A hirtelen szívhalál, melyet jelentős részben malignus kamrai ritmuszavarok okoznak, a fejlett ipari országokban és hazánkban is vezető halálok. A kamrafibrilláció, a legsúlyosabb kamrai szívritmuszavar évente legalább 600 000 ember halálát okozza Európában és az USA-ban együttvéve. A rendelkezésünkre álló hatékony antiaritmiás szerek alkalmazását nagymértékben korlátozza azok szívritmuszavart provokáló, ún. proaritmiás mellékhatása. A szelektív IKr gátló szerek fordított frekvenciafüggő módon nyújtják a kamrai repolarizációt, továbbá ez a hatás elterő mértékben érvényesül különböző kamrai sejttípusokban, ez korai utódepolarizációk kialakulásához, és a repolarizáció diszperziójának fokozódásához vezet, következményes Torsades de Pointes típusú kamrai tachycardia kialakulással, mely kamrafibrillációba mehet át. Jelen projekt célja olyan új antiaritmiás mechanizmus vizsgálata, mely a kamrai repolarizáció diszperziójának csökkentésén alapul, es ezt a hatást az IKr gátlásával, valamint az IK,ATP egyidejű, szívspecifikus aktiválásával éri el. Kiemelendő, hogy a fenti szinergisztikus hatásmechanizmusok egyben a kálium áramokat gátló szerek proaritmiás mellékhatásai jelentős reszéért felelős fordított frekvenciafüggő repolarizáció nyújtást is mérséklik, így gyógyszer indukálta kamrai ritmuszavarok sokkal ritkábban léphetnek fel a fenti szinergisztikus mechanizmusok egyidejű alkalmazásakor. A tervezett vizsgálatok alapkutatás jellegűek, melyek eredményei új, antiaritmiás hatású gyógyszerkombináció kifejlesztésének alapját képezhetik, mely az eddigi antiaritmias farmakológiai beavatkozásokkal ellentétben jóval csekélyebb proaritmiás kockázatot jelenthet.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

A szelektív IKr gátló szerek fordított frekvenciafüggő módon nyújtják a kamrai repolarizációt, továbbá ez a hatás eltérő mértékben érvényesül különböző kamrai sejttípusokban, ez korai utódepolarizációk kialakulásához, és a repolarizáció diszperziójának fokozódásához vezet, következményes Torsades de Pointes típusú kamrai tachycardia kialakulással, mely kamrafibrillációba mehet át. Jelen projekt célja olyan új antiaritmiás mechanizmus vizsgálata, mely a kamrai repolarizácio diszperziójának csökkentésén alapul, és ezt a hatást az IKr gátlásával, valamint az IK,ATP egyidejű, szívspecifikus aktiválásával éri el. Hipotézisünk szerint az IK,ATP aktiváció és IKr gátlás kombinációja csökkenti a kamrai repolarizáció diszperzióját, csökkenti a fordított frekvencia függő repolarizáció megnyúlást, és megtartott antiaritmiás hatást fejt ki állatmodelleken.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

Az eredmények különösen a szívelégtelenséget kísérő ritmuszavarok új kezelési stratégiáinak kidolgozásában hasznosíthathatóak. A tervezett kísérletek alapkutatás jellegűek, de eredményei új mechanizmusokat azonosíthatnak antiaritmiás szerek fejlesztéséhez, melyek a korábbi antiaritmiás szerekkel szemben jelentősen kisebb proaritmiás mellékhatás rizikóval rendelkeznek. Az új koncepciónak jelentős és közvetlen piaci értéke lenne, mivel a jelenlegi ipari antiaritmiás gyógyszerfejlesztést nagymértékben akadályozza a proaritmiás mellékhatások fenyegetése. A projekt a koncepció bizonyítását célozza (szinergista mechanizmusok azonosítása, melyek mérséklik a repolarizáció diszperziót és csökkentik a fordított frekvencia függő repolarizáció megnyúlást lassú szívfrekvenciánál), és elismeri a jelenlegi KATP nyitó szerek nem megfelelő szöveti szelektivitását. A javasolt szinergista mechanizmusok sikeres terápiás alkalmazása szívizom szelektív KATP nyitó szerek kifejlesztését igényli, melyet ipari projekt keretein belül lehet megvalósítani, melyben molekulakönyvtárakat „high-throughput” szűrésnek vetnek alá, hogy a szív KATP csatornáit alacsony koncentrációban aktiváló, de ér simaizom KATP csatornákat nem befolyásoló molekulákat azonosítsanak. Ez minimalizálná a szer vérnyomáscsökkentő hatását, mely korlátozná a kombináció antiaritmiás hatását a kialakuló reflex tachycardia miatt. A létező KATP nyitó szerek kiindulási pontot jelenthetnek ebben a fejlesztésben, mely meghaladja jelen projekt kereteit, és kémiai szintézisben jártas partner kooperációját igényli majd. Az irodalomban leírt KATP csatorna alegységek molekuláris szerkezeti eltérései, valamint a különböző alegységek eltérő jelenléte a különböző szövetekben arra utalnak, hogy a szívizom szelektív KATP nyitók szintetizálhatóak. A közelmúltban szívüreg specifikus eltéréseket is azonosítottak a KATP csatornák felépítésében. A javasolt szinergista mechanizmusokat egy molekula vagy két molekula kombinációja is létrehozhatja.

A kutatás összefoglalója, célkitűzései laikusok számára

A kamrafibrilláció, a legsúlyosabb kamrai szívritmuszavar évente legalább 600 000 ember halálát okozza Európában és az USA-ban együttvéve. Számos gyógyszer áll rendelkezésre a szívritmuszavarok kezelésére és megelőzésére. Ugyanakkor a jelenlegi hatékony antiaritmiás szerek alkalmazása nagymértékben csökkent az elmúlt 15 évben azok paradox módon szívritmuszavart provokáló, ún. proaritmiás mellékhatása miatt. Jelen pályázat fő célja egy új antiaritmiás mechanizmus vizsgálata, mely két kálium ioncsatorna párhuzamos befolyásolásán alapszik, és jelentősen alacsonyabb proaritmiás mellékhatással rendelkezne. A tervezett kísérletek izolált szívizomsejtek, szívizom szövetpreparátumok elektrofiziológiai, valamint altatott állatok EKG vizsgálatát foglalják magukba. A projekt eredményeit új szívritmuszavar kezelési stratégiák kialakítására lehet felhasználni, különös tekintettel a szívelégtelenséget kísérő szívritmuszavarokra.

Sudden cardiac death, mostly caused by malignant cardiac arrhythmias, is a leading cause of death in industrial developed countries, as well as in Hungary. Ventricular fibrillation, the most serious ventricular arrhythmia, kills at least 600 000 people annually in Europe and in the USA combined. The application of currently available antiarrhythmic drugs is severely limited by their cardiac arrhythmia provoking, proarrhythmic side effects. Selective IKr blocker drugs prolong ventricular repolarization in a reverse use-dependent manner, moreover this effect manifests in a different degree in various ventricular cell types, leading to the formation of early afterdepolarizations and to increased dispersion of repolarization with concomitant Torsades de Pointes ventricular tachycardia development that may degenerate into ventricular fibrillation. The main goal of the present project is the investigation of a novel antiarrhythmic mechanism based on the reduction of ventricular dispersion of repolarization via the parallel inhibition of IKr and cardiac-selective activation of IK,ATP. Importantly, the above synergistic mechanisms also reduce reverse use-dependent prolongation of repolarization, that is mainly responsible for the proarrhythmic side effect of potassium channel blocking drugs, thus drug induced ventricular arrhythmias would occur with significantly less frequency. The proposed studies are classified as basic scientific investigations, however, the results may serve as a basis for the development of a novel antiarrhythmic drug combination that has a markedly reduced proarrhythmic risk compared to previous pharmacological antiarrhythmic interventions.

The main goal of the present project is the investigation of a novel antiarrhythmic mechanism based on the reduction of ventricular dispersion of repolarization via the parallel inhibition of IKr and activation of IK,ATP in the heart. Importantly, the above antiarrhythmic synergistic mechanisms would also reduce reverse use-dependent prolongation of repolarization, that is mainly responsible for the proarrhythmic side effect of potassium channel blocking drugs, thus drug-induced ventricular arrhythmias would occur with significantly lower frequency.

We wish to test the hypothesis that the combined activation of IK,ATP and inhibition of IKr reduces ventricular dispersion of repolarization, decreases reverse-use dependent prolongation of repolarization and exerts a preserved antiarrhythmic effect in animal models.

What is the significance of the research?

The results could be utilized in new treatment strategies of arrhythmias associated with heart failure. The proposed studies are classified as basic scientific investigations, however, the results may offer new mechanisms for the development of novel antiarrhythmic drugs based on the investigated synergistic mechanisms with markedly reduced proarrhythmic risk compared to previous pharmacological antiarrhythmic interventions. This novel concept would hold significant and direct market value, since current antiarrhythmic pharmacological compound development is hampered by the threat of proarrhythmic side effects. This project is proposed for the proof of concept (synergistic mechanisms leading to reduced dispersion of repolarization and reduced reverse rate dependency at slower heart rates), and the lack of sufficient tissue selectivity of presently available KATP channel openers are acknowledged. The successful therapeutic application of these synergistic mechanisms would require the development of cardiac selective IK,ATP activators in the framework of an industrial project, where molecular libraries should be subjected to high-throughput screening to identify molecules that open cardiac KATP channels at significantly lower concentrations than those found in vascular smooth muscle. This would minimize the blood pressure lowering effect of KATP openers, that can limit the antiarrhythmic efficacy of these compounds by inducing reflex tachycardia. The existing KATP channel opener compounds can serve as a starting point for such drug development, that is beyond the scope of the present basic research project, with the assistance of a partner that has expertise in chemical synthesis of numerous structurally related compounds. The well described differences in the molecular structures of KATP subunits and the different subunit assembly in various tissues suggest that selective cardiac KATP openers could be synthesized. In this regard, cardiac chamber specific differences in KATP channel composition have also been suggested recently. The synergistic mechanisms proposed in this project can be achieved by the effects of a single molecule or by a combination of two drugs.

Sudden cardiac death, which is mostly caused by lethal heart rhythm disturbances (arrhythmias), is a leading cause of death in industrial developed countries, as well as in Hungary. Ventricular fibrillation, the most serious ventricular arrhythmia, kills at least 600 000 people annually in Europe and the USA combined. A number of drugs are available to treat or prevent heart rhythm disturbances. However, the use of most currently available antiarrhythmic drugs has decreased over the past 15 years due to their most dreadful side effect, i.e. their capacity of paradoxically causing arrhythmias (proarrhythmic adverse effect). The main goal of the present proposal is the investigation of a new antiarrhythmic mechanism that is based on the combined modulation of two potassium ion channels, a mechanism that would possess markedly reduced proarrhythmic side effects. The proposed investigations include electrophysiological studies on single heart cells, heart tissue preparations and ECG studies in anesthetized animals. The results of the project could be utilized in development of new treatment strategies of rhythm disturbances, especially those associated with heart failure.

Jelen projektben olyan új antiaritmiás mechanizmust vizsgáltunk, mely a kamrai repolarizácio diszperziójának csökkentésén alapul, és ezt a hatást az IKr gátlásával, valamint az IK,ATP ionáram egyidejű, szívspecifikus aktiválásával éri el. A fenti szinergisztikus hatások egyben a kálium áramokat gátló szerek proaritmias mellékhatásai jelentős részéért felelős fordított frekvenciafüggő repolarizáció nyújtást is mérséklik, így gyógyszer indukálta kamrai ritmuszavarok sokkal ritkábban léphetnek fel a fenti szinergisztikus mechanizmusok egyidejű alkalmazásakor. Eredményeink szerint az IK,ATP aktivátorok és IKr gátlók kombinációja csökkenti a kamrai repolarizáció diszperzióját, csökkenti a fordított frekvencia függő repolarizáció megnyúlást, így eredményeink kedvezőbb mellékhatásprofillal rendelkező ritmuszavar ellenes terápiás gyógyszerkombináció kifejlesztéséhez vezethetnek. Tisztáztuk továbbá az antiaritmiás és proaritmiás vizsgálatokhoz gyakran használt két faj (nyúl, kutya) eltérő ioncsatorna kifejeződés talaján fennálló eltérő ritmuszavarok iránti érzékenységét.

The above antiarrhythmic synergistic mechanisms also reduce reverse use-dependent prolongation of repolarization, that is mainly responsible for the proarrhythmic side effect of potassium channel blocking drugs, thus drug-induced ventricular arrhythmias occur with significantly lower frequency. Our results show that the combination of IK,ATP activators and IKr inhibitors reduces ventricular dispersion of repolarization, decreases reverse-use dependent prolongation of repolarization, thus these results can lead to the development of a novel antiarrhythmic therapeutic drug combination with a better adverse effect profile. We also clarified species dependent differences in arrhythmia susceptibility based on distinct ion channel expression patterns in two common animal models used for antiarrhythmic and proarrhythmic studies (rabbit and dog).
