A kutatás célja, hogy felderítse azokat a morfológiai elváltozásokat, amelyek a temporális lebeny epilepszia két ismert etiológiájú csoportjában, az agyi térfoglaló folyamatok, valamint az agykérgi fejlődési rendellenességek által okozott epilepsziában az agykéregben megtalálhatóak. Vizsgálatainkat a Pécsi Tudományegyetem Idegsebészeti Klinikáján agykérgi fejlődési rendellenesség és agyi tumor átal indukált gyógyszer-rezisztens epilepszia miatt eltávolított neocorticalis és archicorticalis (hippocampus, subiculum és entorhinalis kéreg) szöveten fény- és elektronmikroszkópos módszerrekkel végezzük. Célunk a sejtpusztulás, a hippocampalis idegsejtproliferáció, szinaptikus reorganizáció, valamint a sejtvándorlás irányításában szerepet játszó Cajal-Retzius sejtek számának és eloszlásának vizsgálata. Ezzel ismertté válhatnak a különböző etilológiájú epilepsziában azok a közös morfológiai elváltozások, melyek valószínűleg az októl függetlenül a hyperexcitáció következményei. Az epilepszia kórokának és a neuropathologiai elváltozásoknak feltárásával az eredményes gyógyszeres kezelés számára adhatunk újabb ötleteket.
Infrastruktúrális változást nem tervezünk, de hozzájárulunk az epilepszia neuropathologiai elemzésének helyi fejlődéséhez. Emellett hozzájárulunk a klinikai érdeklődésű Ph.D. hallgatók elméleti képzéséhez.

Summary
The aim of the project is to shed light on morphological changes of temporal lobe epilepsies induced by brain tumors and cortical developmental malformations. In collaboration with the Neurosurgery and Neurology Departments of University of Pecs we aim to study, using light- and electron microscopical techniques, the surgically removed neo- and archicortical (hippocampus, subiculum and entorhinal cortex) tissues of patients who suffer from drug-resistant temporal lobe epilepsy induced by cortical malformation or brain tumor. Our aim is to study cell loss, cell proliferation of the dentate gyrus, synaptic reorganization and morphology, number and distribution of Cajal-Retzius cells which play a role in neuronal migration. With these investigations, we will reveal those common morphological alterations which most probably are the consequences of hyperexcitations and are independent of the etiology of seizures. The investigation of the neuropathological changes will help us to understand the cause of epilepsies and can be used later in the development of new antiepileptic drugs. We do not plan to buy instruments, therefore no infrastructural changes are expected, but we contribute to the local development of neuropathological investigation of epilepsies and to the training of Ph.D. students.

Célunk azoknak a morfológiai elváltozásoknak a felderítése volt, amelyek a temporális lebeny epilepszia (TLE) két ismert etiológiájú csoportjában, agyi tumorok, valamint az agykérgi fejlődési rendellenességek által okozott epilepsziában az agykéregben megtalálhatóak. A vizsgálatok során nyert szövettani adatokat hippocampus sclerosissal (HS) járó TLE-ben találtakhoz hasonlítottuk. Vizsgálataink során különbséget találtunk a citoarchitektonikai, a sejtpusztulás, az interneuronok morfológiáját érintő elváltozásokban a három betegségcsoportban. Megállapítottuk, hogy különböző etilológiájú TLE-ben közös morfológiai elváltozás a gyrus dentatus szemcsesejteinek calbindin (CB) tartalmának csökkenése, ami valószínűleg az októl függetlenül a hyperexcitáció következménye. A CB-tartalom csökkenése korrelál az epilepszia kezdetével, a betegség műtétig eltelt időtartamával, és a betegek verbális memóriatesztekben nyújtott teljesítményével. A szemcsesejtek éretlensége a TLE kezdetén befolyásolhatja a betegség során azok CB tartalmának csökkenését. Megállapítottuk, hogy emberi TLE-ben, korábbi álltakísérletes adatokkal szemben, a hilaris mohasejtek nem szelektíven érzékenyek, hanem hasonló mértékben pusztulnak, mint a CA1 piramissejtek. Ugyanakkor a mohasejtek pusztulása korrelál a TLE kezdetével, és a betegség időtartamával. Kimutattuk, hogy tumor indukált TLE-ben a hippocampus nem tekinthető kontrollnak abban az esetben sem, ha a tumor nem infiltrálja a hippocampust.
Results in English
Our aim was to study those alterations in the cerebral cortex that can be found in two groups of temporal lobe epilepsy (TLE) with known etiology, in brain tumors- and malformation of cortical development-induced epilepsies. The results obtained with histological examinations were compared to results found in TLE associated with hippocampal sclerosis. We found difference in the cytoarchitectonics, in the degree of cell loss, in morphology of interneurons in TLE with different etiologies. We showed that a common morphological alteration is the loss of calcium binding protein calbindin content in granule cells of the dentate gyrus, which, regardless of the etiology, is the consequence of the hyperexcitation. The decrease in calbindin content correlates with the onset of the epilepsy, the duration of the disease and with the patients’ performance in verbal memory tests. The maturity of granule cells at the beginning of epilepsy may influence the decrease of calbindin content during the disease. We found that, in contrast to previous animal models of epilepsy, in human TLE the hilar mossy cells are not selectively vulnarable, but their loss is parallel with the loss of CA1 pyramidal cells. The degree of mossy cells loss correlates with the age of onset and the duration of TLE. We showed that the hippocampal formation cannot be considered as control tissue in tumor-induced TLE, even in those cases when the hippocampus is not infiltrated by the tumor.
