Egyre több adat utal arra, hogy a paraneoplasztikus trombocitózis aktívan elősegítheti a tumoros metasztázis kialakulását. A patomechanizmus azonban még nem teljesen tisztázott, több hipotézis is felvetődött: 1. a magasabb trombocitaszámú betegeknél a vérlemezkék fokozzák a daganat növekedését és proangiogenetikus citokinek szekréciója révén az angiogenezist. 2. a trombociták az áttétképződésben úgy vesznek részt, hogy a keringő tumorsejteket beborítva védik ezeket a mechanikus károsodásoktól és a szervezet immunválaszától. 3. a trombocitózis paraneopláziás jelenség, mivel a daganat által szekretált citokinek trombopoézist indukálnak. Az azonban egyértelmű, hogy a trombocitózis nagyon jó prediktív faktor a túlélés vonatkozásában. Ismert tény, hogy a cukorbetegség jelentősen befolyásolja a vérlemezkék funkcióit. Hatására növekszik az átlagos vérlemezke térfogat, fokozódik a TXA2 képződés, CD40 ligandumok jelennek meg, illetve fokozódik az adhéziós molekulák (CD31, CD36, CD49b, CD62P, CD63), valamint felületi receptorok (GP Ib és GP IIb / IIIa) expressziója. A kutatás célja annak vizsgálata, hogy a szolid tumorokhoz társuló trombocitózis, mint prediktív faktor változik-e diabétesz jelenlétében. Nemcsak a különféle tumorokhoz társuló trombocitózis prognosztikai jelentőségét pontosítanánk, hanem annak patomechanizmusát is szeretnénk jobban megismerni. Különféle szolid tumorokban szeretnénk a következő megakariocitopoézist stimuláló citokineket vizsgálni: IL-1α, IL-4, IL-6, IL-8, IL-11, G-CSF, GM-CSF, SCF és FLT-3 ligand. Nemcsak a szérumszinteket vizsgálnánk, választ keresünk arra, hogy ezek a citokinek a primer tumorból vagy az azokat övező gyulladásos udvarból származnak-e.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

A kutatás során a szolid tumorokhoz társuló trombocitózis túlélést megmutató prediktív erejét, patomechanizmusát vizsgáljuk. Az eddigi közlemények a műtét előtti vérlemezkeszám szerepét vizsgálták. Jelen tanulmányunkban arra kerestünk választ, hogy mi történik akkor, ha a primer tumor eltávolításra kerül. Ha változik a vérlemezkeszám, akkor szoros kapcsolat igazolódik a trombocitózis és a tumor között. Eddig ovárium tumorokban vetették fel az IL-6 patomechanizmusban játszott szerepét. Az IL-1α, IL-4, IL-6, IL-8, IL-11, G-CSF, GM-CSF, SCF és FLT-3 ligand szérum szintek mérésével a különféle egyéb, megakariocitopoézist és trombopoézist stimuláló citokinek tumorokban játszott szerepét szeretnénk tisztázni. Nagy kérdés, hogy a különféle citokinek a tumorból vagy az azt övező gyulladásos udvarból származnak-e. A tumorsejtek citokin expressziójának vizsgálatával erre keresünk választ. A trombocitózis prediktív erejét több tényező befolyásolja. A különféle tényezők közül leginkább a cukorbetegség érdemel figyelmet/érdekes/nek lehet jelentősége. Ismert tény, hogy a diabétesz több tényezőn keresztül (átlagos vérlemezke térfogat, TXA2 képződés, CD40 ligandumok megjelenése, adhéziós molekulák: CD31, CD36, CD49b, CD62P, CD63, valamint felületi receptorok: GP Ib és GP IIb / IIIa expressziója) megváltoztatja a vérlemezkék funkcióját. Szeretnénk választ kapni arra, hogy a cukorbetegség befolyásolja-e a szolid tumorokhoz társuló trombocitózis prognosztikai erejét.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Ismert, hogy a paraneoplasztikus trombocitózis aktívan elősegítheti a tumoros metasztázis kialakulását, illetve rontja a hosszú távú túlélést. A kérdés tisztázása nagyon fontos, ugyanis ha ez az ok-okozati összefüggés fennáll, akkor a trombocitózis csökkentésével remélhetőleg lassíthatjuk az áttétképződést, és javíthatjuk a tumoros betegek életkilátását. Ebből a szempontból fontos az a nemrég közölt megfigyelés, mely szerint a plazma IL-6, a trombopoetin és a trombocitaszám között szoros összefüggés áll fenn. Állatkísérletek szerint az IL-6 szint csökkentésével (pl. siltuximab kezeléssel) kisebb lesz a vérlemezkeszám. Ez felveti annak a lehetőségét, hogy az IL-6 szint aktív változtatásával elérhető olyan kívánatos trombocitaszám, ami lassabb áttétképződéssel jár együtt. A fenti megfigyeléseket ovárium tumorokban végezték (Stone et al. Paraneoplastic thrombocytosis in ovarian cancer. N Engl J Med. 2012 16; 610-8. IF:51). Kutatásunkkal Stone és mtsai IL-6-al elért eredményeit szeretnénk kolorektális, gyomor-, nyelőcső-, primer máj- és hasnyálmirigy tumorokban megvizsgálni. Az IL-6 mellett kutatásunkat az összes megakariocitopoezist fokozó citokinre szeretnénk kiterjeszteni. Elsőként vizsgálnánk, hogy a primer tumor eltávolítása után, illetve cukorbetegség jelenlétében hogyan változik a trombocitózis mértéke. Úttörő jelleggel vizsgálnánk a primer tumorokhoz társuló trombocitózisokban a tumorok különféle citokin expresszióit. Kutatásunk korábbi nemzetközi kooperáció (Harvard /Boston, USA, Monmouth / Long Branch, USA, Technical University of Denmark /Dánia) folytatása. A kooperációt már több cikk fémjelzi (Baranyai et al. The comparison of thrombocytosis and platelet-lymphocyte ratio as potential prognostic markers in colorectal cancer. Thromb Haemost. 2014;111:483-90. IF.6,01). Több külföldi kohort kerül felállításra, így kutatásunk multicentrikus lesz. Kutatásunk alapján különböző tumorokban közelebb jutunk a daganat és a trombocitózis kapcsolatának megértéséhez. Pontosítani tudjuk a trombocitózist, mint prediktív faktort a primer tumorokban. Vizsgálataink olyan további kutatásoknak adhatja meg az alapját, melyek során új terápiás célpontot tudunk azonosítani.

Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

Az elmúlt évek kutatásai alapján egyre valószínűbbnek látszik, hogy a különféle daganatos betegségekben a magasabb vérlemezkeszám előrevetíti a gyakoribb áttétképződést és a betegek rosszabb életkilátásait. Nem tisztázott azonban, hogy ez csak egy melléklelet, vagy ténylegesen ok-okozati összefüggés áll fenn a magasabb vérlemezkeszám és a daganat között. Vizsgálatainkkal szeretnénk megérteni, hogy a magasabb vérlemezkeszám milyen mechanizmussal keletkezik a különféle daganatos betegségekben. Kutatásaink alapján közelebb kerülnénk a fenti kérdés megválaszolásához. Amennyiben az ok-okozati összefüggés igazolódik, akkor a vérlemezkeszám gyógyszeres csökkentésével javítani lehetne a tumoros betegek életkilátásain.

There is an increasing number of data suggesting that paraneoplastic thrombocytosis may promote metastatic progression. The pathomechanism is not completely elucidated yet and several hypotheses have been proposed; firstly platelets in patients with thrombocytosis induce tumor growth and angiogenesis via secretion of proangiogenic cytokines, secondly platelets play a role in metastatic progression by covering circulating tumor cells and protecting them from mechanic damage and the host’s immune response, thirdly thrombocytosis is a paraneoplastic phenomenon as cytokines secreted by the tumor induce thrombopoiesis. However, it is clear that thrombocytosis is an excellent predictive factor regarding survival. It is widely known that diabetes mellitus has a great impact on platelet functions; the mean platelet volume is increased, TXA2 production is increased, CD40 ligands appear and the expression of adhesion molecules (CD31, CD36, CD49b, CD62P, CD63) and surface receptors (GP Ib and GP IIb/ IIIa) is increased. The aim of the study is to evaluate whether the predictive power of thrombocytosis related to solid tumors will be changed if diabetes mellitus is present. Not only the prognostic significance of thrombocytosis related to different tumors would be specified but also its pathomechanism would be evaluated. The following cytokines stimulating megakariocytopoiesis would be assessed in different solid tumors: IL-1α, IL-4, IL-6, IL-8, IL-11, G-CSF, GM-CSF, SCF and FLT-3 ligand. Not only the serum level would be measured but we would also evaluate whether the cytokines come from the primary tumor or the inflammatory ring around the tumor.

What is the major research question?

The study evaluates the predictive power of thrombocytosis related to solid tumors regarding survival and pathomechanism. Reports have assessed the role of preoperative platelet count so far. In our present study we would like to evaluate the effect of tumor removal. If the platelet count changes then a strong correlation will be proved between thrombocytosis and the tumor. In the pathomechanism the role of IL-6 has been proposed in ovarian cancer. By measuring the serum level of IL-1α, IL-4, IL-6, IL-8, IL-11, G-CSF, GM-CSF, SCF and FLT-3 ligand, we would like to clarify the role of other cytokines stimulating megakariocytopoiesis and thrombopoiesis in tumors. It is also interesting whether the different cytokines are originated from the tumor itself or the inflammatory ring around the tumor. The predictive power of thrombocytosis is affected by several factors from which diabetes mellitus may be the most interesting. It is well known that diabetes mellitus modifies platelet function through several factors (mean platelet volume, TXA2 production, appearance of CD40 ligands, expression of adhesion molecules such as CD31, CD36, CD49b, CD62P, CD63 and of surface receptors such as GP Ib and GP IIb/ IIIa). We would like to see whether diabetes mellitus influences the prognostic power of thrombocytosis related to solid tumors.

What is the significance of the research?
Describe the new perspectives opened by the results achieved, including the scientific basics of potential societal applications. Please describe the unique strengths of your proposal in comparison to your domestic and international competitors in the given field.

It is known that paraneoplastic thrombocytosis can actively promote metastatic progression and impair long-term survival. It is very important to clarify this subject because if a causal relationship exists we may be able to slow down metastatization by decreasing thrombocytosis and may improve patients’ life expectancy. It is important in this aspect that a strong relationship between plasma IL-6, thrombopoietin and platelet count has been observed recently. Based on animal studies platelet count can be decreased by reducing IL-6 level (e.g. with siltuximab therapy). It raises the possibility that the active change of IL-6 level can result in a platelet count that is related to a slower metastatic progression. The observations above were made in ovarian cancer (Stone et al Paraneoplastic thrombocytosis in ovarian cancer. N Engl J Med. 2012 16; 610-8. IF: 51). In our study we would like to evaluate the findings of Stone et al in colorectal, esophageal, primary liver and pancreatic cancer. In addition to IL-6, we would like to extend our study to all cytokines stimulating megakariocytopoiesis. We would be first who evaluate how thrombocytosis changes following the removal of the primary tumor or in the presence of diabetes mellitus. Furthermore, we would also assess the expression of different cytokines of tumors in thrombocytosis related to primary tumors. Our study is the continuation of a previous international cooperation (Harvard / Boston, USA, Monmouth / Long Branch, USA, Technical University of Denmark, Dánia). Several articles hallmark this cooperation (Baranyai et al. the comparison if thrombocytosis and platelet-lymphocyte ratio as potential prognostic markers in colorectal cancer. Thromb Haemost. 2014; 111:483-90. IF: 6.01). Several external cohorts will be set up, thus, it will be a multicentrical study. Based on our study we may better understand the relationship of the tumor and thrombocytosis in different malignancies. We can also specify thrombocytosis as predictive marker in primary tumors. Our findings can be the starting point for further studies that aim to identify new therapeutic targets.

Based on recent studies it seems to be more and more likely that elevated platelet count predicts a higher incidence of metastatic progression and worse life expectancy of patients in different malignancies. However, it is not clear yet whether it is only an occasional finding or there is a causal relationship between the elevated platelet count and the tumor. We would like to understand with our evaluations what kind of underlying mechanisms lead to the development of thrombocytosis in different malignancies. Our study would help us to get closer to answer this question. If a causal relationship will be confirmed the life expectancy of tumor patients could be improved by medically decreasing the platelet count.

Pályázatunkban a thrombocytosis és a 2-es típusú diabetes mellitus (T2DM) szolid tumorokban betöltött szerepét vizsgáltuk. 1. Retrospektív vizsgálat: a. Egy új, a thrombocytaszám változásait egyénre szabott módon jelző mutatót definiáltunk (personalized indicator thrombocytosis, PIT). Kolorektális tumorban (CRC) szoros összefüggés igazolható a PIT és a staging, a túlélési idő, és a CRC-betegek általános állapota között. b. Emlőtumorban a thrombocytosis nem, viszont a T2DM jelentősen befolyásolja a betegek túlélését. 2. Prospektív vizsgálat: a. A CRC-k egy lehetséges új alcsoportját azonosítottuk, melyek immunhisztokémiailag kromogranin A-pozitívak (CgA+). A CgA+ tumorokra a vérzéses, míg a CgA– tumorokra a paraneoplasztikus thrombocytosis jellemző. A CgA+ betegek túlélése rosszabb. b. T2DM-ben jellemzőek a thrombocytákhoz köthető kóros elváltozások, és a T2DM jelenléte tumoros betegségekben rossz prognosztikai jel. A CRC és a T2DM együttes fennállása nem jár fokozott thrombocytosissal. c. CRC-ben a keringő CD40 ligand koncentrációja áttétes CRC-ben a legmagasabb. A diagnóziskori magas CD40 ligand szint rossz prognosztikai jel. 3. Genetikai vizsgálatainkban vér és CRC szövetmintákat elemeztünk. Nem-CRC cukorbetegekben a PCK2, MAPK9, CCND1, HMBS és TLR3 gének expressziója alacsonyabb, mint a tumoros cukorbetegeké. Cukorbeteg CRC betegekben szöveti génexpresszió-emelkedés volt igazolható a Wnt, Hippo, TNF, PIK3/Akt, és a thrombocyta-aktivációs jelátviteli útvonalakon.
Results in English
The role of thrombocytosis and type 2 diabetes mellitus (T2DM) on solid tumors had been studied. 1. Retrospective study: a. A new measure of platelet changes at an individual level, named personalized indicator thrombocytosis (PIT) was defined. In colorectal cancer (CRC), PIT values correlated with staging, survival times, and with the general condition of patients. b. T2DM affects, while thrombocytosis does not affect the survival of breast cancer patients. 2. Prospective study: a. A new subtype of CRC, which can be characterized by chromogranin A-positive (CgA+) immunohistochemistry, has been identified. CgA+ tumors are characterized by worse survival and reactive thrombocytosis, while CgA– tumors by paraneoplastic thrombocytosis. b. T2DM is associated with pathological platelet alterations, and its presence in malignant diseases is associated with poor survival. The coexistence of T2DM and CRC was not associated with increased thrombocytosis. c. Highest soluble CD40 level was measured in those CRC patients with metastases. Higher CD40 ligand levels at CRC-diagnosis predicted shorter survival times. 3. Blood and colon biopsy samples were collected for genetic analysis. Significant PCK2, MAPK9, CCND1, HMBS and TLR3 expression difference was found in the CRC + T2DM versus T2DM comparison of blood samples. And an increase in gene expression of Wnt, Hippo, TNF, PIK3/Akt, and platelet activation pathways was demonstrated within tissue samples of diabetic CRC patients.
