A hidroxiapatit (HAp) egy nagyon széleskörűen alkalmazható biokerámia anyag. Fő felhasználási területei közé tartozik az orvostechnológia és a gyógyászat. Alkalmazzák ortopédiai műtétek során csontpótló anyagokként, fogorvosi implantációnál, illetve fémimplantátumok esetében bevonatokként. Kiváló bioaktivitással rendelkezik, mivel megfelelő alapot szolgáltat az implantátum és a környező élő szövetek valamint a csont között. Elősegíti a csontsejtek megtapadását az implantátum felületén. Annak ellenére azonban, hogy kedvezőtlen, gyenge a mechanikai tulajdonsága, bevonatként még sikeresen alkalmazható a fémimplantátumok felületén. Napjainkban intenzív kutatások folynak olyan bevonatok kifejlesztésére, melyek megnövelik az alap fémes implantátum biokompatibilitását és emellett még antibakteriális jelleggel is rendelkeznek. A hidroxiapatit fázis módosítása különféle bioaktív és antibakteriális hatású ionokkal még jobban javíthatja a bevonat mechanikai, kémiai és biológiai tulajdonságait. A pályázat fő célkitűzése olyan ionokkal adalékolt HAp-biopolimer kompozit bevonatok kifejlesztése, melyek biológiailag lebomlanak. Munkánk során vizsgáljuk a bevonatok szerkezetét, morfológiáját, valamint a különféle adalékoló ionok eloszlását és beépülését. Elemezzük az adalékoló elemek kioldódásának sebességét és mechanizmusát biológiai oldatokban, ami meghatározza a bevonat biológiailag lebomló jellegét. Kísérleteink során szisztematikusan, ellenőrzött körülmények között változtatjuk a különböző adalékoló elemek mennyiségét és arányát, elemezzük és kiértékeljük azok hatását a bevonat szerkezetére, tulajdonságaira.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

A hidroxiapatit kiváló biokompatibilitása és a kedvező biológiai környezet az emberi szervezetben elősegíti a káros baktériumok megtapadását az implantátumon, melyek biofilmet képezhetnek. Ez az implantátum elfertőződéséhez és kilökődéséhez vezethet. Elmondható, hogy az operációt követő elfertőződések még manapság is súlyos problémát jelentenek, amit megoldásra szorul. Rengeteg tudományos kutatómunka foglalkozik biológiailag lebomló, biokompatibilis bevonatok kifejlesztésével, azonban csak kevés szentel figyelmet a különböző adalékoló elemek beépülési mechanizmusának, jellegének, arányuknak és eloszlásuknak a tanulmányozására illetve mélyebb feltérképezésére. Szintén hiányzik kiterjedt kutatás az elemek kioldódási mechanizmusának és sebességének feltérképezésére és optimalizálására.
A HAp bevonatok kristályossági foka és fázisösszetétele függ az előállítási technológiáktól és a paraméterektől. Az amorf fázis jelenléte meggyorsítja az oldódási folyamatokat, ami befolyásolja a bevonat lebomlási sebességét. A HAp kémiai tulajdonságai még jobban hasonlóvá tehetők a természetes csontéhoz különféle ionok beépítésével a HAp kristályszerkezetébe. Ezek az ionok lehetnek karbonátok, szilícium, magnézium és cink. A kutatás hipotézise az, hogy a megfelelő bioaktív és antibakteriális elemek optimalizált arányban és homogénen eloszlatva és beépítve a HAp szerkezetébe a bevonat biológiai lebomlása befolyásolható és az igényekhez képest alakítható, szabályozható. Ennek igazolására kísérleteinkben változtatjuk a különféle adalékoló elemek arányát a bevonatban ellenőrzött körülmények között és mérjük, kiértékeljük az ezzel összefüggő morfológiai változásokat a bevonat szerkezetében.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

Alapkutatásként a pályázat fő eredménye olyan bevonatok kifejlesztése a kereskedelemben használatos fémes implantátumok felületére, melyek egyszerre bioaktívak és antibakteriálisak. Nagy szükség van ilyen bevonatok kialakítására és alkalmazására, mivel az operációt követő elfertőződések sajnos még manapság is súlyos problémát jelentenek világszerte. A kifejlesztett, elemekkel adalékolt HAp és HAp/biopolimer kompozit bevonatok fizikai-kémiai és mechanikai tulajdonságai és nanoszerkezetük közötti összefüggés mélyebb megértése elősegíti a megfelelő tulajdonságokkal rendelkező olyan bevonatok megtervezését és kivitelezését, melyek biológiailag lebomlanak és használhatók az orvostechnológiában, kórházakban. A bevonatok fizikai, kémiai és biológiai tulajdonságai optimalizálással úgy alakíthatók, hogy teljesen hasonlóvá váljanak a természetes csontokat alkotó apatitok kémiai tulajdonságaihoz és oldhatóságához. Az optimalizált bevonatokkal megelőzhetők a csontpótlást, illetve implantációt követő gyulladásos reakciók. Kutatásunkban vizsgáljuk a bioaktív és antibakteriális adalékoló elemek beépülési mechanizmusát a HAp kristályrácsába, valamint a bevonat lebomlásának/oldódásának a mechanizmusát és sebességét. A jelenleg elérhető tudományos irodalomban hasonló kutatási jelentések, publikációk nem találhatók. A megfelelő elemekkel adalékolt HAp bevonatok valamint a HAp/biopolimer kompozit bevonatok kémiai, biológiai és lebomlási jellegének összehasonlításával lehetőség nyílik az adalékoló elemek optimális koncentrációjának és arányának megtalálására és a legjobb tulajdonságokkal rendelkező bevonatok elérésére, a legnagyobb bioaktivitással és antibakteriális hatással. A kutatási eredmények kiértékelésével lehetőség nyílik egy újszerű és alternatív technológia kidolgozására ezen bevonatok kialakításához. Ezen kívül, mivel a bevonatok szerkezete, ezáltal a tulajdonságai változtathatóak az előállítási paraméterekkel, a bevonatok kémiai és biológiai jellemzői tetszőlegesen alakíthatóak bármilyen speciális igényhez igazodva.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

A hidroxiapatit az egyik legszéleskörűbben alkalmazható anyag és számos előnyös tulajdonságának köszönhetően a társadalom számára is hasznos. A legelterjedtebben a környezetiparban, az agráriparban és az egészségügyben alkalmazzák. Egyre növekvő érdeklődésre tart számot az orvostechnológia területén is, mint biokerámia alapanyag. Ez annak köszönhető, hogy összetétele, kristályszerkezete, ezáltal a fizikai, kémiai tulajdonságai valamint biológiai hatása nagyon hasonló a természetes csontokéhoz. A hidroxiapatit fizikai-kémiai tulajdonságai jelentős mértékben függenek a kristályszerkezettől és a szemcsék méretétől, formájától.
Köztudott, hogy manapság súlyos problémát jelentenek az operációt követő elfertőződések világszerte. Ennek hatására az implantátum kilökődik a szervezetből, ezáltal újabb rekonstrukciós műtét válik szükségessé. Ez nagy terhet jelent mind a páciensek, mind az egészségügyi rendszer számára, mivel nagyon költséges. A probléma kiküszöbölhető bioaktív és antibakteriális hatású elemek beépítésével a hidroxiapatit kristályszerkezetébe. Ez az elemekkel adalékolt hidroxiapatit előnyösen alkalmazható bevonat formájában a fémes implantátumok felületén. A projekt fő eredménye olyan bevonat kialakítása, amely bioaktív, elősegíti a csontosodást, valamint van antibakteriális hatása is, tehát megakadályozza a káros baktériumok megtapadását az implantátum felületén. Az elemekkel adalékolt hidroxiapatit nanoszerkezetének mélyebb megértése elősegíti a bevonat fizikai, kémiai és biológiai tulajdonságainak az optimalizálását. Ezek az újszerű bevonatok jelentősen javíthatják a páciensek életszínvonalát azáltal, hogy lerövidítik az operációt követő gyógyulási folyamatot.

Hydroxyapatite (HAp) is a bioceramic that has been extensively used for biomedical application such as orthopaedic, dental implant, coatings on hard tissues and as fillers for bone defects. It has bioactivity and bioaffinity to provide a strong interaction and attachment between a replacement implant and the surrounding tissue. Despite its low mechanical properties the HAp can be applied successfully as coatings on different load bearing metallic implants thus enhancing their biocompatibility. Up to now, there are enormous efforts to develop such coatings that can enhance the biocompatibility properties of metallic implant materials and even provide antimicrobial effect. The ionic modification of HAp phases can improve the biocompatibility property of materials even more. The main goal of this proposal is to develop biodegradable, multi-element doped hydroxyapatite-biopolymer composite coatings onto implant materials. The structure of HAp-biopolymer composites as well as the incorporation rate and distribution of different doping elements are going to be investigated. The dissolution mechanisms and corrosion rate of doping elements in different biological conditions will be examined which determine the biodegradability of coatings. The bioresorbability of coating can be changed by varying the concentrations of doping elements. In our experiments, we will vary the ratio of different doping elements such as organic silver, Zn, Mg, Sr, in synthetic apatites under controlled conditions and measure and interpret the related structural changes.

What is the major research question?

The excellent bioactivity of hydroxyapatite and the appropriate conditions in the human body are favourable to the adhesion and reproduction of bacteria on its surface, leading to wound infection and implant failure. Hence, the post-operative infections are still serious problems that need to be solved. While there is extended literature on preparing biodegradable coatings with good biocompatibility there are still little work on understanding the incorporation rate and distribution mechanisms of doping elements, on optimization of their concentrations and also on the dissolution mechanisms and rates of these different elements. These crucial pieces of information are also desired to tailor appropriate implant materials for biomedical applications. The crystallinity and phase composition of the HAp can vary with the coating process and preparation parameters. The presence of amorphous phase accelerates the coating dissolution, which can affect the the long-term influence of the coating. HAp’s chemistry can be made even closer to bone mineral with the substitution of other ions into the lattice, including carbonate, silicon, magnesium, strontium and zinc. We hypothesize that with optimization of the concentration of doping elements and by achieving homogeneous distribution, the dissolution rate, thus the bioresorbability of coating can be adjusted according to any specific requirements. Thus, our aim is to vary experimentally the ratio of different doping elements in synthetic apatites under controlled conditions and measure and interpret the related structural changes.

What is the significance of the research?

As a fundamental research, the main output of the present proposal is to develop such coatings that simultaneously possess bioactive and antibacterial properties. This is a very crucial issue today due to the high post-operative infection rates worldwide and the growing number of orthopeadic surgeries.
The relationship between the physico-chemical and mechanical properties and nanostructure of developed novel multi-element doped HAp coatings and biopolymer composite coatings will inspire the design and synthesis of implant materials with bioactive and biodegradable layer on their surface for medical applications and clinical use. These coatings can be adapted to mimic the crystal size, surface chemistry and solubility of natural bone apatite. This is essential for the successful bone grafting, and for the reduction of inflammatory reactions after implantations.
We will also explore the mechanism of incorporation of different bioactive and antibacterial elements into the HAp crystal structure as well as the mechanism and rate of bioresorption/biodegradation of developed coatings. These kinds of experiments and reports cannot be found in the scientific literature up to date. The comparison of the bioresorption mechanism and rate of the multi-element doped HAp and HAp/biopolymer composites will also help in finding the optimum concentration and ratio of doping elements to achieve coatings with best bioactivity and with antibacterial effect. Gathering all the knowledge from the experimental results we can propose a novel and alternative route for preparing bioactive and antibacterial coatings on metallic implant surfaces.
Moreover, since the composition and particle size can vary by changing the preparation parameters, the chemical and biological properties of coatings can adjust to any special needs or applications.

The hydroxyapatite has several applications that are beneficial for the society. It is widely used in environmental industry, in agriculture and it is attracting more and more interest as biomaterial for use in prosthetic applications due to its similarity in size, crystallography and chemical composition to human hard tissue. Bone is largely composed of a form of this mineral. The physico-chemical properties of hydroxyapatite nanocrystals largely depend on their crystal structure, grain size and form.
It is well known that post-operative infections are still serious problems that need to be solved. Due to post-operative infections, the implants might be rejected by the body and reconstructive surgeries are needed. This is very expensive and it puts financial burden on the healthcare system owing to the growing number of orthopeadic surgeries. Therefore, incorporation of antibacterial particles and other bioactive minerals into the hydroxyapatite coating has important clinical value to provide antibacterial property as well to the medical implants. The main output of the present proposal is to develop such coating that is bioactive, which means that it can promote the bone cell growth on implant surface and it has also antibacterial properties and prevent the adhe sion of infectious bacteria.
Understanding the nanostructure of multi-element doped hydroxyapatite will allow the fine-tuning of the physico-chemical properties of deposited bioceramic and composite coatings on implants. These novel coatings will provide superior biocompatibility which, used as implants may significantly improve everyday life in the XXI. century and provide better life conditions to patients.

Kutatómunkánk során sikeresen kifejlesztünk implantátum felületekre jól tapadó és biológiailag lebomló bioaktív ionokkal adalékolt apatit/biopolimer kompozit bevonatokat. A különféle bevonattípusok fizikai, kémiai és biológiai jellemzőit átfogó módon meghatároztuk és összehasonlítottuk. Az eredmények igazolták, hogy ezek a kompozit bevonatok megfelelő kémiai és fizikai tulajdonságokkal rendelkeznek, és ideálisan alkalmazhatók a közép- és hosszú távú implantátumok esetében. Mind a por, mind a kompozit bevonatok porózus szerkezetűek, nagy fajlagos felülettel rendelkeztek, és az apatit eloszlása egyenletes volt az összes alkalmazott polimer mátrixban. A közép- és hosszú távú implantátumok esetében a legkritikusabb tényező az implantátum anyagának tökéletes integrációja anélkül, hogy az emberi szervezet kilökné. Ebben az esetben nagyon fontos az implantáció korai szakasza. Az implantátumok integrálása és a csontok regenerálódása után a bevonatok in vivo lebomlása előnyös. Ezek a biokompozit bevonatok a legígéretesebb anyagok erre, mivel a porózus mikroszerkezetüknek és a természetes csontokhoz hasonló nagy fajlagos felületüknek köszönhetően elősegíthetik a csontsejtek adhézióját és növekedését. Ezen túlmenően, ha az csontintegráció befejeződött, a réteg lebomlik anélkül, hogy az emberi testet károsítaná. Ezen túlmenően a kompozit minták elkészítésének reprodukálhatósága megfelelő volt és a rétegek tapadása megfelelő volt.
Results in English
Well-adherent and biodegradable bioactive ions added apatite/biopolymer composite coatings were successfully developed onto implant surfaces in our research work. Thorough characterization methods were performed on all types of coatings and the results supported that these composite coatings had suitable chemical and physical properties and can be ideal candidates for implant coatings in middle- and long-term implantations. Both the powder and the composite coatings were very porous with a high specific surface area, and the apatite distribution was homogeneous in the used polymer matrix. In the cases of middle- and long-term implants the most critical factor is the perfect integration of the implant material without being rejected by the human body. In this case, the early stage of the implantation is very important. After the implants have been integrated and the bones repaired, the in vivo degradation of coatings is beneficial. These biocomposite coatings are the most promising solutions since they can promote bone cell adhesion and growth owing to their porous microstructure and large specific surface area which is similar to the natural bones. In addition, once the osseointegration is completed, the layer will degrade without causing any harm to the human body. Moreover, the reproducibility of the preparation of composite samples was appropriate, and the adherence of coatings was efficient.
