Jelen pályázatunk, egy az előzőekben már benyújtott (78735), de az OTKA által nem támogatott pályázatunk-az értékeléseknek és a bíráló bizottság javaslatainak megfelelően-átdolgozott változata. (indoklást ld: Appendix 1.).
Pályázatunkban tanulmányozzuk a fennálló és jövőben várható Fiatalkori Kezdetű Depresszió (FKD) rizikója - és két viselkedésbiológiai rendszer: a kardiális vagus kontroll (CVC) és a hangulat-stabilizálás (HS) közötti kapcsolatot. A CVC rugalmatlanságának jellemzéséhez a CVC kísérleti stressz helyzetekben (szomatikus ill. pszichológiai) való működését, különböző paramétereit (nyugalmi, CVC reaktivitás, visszaállítódás) vizsgáljuk. A rugalmatlan HS jellemzéséhez a negatív hangulatot indukáló stressz átélését követő diszfória csökkentésére való képességet vizsgáljuk, két különböző HS stratégia (pozitív élmény felidézés ill. figyelem elterelés) alkalmazása és különböző típusú HS-i lehetőségek során. Mintánkat 200 fő FKD-ban szenvedő proband (a kiválasztás egy korábban körültekintően diagnosztizált 700 fős depressziós mintából történik), 200 fő veszélyeztetett, eddigiekben nem depressziós testvérük és 100 fő egészséges kontroll alkotja. Tanulmányunkban a CVC, a HS, a pszichés státusz és a pszichoszociális funkciók keresztmetszeti vizsgálatát, továbbá a klinikai státusz hosszú távú után-követését tervezzük. Feltételezzük, hogy (1) a fiziológiai és a viselkedési rugalmatlanság, ill. ezek kombinációja keresztmetszeti vizsgálatban elkülönítik a probandokat, a testvéreket és a kontroll csoportot, továbbá (2) a globális viselkedésbiológiai rugalmatlansági mutatók kórjelzőek a depresszió kimenetele (tünetszám, visszatérés) szempontjából.

Summary

Our application is a revised version of a previously submitted application (78735) that was not funded by OTKA. We revised the application according to the evaluation and the feedbacks of the Committee (for details see: Appendix 1.).
In this application, we propose to study the relation between current and prospective Juvenile Onset-Depression (JOD) risk and two functionally important bio-behavioral systems: cardiac vagal control (CVC) and mood repair. To characterize CVC inflexibility, we will assess CVC functioning across different stressful experimental challenges (somatic/psychological); and different parameters of CVC (resting, CVC reactivity, recovery). To characterize inflexible mood repair, we will assess subjects’ ability to attenuate dysphoric mood subsequent to negative mood challenges by implementing 2 different mood repair strategies (positive autobiographical recall vs. distraction) and across different mood repair opportunities (experimentally controlled vs. unstructured).
Our sample is consisted of 200, 11-18 year-old probands, suffering from JOD at the date of the intake procedure, their 200 at risk, but non-depressive siblings and 100 community controls (recruited from a previously diagnosed sample of 700 patients).
In this study we plan the cross-sectional examination of CVC, mood repair, psychic status, psychosocial functions, and the long-term follow-up of clinical status.
We propose that: (1) physiological and behavioral inflexibility and their combination will cross-sectionally distinguish probands, at-risk siblings and controls, and that (2) global bio-behavioral inflexibility indices will prospectively predict the clinical course of depression (number of symptoms, recurrence).

Pályázatunkban a fennálló és a jövőben kialakuló Fiatalkori Kezdetű Depresszió (FKD) rizikótényezőit tanulmányoztuk. Az eredetileg tervezett minta (összesen 500 fő) számot meghaladta a bevételre került kutatási alanyok száma (összesen:702). A kutatásba bekerült alanyok száma végül: probandok: 224 fő, testvérek: 279 fő. A bekerült (199) és kontroll alanynak megfelelő fiatalok száma pedig 181 fő. A munkaterv megvalósulása: Az utánkövetéses vizsgálatok interjúk és/vagy önkitöltős tesztek felvétele formájában valósult meg. Egy alanynál a betegség kimenetelének követése érdekében több alkalommal is végzünk (maximum évente 1alkalommal) vizsgálatot. A tesztek kiküldése, dokumentálása és az adatok rendszerezése a szegedi központi irodában történt. Az elmúlt fél évben újabb utánkövetés nem történt, a tesztek folyamatos visszaérkezésének dokumentálása és adatrögzítése történt. Emellett az OTKA jóváhagyásával, a már meglévő papír alapú dokumentumok és az utánkövetések vizsgálati dokumentumainak digitalizálását is végeztük. A projekt teljes időtartama alatt összesen 1387 utánkövetéses vizsgálat történt meg. Az archiválás tekintetében, digitalizálásra került 1002 interjúcsomag. A kutatás adatainak elemzése folyamatos, jelenleg is. Az eredményeink közlése kongresszusi előadások és folyóirat publikációk formájában szintén folyamatos. Results in English

In our project we investigated the present and the future developing risk factors of the Juvenile Onset Depression (JOD). The number of enrolled participants (n=702) exceeded the planned number (n=500). Finally the enrolled subjects are: probands n=224, siblings n=279. We enrolled (n=199) control subjects but finally n=181 were eligible. The execution of the workplan: The follow-up procedures were conducted through follow-up interviews and/or self-reporting questionnaires collections. In order to follow up the outcome of the disease, the subjects were assessed more times (maximum 1 per year). Sending out the forms, documentation activities and organizing the collected data were done in the Central Office in Szeged. In the last half year (extended period) new follow-up procedure did not happen, but the documentation of the continuously arriving back forms and data entry were done. Furthermore, with the approval of the OTKA we digitized the already collected and stored printed forms and follow-up assessment documentations. We conducted altogether n=1387 follow-up assessments under the entire period of the project, and n=1002 interview package had been digitized. Analysis of the study data is continuous currently also. The presentation of the results in journals and in congresses are also ongoing.
