súlyos fenotípussal és a fokozott fibrogenezis miatt kedvezőtlen túléléssel rendelkező betegek alcsoportját. Az anti-GP2 a bél immunitás fehérje, a GP2-vel szembeni tolerancia elvesztését reprezentálja, mely utóbbi a FimH-pozitív baktériumokkal van kölcsönhatásban. A GP2-mediálta transzcitózis szükséges az ilyen típusú bakteriális antigénekkel szembeni, antigén-specifikus mukózális immunválasz beindulásához. A FimH a TLR 4 új ligandja, folyamatos aktivációja TGF-béta szignalizáción keresztül fokozott fibrózishoz vezet. Egy új IIFT renszer került kidolgozásra, mely alkalmas mind a 4 izotípusú aGPabs kimutatására Kitináz 3-szerű protein 1(CHI3L1)-t az IgA típusú atípusos P-ANCA új target antigénjeként azonosítottuk.

IgA type F-actin antibody(AAA) identified a patient subgroup in primary sclerosing cholangitis(PSC) and in alcoholic cirrhosis with progressive disease course and associated with enhanced mucosal immune response to various microbial antigens and enterocyte damage. The cytoskeletal F-actin is a novel extracellular DAMP signal coupled by dendritic receptor DNGR-1/CLEC9A. Syk-SFK signalling results in antigen cross-presentation to CD8+T-cells. CD8+T-cells being activated in gut are able to be recruited to liver via enterohepatic circuit and induce immune-mediated cholangitis in mice. IgA type anti-glycoprotein 2(GP2) antibodies was found as a novel marker of PSC and marker of severe disease phenotype associated with poor survival due to enhanced fibrogenesis. Anti-GP2 represents loss of tolerance to gut immunity protein, GP2 that interact with FimH-positive bacteria. GP2-mediated transcytosis is necessary for the initiation of antigen-specific mucosal immune responses against this type of bacterial antigen. FimH is a novel ligand of tolllike receptor 4. Sustained activation of TLR-4 leads to enhanced fibrosis through TGF-beta signalling. A new IIFT system was elaborated to test all the 4 type of aGPabs. Chitinase 3-like protein 1(CHI3L1) was identified as a novel neutrophil autoantigenic target of the IgA type atypical P-ANCA.
