A szisztémás lupus erythematosus (SLE) pathogenesisének kutatásában kiemelt jelentőségű, hogy az adott genetikai háttér mellett milyen indukáló tényezők képesek az autoreaktív mechanizmust elindítani, fenntartani, aktiválni. Egyik ilyen kiváló tényező lehet a vírusfertőzés, melyet a chip technikával végzett Interferon-alfa/béta microarray vizsgálatok megalapozottan bizonyítanak. Igazolták, hogy SLE-ben interferon-alfa aktivált géneket, ún. „interferon-szignatúrát” lehet kimutatni
A Sjögren-szindróma (Ss) klinikai tüneteiben is hordoz hasonlóságokat az SLE-hez (eltekintve az autoimmun sialoadenítistől), hiszen a szisztémás/extraglandularis manifesztációk kiváltásában az autoantitestek (anti-Ss-A, anti-ss-B) és az immunkomplexek jelentősége nem kérdéses. Ennek ellenére a Sjögren-szindróma pathogenezisének kutatása során eddig mindössze egy nem közölt irodalmi adat utal arra, hogy az Interferon-alfa stimulatív faktorként szerepelhet.
Az utóbbi években került az érdeklődés középpontjába a regulatórikus sejtek szerepének vizsgálata az immuntolerancia és az autoimmunitás vonatkozásában. Nagyobb figyelem fordítódott a thymusban képződő CD4+/CD25+ regulatórikus T-sejtekre, melyek depletiója esetén spontán autoimmun állatmodellek hozhatók létre.
A pályázatban vázolt kutatási tervünkben olyan betegeket szeretnénk vizsgálni, akikben mindkét betegség (SLE + Ss) megjelenik.
Választ szeretnénk arra kapni, hogy az Interferon-alfa aktivált gének, valamint az immunregulatív sejtek funkcionális változásai szerepet játszhatnak –e a betegségek pathogenezisében. Az SLE és Sjögren-szindróma társulása hajlamosít –e egyéb autoimmun anomáliák, vagy betegségek kialakulására, illetve hátterében genetikai tényezők szerepe igazolható –e. A betegségek szövődményei tekintetében elsősorban az akcelerált atherosclerosis, valamint a lymphoma képződés fokozódására kapunk adatokat, melyeket eddig a két kórkép társulásakor nem vizsgáltak.
Summary
At the investigation of pathogenesis of systemic lupus erythematosus (SLE), it has high importance to know what kind of inductive elements are able to launch, maintain, and activate an autoreactive mechanism in the presence of a given genetic background. Examinations with Interferon α/β microarray, using chip technique, have clearly showed the potential role of virus infection in this process. The presence of Interferon α activated genes in SLE was also proved. Features of Sjögren’s syndrome (Ss) have similarities to SLE regarding to clinical symptom’s (except autoimmun sialoadenitis), because there can be no doubt about the critical role of the autoantibodies (anti-SSA, anti-SSB) and immuncomplexes in the development of systemic/extraglandular manifestations. In contrast only one unpublicated data reveals in the literature, that Interferon α might be a stimulative factor in the pathogenesis of Sjögren's syndrome.
Investigation of the regulatory cell functions regarding to the immuntolerance and autoimmunity was taken in the center of interest in the last few years. There is a higher attention to the thymal CD4+/CD25+ regulatory T-cells, as their depletion resulted spontaneously animals, modeling autoimmun disorders.
In the work schedule presented in the application, we would like to examine patients, who have both (SLE + Ss) diseases.
We would like to know, whether the Interferon α activated genes, or the functional changes of the immunregulative cells play an important role in the pathogenesis of the diseases. Does the association of SLE and Sjögren's syndrome predict to the development of other autoimmun disorders, or diseases, and what are the roles of the genetic factors in this process. We will have data about accelerated atherosclerosis, and about the risk of lymphoma development, regarding to the complications of the diseases, which were not examined at the association of these two diseases yet.

A zsírban-oldódó vitaminok immunmodulans hatását tanulmányoztuk 25 primer Sjögren-szindrómás (SS) betegben.Megállapítottuk, hogy a zsírban-oldódó vitaminoknak immunmodulans hatása van,és szerepük van az autoimmun betegségek patogenezisében.Az A és E vitamin szintje korrelál az immunregulatorikus sejtek számával és befolyásolhatja az autoimmun folyamatot.(Rheumatol 2010;49:211-17.) Megállapítottuk,hogy a Graves-betegség és a Hashimoto thyreoiditis kialakulása megelőzheti, de követheti is az SS jelentkezését.Mivel a SS-val társult autoimmun thyreoiditisek korábbi életkorban alakulnak ki fontos annak a további vizsgálata, hogy a SS valóban perdisponáló faktor-e az autoimmun thyreoiditis kifejlődésében.(Thyroid 2009;19(1):39-45.) Meghatároztuk az Interferon omega (IFN omega) szérumkoncentrációjának korrelációját az APSI és más szisztémás autoimmun betegségekkel.Az AIRE gén egy új mutációja, a c.1344 delC-t tudtuk kimutatni. Úgy tűnik, az anti-IFNomega antitestek a nagyon korai életkorban jelennek meg és segítenek abban, hogy az APSI-et differenciálni tudjuk más szisztémás autoimmun kórképtől.(Clin Endocrinol(Oxf); 2009Oct 26.[Epub ahead of print]) A SS nagyon gyakran társul más autoimmun betegségekkel. Az utóbbi évek kutatásai nagyban hozzájárultak a betegség etiopathogenezisének megértéséhez. We have investigated the immunomodulating role of fat-soluble vitamins in 25 patients with primary Sjögren’s syndrome (SS).We have stated that fat-soluble vitamins possess immunoregulatory properties and have been implicated in the development of autoimmune diseases. The levels of vitamin A and E correlate with immunoregulatory cell counts, and may influence the autoimmune processes.(Rheumatol 2010;49:211-217.) We noted that both Graves disease and Hashimoto-thyroiditis can appear either before or after the onset of SS. Since most cases of SS predate the appearance of autoimmune thyroid diseases it is important to determine if SS is a predisposing factor for the development of autoimmune thyroiditis. (Thyroid 2009; 19(1):39-45.) The correlation of Interferon omega (IFN omega) serum concentration with APSI and other multiorgan autoimmun diseases were measured. A novel c. 1344 delC mutation in AIRE was detected. Anti-IFN-omega antibodies seem to appear very early in life and are helpful to differentiate APSI from other multi-organ autoimmune diseases.
