1. A szisztémás lupus erythematosus (SLE) jelenlegi gyógyszeres terápiája már sok szempontból hatékony, de nem teljesen eredményes minden esetben. Ezért további igény van új gyógyító eljárásokra. Új gyógyszeres terápiás lehetőségként vetődik fel két, már forgalomban levő, de egyéb területeken használt, proteoszoma inhibitor tulajdonságú anyag a.) a bortezomib (Velcade), b.) D3 vitamin hatásának kipróbálása az SLE terápiájában (a D3 vitamin hatásában a proteoszoma gátláson kívül más hatások is érvényesülnek). E szerektől az várható, hogy az SLE-s T sejtekben az általunk korábban kimutatott, kóros mértékben csökkent PKC theta, delta és zeta izoenzim expresszió megszüntethető, és e fehérjék szintjének megemelésével a kóros jelátviteli utak egy része javítható ezekben a sejtekben, ami klinikai javulást eredményezhet. E vizsgálatokhoz a klinikai kipróbálás előtt in vitro kísérletekben, Jurcat típusú humán T sejtvonalon, továbbá SLE-s betegekből származó T sejteken vizsgáljuk meg a bortezomib és D3 vitamin hatását a PKC izoenzimek expressziójára. Az in vitro kapott, az előkísérletek alapján várhatóan pozitív eredmények birtokában, megfelelő előkészítés után, próbáljuk ki a két szer alkalmazását a klinikumban is.
2. A DE OEC III. sz. Belgyógyászati Klinikán 2007-ben elkezdődött a terápia rezisztens poliszisztémás autoimmun betegek gyógyítása autolog őssejt terápiában. Az eddigi eredmények SLE-s és rheumatoid arthritises betegekben eredményesnek mutatkoznak, és ennek a terápiának a további kiszélesítése történik a jövőben. Az itt levő klinikai immunológiai laboratórium adottságai lehetővé teszik, hogy az autotranszplantált betegek perifériás vérében több éven keresztül követni lehessen részletesen az immunrendszer sejtes és humorális elemeinek regenerálódását a klinikai állapot tükrében. Ezek a megfigyelések fontos új adatokat szolgáltatnak az autolog őssejt transzpantáció hatásmechanizmusának jobb megértéséhez az autoimmun kórképek gyógyítása során.
Summary
1. The therapy of patients with systemic lupus erythematosus (SLE) is still further requiring the new ways of treatment. The application of two durgs with a capability to inhibit proteosomes a.) bortezomib (Velcade), b.) Vitamine D3 seems to be promising as new agents for the therapy of SLE. Otherwise, they are already applied in other fields of clinical practice now. (In the effects of vitamine D3 other pathways than the inhibition of proteosomes can also play roles.) We found earlier that in the T lymphocytes of SLE patients the expression of PKC theta, delta and zeta isoenzymes is decreased. It can be expected that the treatments with bortezomib and vitamine D3 can result in the elevation of these proteins correcting the signal transduction in the cells and the clinical state in the patients. Before the clinical studies, however, a series of in vitro studies are planned to be carried out in Jurcat cells (human T cell line) and T cells from SLE patients in order to test the effects of these two drugs. The clinical application can take place after the expectedly positive in vitro results (as the preliminary experiment show) and careful clinical preparations.
2. The treatment of therapy resistant patients with polysystemic autoimmune diseases with autologous stem cell therapy was started at the 3rd Department of Internal Medicine in Debrecen in 2007. The present data are showing the efficacy of this new intervention in patients with SLE and rheumatoid arthritis. Therefore, the widening of this new therapy can be expected in the next years. In the Immunological Laboratory of the Department the conditions allow the detailed following up study on the changes in the elements of cellular and humoral immune systems in the peripheral blood of autotransplanted patients for a longer run. These observations can give important new data for the better understanding of the way as autologous stem cell transplantation can heal the patients with polysystemic autoimmune diseases.

1.Az autoimmun betegeknél végzett autolog haematopetikus őssejt transzplantációk után a CD8+ citotoxikus T sejtek számának követése a legalkalmasabb laboratóriumi módszer a perifériás limfociták regenerációjának monitorozására. Ennek van prognosztikus értéke. A CD8+ sejtszám és szérum IL-6 emelkedése párhuzamos. 2.A Bortezomib új, kiegészítő terápiás szerként történő alkalmazása az SLE-s betegekben korlátozottnak látszik a protein kináz C (PKC) izoenzim szint emelő hatás elmaradása miatt, ugyanakkor az apotózis indukció hasznos lehet az autoreaktív sejtklónok eliminációjában. 3.Az SLE-s betegek D3 vitamin kezelésében a kisebb értékekkel való kezdés és az egyéni adagolás ajánlott, mivel az alacsony, 1 nM (80 NE/nap) vitaminnak is már lehet gátló hatása a T sejtek IL-2 termelésre, ami egyébként is kórosan csökkent. A magas (100 nM, 8000 NE/nap) vitamin kezelés nem befolyásolja a PKC enzim szinteket sem egészséges, sem SLE-s személyek T sejtjeiben, de egészségesekben szignifikánsan emeli az IL-10, míg gátolja az IL-2, IL-6, IFNγ és TNFα termelést. 4.Sjögren szindrómás betegek mononukleáris sejtjeiben fokozott mikroRNS-146/a/b, TRAF6 gén és csökkent IRAK1 gén expressziót találtunk az egészséges személyek sejtjeihez képest. A miRNS-146a és TRAF6 gének fokozott párhuzamos expresszióját betegség specifikus jelenségnek tartjuk a Sjögren szindrómában. 5.Modellt hoztunk létre a monociták arachidonsav termelésének különbségeinek megvilágítására az egészséges személyekben és SLE-ben.
Results in English
1.The best laboratory method for the monitoring of the regeneration of peripheral lymphocytes after the autologuos haematopetic stem cell transplantations is the measurement of the increase in number of CD8+ cytotoxic T cells for the patients with therapy resistant autoimmune diseases. It has also some prognostic value. The change in serum IL-6 is in parallel with CD8+ cell count. 2.The use of Bortezomib as a new complementary therapeutic agent in SLE seems to be limited because of the lack of the elevating effect on the expression of PKC isoenzymes in the T lymphocytes.But its apoptosis inducing effect may have some advantage in the elimination of autoreactive cell clones. 3.In the treatment of SLE patients with vitamin D3, to start with lower doses, and to find the optimal individual dose is recommended, as already 1 nM (80 IU/day) of vitamin can inhibit the production of IL-2 what is otherwise low. The high dose of vitamin D3 (100 nM, 8000 IU/day) inhibits the production of IL-2, IL-6, IFNγ, TNFα, increases the production of IL-10 in healthy persons. Even this dose of vitamin D3 is ineffective on the expression of PKC isoenzymes in the T cells of both groups. 4.We suppose that the parallel increase in the expressions of miR-146a and TRAF6 genes can be a disease specific marker in the mononuclear cells of Sjögren’s syndrome. 5.We created a model for the explanation of difference between the production of arachidonic acid in the monocytes of healthy persons and SLE patients.
