A tüdőrák a leggyakoribb rosszindulatú daganat mind férfiak, mind nők között Magyarországon. Bár a sebészi rezekció, a platina alapú kemoterápiás kombinációk, a sugárterápia és az újabban elérhető célzott terápiák javítottak a túlélési eredményeken, a nem kissejtes tüdőrákban (NSCLC) szenvedő betegek túlélés növekedési görbéje plató fázisához érkezett. Számos tirozin-kináz inhibitort is teszteltek, de az ezen kezelések hatékonyságát mejósolni képes biomarkerek egyelőre nem elérhetőek. Feltételezzük, hogy megértve az adott hatóanyag hatásmechanizmusát, annak hatékonyságát előrejelző és biztonságos stratégiákat lehet kidolgozni tüdőrákos betegek számára "imaging tömegspektrometria" (IMS) és biobank létesítmények létrehozásával az alábbiakban részletezett együttműködésünk segítségével.

Célunk, hogy tirozin-kináz-inhibitorok támadáspontjainak meghatározásával, koreai partnerünk segítségével és az IMS módszer alkalmazásával humán daganatminták vizsgálatával új támadáspontokat azonosítsunk. A kísérletes munka a koreai létesítménnyel és azon keresztül a svédországi Lundi Egyetemmel történik (utóbbi helyszínen szintén elkülönítettek forrásokat e törekvésre).

• lokalizálni és számszerűsíteni a gyógyszereket és metabolitokat 5-10 um szöveti felbontásban

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

A projekt a gyógyszeres kezelések pontosabb megértését, a mechanizmus alátámasztását szolgálja a tirozin-kináz inhibitor család újabb tagjainak azonosításával tüdőtumoros betegek számára. A gyógyszer-célpont affinitás kölcsönhatást és a protein-target kötési specificitást és redundanciát vizsgáljuk a hatásosság és biztonságos gyógyszer felhasználás elérése érdekében. Az integrált rendszer először teszi lehetővé a kombinált makroszkopikus és nanoméretű képalkotást a gyógyszer és mechanikai információ tekintetében, megjósolva a potenciális válaszadók körét. DART (Drug Affinity Responsive Target affinity) technológiát alkalmazunk, amely a fehérje-vegyület interakciókat képadatokkal kombinálva segíti a mellékhatások és a nemkívánatos események előrejelzését, valamint a hatásosság predikcióját.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Egy daganatos beteg személyre szabott kezelése átlagosan 100K EUR / év költségterhet jelen ma Európában. A beteg és az egészségügyi költségvetés, valamint a klinikai döntéshozó együttesen mérlegeli, hogy a beteg megkaphatja-e a terápiát. A válaszadó páciensek kiválasztása ezért az egyik legfontosabb kihívás ma az EU-ban, ahol az daganatellenes kezelésekre szánt összeg 50%-a feleslegesen kerül elköltésre az adott gyógyerre nem reagáló betegek kezelésével.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

A gyógyszeripar által kifejtett nyomás vagy a széles marketing potenciált jelentő relatíve olcsóbb fejlesztések irányába mutat, vagy más stratégia szerint kisebb szegmens megcélzásával személyre szabott, drága kezelési eljárások kidolgozására koncentrál. Azonban ezen megközelítések egyike sem bizonyul tökéletesnek, és egyik sem nyújt megoldást önmagában a sikeres terápia meghatározására.
A fejlesztések egyik iránya lehet, ha a gyógyszerek felhasználásának, útjának pontosabb megértésével azok hatásának előrejelzésére keresünk választ. Ehhez szükséges lesz az orvosi képalkotás molekuláris szinten történő alkalmazása a hatóanyagok azonosítására a szervezeten, szerveken, szöveteken, és sejteken belül. Kihívást jelent, hogy jelenleg nincsen általánosan elfogadott szabványosított mérési módszer, amely a gyógyszerek szövetbeli eloszlását megmutatná, így egyben hatékonyságukat és biztonságosságukat előre jelezné. Jelenleg költséges képalkotó módszerek elérhetőek, sokszor radioaktív jelzéssel, amik a késői klinikai tesztelés fázisában állnak. Munkacsoportunk együttműködésben először volt képes két betegségben (tüdőrák és COPD) jelöletlen gyógyszer szöveti azonosítására tömegspektrometriás eljárás segítségével.
Jelen vizsgálatunkkal a tüdőrák kezelésének fejlesztésére teszünk kísérletet, azonosítani szándékozunk új gyógyszereket, és azok hatásmechanizmusát is feltérképezzük.

Lung cancer is the most common cancer in men and women in Hungary. The treatment options and the outcome are modest and clinical trials developing strategies and multimodal therapy today proved to provide limited benefit.

Tyrosin kinase inhibitors have been tested but the biomarkers were missing. Our hypothesis is that by understanding the mode of drug action mechanisms, efficacy prediction and safety, strategies can be developed in lung cancer by Imaging Mass Spectrometry (IMS) and Biobank establishments to improve patient treatments.

Here we aim to develop and provide further targets for tyrosin kinase inhibitors with the unique opportunity of Korean collaboration using the IMS method utilizing patient samples from Hungary. Laboratory intensive work-up will be provided by the Korean facility in cooperation with Lund University in Sweden who also allocated funds to support this endeavor.

OBJECTIVES
• Establishment of comprehensive Lung Cancer Biobank hosting tissue and blood fractions from patient cohorts
• Establishment of comprehensive clinical follow-up database
• Localize and quantify Drug and metabolites at 5-10 µm tissue resolution
• Identify Lung Cancer drug candidates and Targets by DART
• Launch a new Mass Spectrometry Platform coupled with Microscopic imaging for point by point sampling to identify sub-cellular localization of drugs and metabolites.
• Novel Imaging software to co-localize Drug and target using algorithms and user modules in joint development

What is the major research question?

The project will provide drug mechanism evidence by drug-target affinity interaction and protein target binding specificity and redundancy, the key elements of efficacy and safe drug use. The integrated system for the first time allows the combination of macroscopic & nanoscale imaging with drug and mechanistic information that predicts responders. DART (Drug Affinity Responsive Target Stability) technology, providing protein-compound interactions-, in combination with imaging Data- will aid in the prediction of side effects and adverse events that often ends with mortality.

What is the significance of the research?
Describe the new perspectives opened by the results achieved, including the scientific basics of potential societal applications. Please describe the unique strengths of your proposal in comparison to your domestic and international competitors in the given field.

There is a great urgent unmet need today to develop affordable new drugs to treat the deadly diseases of mankind; from cancer to diabetes, from cardiovascular disease to antibiotic resistant infections. Another area of concern is the actual cost of the drug to the patient. In Cancer today, a Personalized Medicine treatment (PM) will cost 100K EUR/year in Europe. The importance to the patient and healthcare budget is the predicament of the clinical decision maker to decide just who should receive this therapy. To identify the patient responder is the challenge for the society; where in EU 50% of the healthcare costs do not provide impact to the cancer patient. Improvement in imaging technology development would aid in providing the right medicine to the right patient at the right time point. This need is manifested today by great societal pressures such as poverty and lack of resources, an overwhelmed social and medical care system, the ever increasing numbers of aged people, and the global political and global financial landscapes. Drug responders and those intolerant to the drug (4-8% mortality in PM patients, reportable event) are key concerns of modern healthcare. Finding better means for treating disease is at the heart of the development programs of all major pharmaceutical manufactures whose current portfolios of marketed drug products is falling dramatically as their pipelines diminish and dry up. New strategies and work plans are needed to address these concerns.
Here we aim to develop a method which helps the above concerns and we have our unique collaboration to overcome the technical issues and get closer to the information of drug efficacy in lung cancer.

The pharmaceutical industry has answered by becoming more concentrated in their activities around certain disease areas of high marketing potential. Other strategies include the development of niche products to fulfill the needs of personalized medicine approaches to treating disease. However none of these strategies has succeeded in either providing solutions for successful therapy of any of the great diseases of mankind mentioned above.
One of the ways to meet the need for obtaining better understandings about the fate of drugs after delivery is to directly measure the compound within the tissue compartments where it has been transported. New methods in medical imaging that identify drugs in situ are highly sought after. The problem today is that we do not have a generally accepted standardized method for measuring drug dispersal in tissue, providing evidence of efficacy and safety. Currently costly imaging platforms requiring radioactive labeling are used at late phases of clinical testing. We were the first research team in the world, in collaboration, in two disease studies to report the identification of an unlabeled drug within local tissue sites of diseased lung cancer and COPD patients using a new technology MSI.
For lung cancer, we are taking a step further, to identify personalized medical approaches to overcome this disease.

Munkánkban a koreai-svéd-magyar együttműködésben tervezett projekt végrehajtása folyik. Rendszeres személyes megbeszélést tartottunk minden évben, amíg erre a COVID-19 pandémia előtt lehetőségünk volt, a koreai és svéd partnerek is eljöttek Budapestre nyaranta. A tüdőgyógyászati téma miatt a 2020. évben számos megvalósításban akadályba ütköztünk, leginkább az intézet hozzáférhetetlensége miatt, amit igyekeztünk munkánkkal ellensúlyozni. A bioaktív kis molekulák célfehérjéinek azonosítása kulcsfontosságú lépés a gyógyszer hatásmódjának megértésében és az adott fenotípusért felelős mögöttes mechanizmusok kezelésében. Áttekintettük a lépéseket, amelyek alapján a proteomikát sikeresen alkalmaztuk a módosítatlan és a ligandummal módosított bioaktív kis molekulák célfehérje profiljának tisztázására. Tüdőtumoros vizsgálatainkban különböző projektek megvalósítása történt: ALK génátrendeződést hordozó tüdőtumorokban a tumor heterogenitás vizsgálata, atypusos carcinoidokban klinikai és digitális patológiai elemzés, primer tumorok és összes áttétük párhuzamos vizsgálata folyik svéd partnereinkkel, primer korai és késői tüdőrákokat folyadékkromatográfiás-tandem tömegspektrometriával elemeztük, a DNS-polimeráz alfa B alegységet (POLA2) új Erlotinib-kötő fehérjeként azonosítottuk. A modulált elektro-hipertermiát értékeltük a pulmonalis metasztatikus melanoma kezelésében. Elemeztük a SARS-CoV-2 spike glikoprotein gátlását antivirális gyógyszerekkel és ezek kombinációival.
Results in English
In our work, the project was performed in the Korean-Swedish-Hungarian cooperation. We held regular face-to-face meetings every year until we had the opportunity to do so before the COVID-19 pandemic, with Korean and Swedish partners also coming to Budapest in the summer. Due to the topic of pulmonary medicine, we encountered obstacles in many implementations in 2020, mostly due to the inaccessibility of the institute, which we tried to counterbalance with our work. Identification of target proteins for bioactive small molecules is a key step in understanding the mode of action of the drug and in addressing the underlying mechanisms responsible for a given phenotype. We reviewed the steps by which proteomics were successfully used to elucidate the target protein profile of unmodified and ligand-modified bioactive small molecules. Various projects have been implemented in our lung tumor studies: examination of tumor heterogeneity in lung tumors with ALK gene rearrangement, clinical and digital pathological analysis in atypical carcinoids, parallel examination of primary tumors and all their metastases with DNA partners, primary early and late lung cancer alpha B subunit (POLA2) was identified as a novel Erlotinib binding protein. Modulated electro-hyperthermia was evaluated in the treatment of pulmonary metastatic melanoma. Inhibition of SARS-CoV-2 spike glycoprotein by antiviral drugs and combinations thereof was analyzed.
