A hirtelen szívhalál világszerte az egyik vezető halálok. Gyógyszerszedési, genetikai okok, illetve étkezési szokások által kiváltott káliumcsatorna rendellenességek megváltoztathatják a szív repolarizációját és megemelhetik az életet veszélyetető aritmiák és hirtelen szívhalál kockázatát. Mivel a szív repolarizációja több káliumáram finom egyensúlyán alapszik, ezen áramok kölcsönhatása rendkívül fontos a normál repolarizáció fenntartásához. Ez azt is jelenti, ha egyetlen áram működése megváltozik, akkor a többi áram ezt még kompenzálni tudja (repolarizációs tartalék), úgy hogy a repolarizációs folyamat ill. a szív elektrofiziológiai működése látszólag nem sérül és az egyes zavarok észrevétlenek maradnak. Viszont további esetleges változások, behatások már erőteljesen befolyásolnák a szív repolarizációját, és talajul szolgálnának az életet veszélyeztető aritmiák fellépésére, és a hirtelen szívhalálnak. A repolarizációs tartalék szerepének pontosabb megismerése ezért rendkívüli fontosságú, hogy megérthessük az aritmiák keletkezésnek mechanizmusát, valamint a hirtelen szívhalál és az aritmiák kivédésének lehetőségét. A jelen projektjavaslatban számos sokrétű kísérletes tapasztalattal rendelkező kutató tervezi Varró András vezetésével a közreműködését, hogy tanulmányozza a repolarizációs tartalék szerepét az aritmogenézisben. A kísérleteket modern in vitro celluláris elektrofiziológiai (standard mikroelektróda és patch-clamp technika, Ca- mérések), molekuláris biológiai (Q-PCR, Western-blot, Q-RT-PCR), egész szíves (epifluorescens térképezés) és in vivo elektrofiziológiai segítségével valósítanánk meg.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

A projekt fő célja a repolarizációs tartalék (RT) aritmiák patomechanizmusában játszott szerepének a vizsgálata. A cél eléréshez sokrétű technikai szakértelemmel rendelkező tapasztalt kutató fogott össze a következő hat RT-hoz kapcsolódó témában. 1) Ismert, hogy élsportolókban az intenzív tréning szívhipertrófiát okoz. A szívhipertrófia elektrofiziológiai átrendeződést (K csatorna remodelling) okozhat, amely megemelheti az életet veszélyeztető aritmiák kockázatát. Ebből a munkahipotézisből kiindulva kutya ill. nyúlmodellt kidolgozva különböző molekuláris biológiai és elektrofiziológiai technikák segítségével kívánjuk megvizsgálni a remodeling kialakulásának a mechanizmusát. 2) Megvizsgálnánk az eddig kevéssé ismert Kv1.7, Kv3.4 és SK2 kálium csatornák szerepét a szív RT-ban. 3) a HERG csatorna a szív egyik legfontosabb K csatornája, azonban az RT csökkentése révén számos proaritmiás mellékhatás célpontja is. A HERG csatorna két ismert izoformájának molekuláris szintű vizsgálata tovább segíthet megismerni a gyógyszer indukálta proaritmiás mellékhatások kialakulásának mechanizmusát. 4) Olyan hitelesített szív akciós potenciál modelleket tervezünk kialakítani, amelyek új csatornák, illetve az ismert csatornák új tulajdonságainak megismerését segíthetik elő. 5) Tervezzük a Ca homeosztázisnak a repolarizációs rendellenességekben játszott szerepének a tisztázását. A kísérletek során különösen a Na+/Ca2+ csereáramnak a szerepét szeretnénk megvizsgálni, újonnan szintetizált hatékony csatornagátlók segítségével. 6)Tervezzük új transzgenikus nyúlmodell kialakítását, amely elősegítheti a genetikai eredetű proaritmiás repolarizációs rendellenességek mechanizmusainak tisztázását.

Mi a kutatás jelentősége?

A jelen projektjavaslat olyan eredmények elérését tűzte ki célul, amely az élsportolók hirtelen szívhalálának a mechanizmusát segítene megérteni. Ugyanakkor hozzájárulhat ahhoz is, hogy miképpen előzhető meg e hirtelen szívhalál kialakulása élsportolókban ill. szívhipertrófiás betegekben. A projekt néhány olyan kevésbé ismert káliumcsatorna vizsgálatát is tűzte ki célul, mint a Kv1.5, Kv1.7, Kv3.4 és SK2 csatornák, amely segíthet kiszélesíteni ismereteinket a szív repolarizációjáról és repolarizációs tartalékról valamint új antiaritmiás hatású gyógyszertámadási célpontok kijelőlését is lehetővé tenné. A tervezett HERG1/a és HERG1/B izoformák vizsgálata segíthet megismerni számos olyan váratlan proaritmiás gyógyszermelléhatás mechanizmust, amely jelenleg világszerte megnehezíti a biztonság farmakológiai vizsgálatokat. Új kísérletes adatokkal hitelesített megfelelő akciós potenciál számítógépes modellek kifejlesztése elősegítheti több új, eddig nem ismert ioncsatorna leírását, vagy ismereteink pontosítását eddig ismert ioncsatornákról. Ezek a modellek segíthetnek a gyógyszeriparnak is azon gyógyszerfejlesztési programban, amelyeket genetikailag módosított sejtvonalakon történő voltage-clamp technikákra alapoznak, mert csökkenthetnék az állatkísérleten alapuló kísérletek számát. Az ion csatorna deficiens transzgenikus nyúlmodell kifejlesztése a jelen projekt javaslat kiemelt fontosságú eleme. Az irodalomban számos ioncsatornához kapcsolódó transzgenikus egérmodell használatos, de csak két csatornahibás transzgenikus nyúlmodell (hosszú QT1 ill. hosszú QT2) ismert, amelyet egyébként ugyanaz az egy csoport fejlesztett ki. A jelen javaslatban egy ezen modellektől eltérő, a MinK és a natriumcsatorna örökletes genetikai rendellenességének vizsgálatára alkalmas nyúlmodell kifejlesztését tűztük ki célul. A nyúlmodell előnye az az egér és patkánymodellekkel szemben megkérdőjelezhetlen, mert utóbbi két faj szívelektrofiziológiai működése jelentősen eltér az emberétől, kutyáétól és nyúlétól.

A hirtelen szívhalál a legtöbbször az aritmia következménye és jelentős mértékben hozzájárul a kardiovaszkuláris mortalitáshoz. A hirtelen szívhalál sokszor elektrofiziológiai rendellenességek eredménye, amely előzetes iszkémiás eredetű betegségek hiányában is felléphet. A nem iszkémiás eredetű hirtelen szívhalál tipikus lehet azon élsportolóknál, akik egyébként nem veszélyes gyógyszereket vesznek be, vagy valamilyen tünetmentes genetikai hibától szenvednek, és leggyakrabban gyermek, vagy ifjúkorban váratlanul tragikus körülmények között halnak meg. A jelen projektjavaslatban a kutatók a transzmembrán ioncsatornák tulajdonságait szeretnék megvizsgálni. Az ioncsatorna egy fehérje, amely a molekuláris alapegysége a szív bioelektromos tevékenységének. A kísérletek során modern egész állaton végzett ill sejtszintű és molekuláris szintű vizsgálatokat végeznénk a hirtelen szívhalál mechanizmusának megismerése céljából. Az eredmények kibővíthetik ismereteinket az életet veszélyeztető szívritmuszavarok mechanizmusairól, és segítenének egyaránt megismerni a kockázati tényezőket ill ezen veszélyes betegségek megelőzési vagy terápiás lehetőségeit.

Sudden cardiac death (SCD) is one of the leading causes of mortality worldwide. Abnormalities of cardiac potassium channels due to diseases, drugs, genetical or alimentary reasons can alter cardiac repolarization and can enhance risk of life threatening arrhythmias leading to SCD. Since cardiac ventricular repolarization is determined by the fine balance of potassium currents, the interaction of these currents is crucial to maintain normal repolarization. This includes that impairment of one single type of ion channel can be compensated by the function of the others (repolarization reserve), and it seems that the repolarization process and the electrophysiological function of the heart is stable, so the abnormality is often remained unnoticed. However, further hits in the cardiac ventricular channel system can lead to manifest and robust changes in repolarization, and provide a substrate for life threatening cardiac arrhythmias and SCD. Therefore, better understand the role of repolarization reserve is important to reveal the mechanism of arrhythmias for prevention and possible treatment of arrhythmia induced SCD. In this project proposal by the cooperation of several senior scientists having different experimental skills under the leadership of András Varró would devote their joint effort to study the function of repolarization reserve and its role in the arrhythmogenesis with wide range of in vitro techniques as cellular electrophysiology (standard microelectrode, patch-clamp techniques, Ca2+ measurements), molecular biology (Q-PCR, Western-blot, Q-RT-PCR), whole heart (epifluorescent mapping) and in vivo electrophysiological techniques.

What is the major research question?
Describe here briefly the problem to be solved by the research, the starting hypothesis, and the questions addressed by the experiments.

The major goal of the research project is to elucidate the role of repolarization reserve (ReR) in the pathophysiology of arrhythmias. To achieve this goal the proposal offers cooperative effort of several scientists with wide range of expertise in six topics. 1) Top athletes develop endurance training induced cardiac hypertrophy. This is expected to lead cardiac remodelling (K-channel downregulation), which may enhance the risk of life threatening arrhythmias. Based on this working hypothesis in experimental rabbit and dog models the molecular biological and cellular electrophysiological mechanism of these factors would be investigated. 2) We would study the role of Kv1.7 and Kv3.4 and SK2 channels in cardiac ReR in relation with arrhythmogenesis. 3) HERG channels represent the most important cardiac potassium channels, and serves target for drug induced proarrhythmic complication presumably by decreasing the ReR. Therefore, the elucidation of the function of the two HERG isoforms may lead to better understanding of drug induced arrhythmias. 4) To construct and validate cardiac action potential models would help to identify new channels or modify the view regarding the function of the well explored ones. 5) We would like to investigate the role of intracellular Ca2+ changes in the genesis of the repolarization disturbances based arrhythmias, by focusing on the function of the Na+/Ca2+ exchanger (NCX) by applying newly synthetized NCX blockers. 6)To develop and test new ion channel deficient transgene rabbits with molecular biological and electrophysiological techniques would help to elucidate the proarrhythmic mechanism of gene defect based repolarization abnormalities.

What is the significance of the research?

The proposed research proposal suggests getting new results which would help to better understand the mechanism of sudden cardiac deaths in top athletes. This is expected to improve the prevention of SCD in athletes and also patients suffering from hypertrophic cardiomyopathy. The studies also focus on new less well explored transmembrane potassium channels such as Kv1.5, Kv1.7, Kv3.4 and SK2, and can further improve our existing knowledge regarding cardiac ventricular repolarization and ReR, and may identify new drug targets for preventing cardiac arrhythmia and SCD. Elucidating the physiology and pharmacology of the HERG1/A and 1/B ion channel isoforms would help to improve our understanding regarding unexpected proarrhythmic drug adverse effects, which is currently one of the major problem of the safety pharmacological investigations worldwide. The development of new cardiac ventricular action potential models with proper and systemic validation would provide proper future research to better identify possible so far unrecognized ion channels and change our existing view of the functions of the cardiac ion channels. These new models also would offer more focused drug development programs for the drug industry using voltage clamp results obtained in transfected cell lines to predict potential results and may help to avoid complicated animal welfare issues. The development of ion channel deficient transgenic rabbit models is a unique part of the present proposal. In the existing literature there are many ion channel related transgene models in mice, however there are only two published ion channelopathic models in rabbit (LQT1 and LQT2) by the same research team. In our research proposal we would develop transgene rabbits expressing dominant negative mutant human MinK and sodium channel proteins, respectively, which are different from the published rabbit models. The significant advantage of rabbit versus mouse regarding human relevance is obvious since mice and rats have fundamentally different cardiac electrophysiological functions than rabbits, dogs or human.

Sudden cardiac death is most often a consequence of cardiac arrhythmias and contributes significantly to cardiovascular mortality. One aspect of sudden cardiac death is due to the electrophysiological alteration of the heart muscle without the previous existence of ischaemic heart disease. These types sudden deaths can be associated to the sudden death in to athletes after taking otherwise harmless medications, or in individuals having symptomless genetical mutation mostly causing the unexpected and tragic death in childhood or in young ages. In the research proposal the investigators would like to study the functions of cardiac transmembrane ion channels, ie. proteins which are the basic molecular units of cardiac bioelectricity. During their research they would apply different modern whole animal, cellular and molecular biological experimental models to gain useful information regarding the basic mechanism of sudden cardiac death. The obtained results can be expected to improve our existing knowledge of the mechanisms of the life threatening cardiac arrhythmias causing sudden cardiac death, help to identify those of having enhanced risk and improve our tools to prevent or cure this dangerous disease.
