A szívizom ischaemiás/reperfúziós anyagcserezavaraiban, a krónikus szívelégtelenség kialakulása során, és a gyulladásos citokinek felszaporodásával járó állapotokban az oxidatív stressz hozzájárul a szív pumpafunkciójának romlásához. Az oxidatív stressz intracelluláris célpontjai, melyek között a myofibrilláris fehérjék kitüntetett figyelmet érdemelnek, még nem tisztázottak kellőképpen. Feltételezésünk szerint oxidatív stressz-állapotokban a kontraktilis fehérjék szulfhidril csoportjainak oxidációja meghatározó szerepet kap a kamrai pumpafunkció csökkenésében. Ezért célunk annak felderítése, hogy az oxidatív természetű myofilamentális fehérje-eltérések és a szív pumpafunkciójának változásai között milyen kapcsolatok érvényesülnek a humán myocardiumban. A membrán-permeabilizált izolált humán szívizomsejtek Ca2+-regulált erőgenerálására kifejtett hatásokat a fehérjék szulfhidril-csoportjaira specifikus oxidatív és reduktív ágensek alkalmazását követően in vitro tanulmányozzuk. Párhuzamosan végrehajtott biokémiai/biofizikai jellegű mérésekkel, ischaemiás/reperfúziós szívizom-károsodásokat utánzó állatmodellekkel, terminális szívelégtelenségből származó humán szívizomszövetek feldolgozásával és a haemodinamikai paraméterek kritikus feldolgozásával kívánjuk igazolni a myofilamentális fehérjék oxidációjának szerepét a szív pumpafunkciójának zavaraiban. Továbbá, vizsgálni kívánjuk az oxidatív stressz-kiváltotta kontraktilis fehérje-eltérések megelőzésére és/vagy visszafordítására alkalmas vegyületek hatékonyságát azzal a reménnyel, hogy ezek a kísérletes adatok új stratégiák bevezetését kezdeményezhetik a cardiális gyógyszerfejlesztések területén.
Summary
Oxidative stress is known to be involved in disorders of cardiac pump function upon ischaemia-reperfusion, in heart failure, and in response to inflammatory cytokines. Still, the possible intracellular targets of oxidative stress, especially at the level of human cardiac myofibrillar proteins are not yet fully characterized. We hypothesize that sulfhydryl group oxidation of contractile proteins has a major detriment on the ventricular pump function during oxidative stress. Therefore, our objective is to characterize the postulated mechanistic link between oxidative myofilament protein alterations and changes in cardiac pump function in the human myocardium. Sulfhydryl group-specific oxidative and reducing agents will be employed in permeabilized isolated human cardiomyocytes and the consequent changes in Ca2+-regulated myofilament force production will be determined in vitro. Parallel biochemical/biophysical assays, the inclusion of animal models of myocardial ischaemia/reperfusion, heart samples of terminal human heart failure, and the analysis of haemodynamic indices will assist the recognition of the hypothetical relationship between myofilament protein oxidation and disorders of cardiac pump function. Moreover, we intend to evaluate the effects of different chemicals for the prevention and/or reversion of oxidative stress-induced contractile protein alterations with the hope that this experimental information can stimulate new strategies for cardiac drug development.

Megfigyeléseink szerint a szívelégtelenség előrehaladott stádiumaiban a kontraktilis fehérjék Ca2+-érzékenysége fokozódik, mely leginkább a troponin I fehérjék alul-foszforforiláltságával függhet össze. Amennyiben a funkciózavar diastolés komponenssel is rendelkezik, úgy a csökkent miofilemantáris fehérjefoszforiláció a szarkomer titin nevű óriásfehérjéjét is érintheti, ezáltal fokozva a szívizomsejtek passzív feszülését. Ezeket az eltéréseket a nitro-oxidatív stressz fokozódásával járó állapotokban további fehérjekárosodások egészíthetik ki. In vitro teszt körülmények között a szívizomfehérjék szulfhidril csoportjainak (SH) ditiodipiridin segítségével létrehozott oxidációja, a tirozin oldalláncok peroxinitrit segítségével kiváltott nitrációja, vagy karbonilációja a szívizomsejtek Ca2+-ra adott válaszadó-képességét egyaránt csökkentik. A szarkomerek mechanikai funkciójának változásit azonban ezekben az esetekben más és más fehérjék idézik elő. Míg a szarkomerfehérjék nitrációja elsősorban strukturális változásokat eredményez, addig az SH-oxidáció és a fehérjekarboniláció a kontraktilis fehérjerendszer működésének szabályozását károsítja. Összességében, a szívbetegségek során a kontraktilis erő Ca2+-érzékenységének növekedése vagy csökkenése a szakomert alkotó fehérjék egymáshoz viszonyított hierarchikus rendszerétől és a fehérjekárosodások egymásra gyakorolt hatásaitól függhetnek. Ez magyarázhatja azt is, hogy infarktuson átesett egerek (fokozott fehéjekarbonilációjával járó) szívelégtelenségében, dacára a troponin I hipofoszforilációjának, a kontraktilis erő Ca2+-érzékenységének csökkenése alakul ki.
Results in English
Our results suggest that at progressed stages of heart failure beta-adrenergic down-regulation is accompanied by an increase in the Ca2+-sensitivity of force production mainly because of troponin I hypophosphorylation. In cases of diastolic dysfunction myofilament hypophosphorylation involves the giant sarcomeric protein titin, thereby increasing the passive stiffness of cardiomyocytes. These protein alterations might be complemented by additional ones during conditions leading to nitro-oxidative stress. Under in vitro test conditions selective oxidation of sulfhydryl groups (SH) by dithiodipyridine, nitration of tyrosine residues by peroxynitrite, or protein carbonylation all decrease the Ca2+-responsiveness of force production in permeabilized cardiomyocytes. However, the sarcomeric proteins affected by these insults and the mechanisms by which they evoke sarcomeric dysfunctions are divergent. Our results suggested predominant structural alterations of the sarcomere following protein nitration, while SH-oxidation or protein carbonylation were associated with changes in the fine regulation of sarcomeric function. Collectively, the direction of change in the Ca2+-sensitivity of force production may depend on the hierarchy of sarcomeric protein alterations and on their interactions in the diseased heart. For example, in a postischemic murine model of heart failure (with increased protein carbonylation) we found a decreased Ca2+-sensitivity of force production in cardiomyocytes at the infarcted zone despite troponin I hypophosphorylation.
