A kutatás összefoglalója, célkitűzései szakemberek számára
Itt írja le a kutatás fő célkitűzéseit a témában jártas szakember számára.

A közelmúltban felfedezett, sejtek által kibocsátott extracelluláris vezikulák orvosi felhasználhatóságát kívánjuk vizsgálni tumoros és gyulladásos megbetegedésekben. A projekt alapját a pályázó munkacsoportjának és kollaborációs partnereinek az extracelluláris vezikula-kutatás területén szerzett, csaknem egy évtizedes szakmai tapasztalata teremti meg.
A kutatási terv egy-egy kiemelkedő jelentőségű, tumor és gyulladásos megbetegedéssel kapcsolatos diagnosztikai problémára fókuszál. A vizsgált kórképekben a korai diagnózis jelenleg megoldatlan, és a korai felismerés az adekvát terápiás döntések lehetővé tétele révén alapvetően meghatározhatja a betegség kimenetelét.

A fentiek alapján a tervezett vizsgálatok célja:
1. Originális, korábban nem alkalmazott, extracelluláris vezikula irányú vizsgálatok módszertani kifejlesztése
2. A fenti új, extracelluláris vezikula alapú tesztekkel pancreas carcinomás betegek vizsgálata
3. Az fenti új, extracelluláris vezikulák alapú tesztekkel nem differenciált arthritisben szenvedő betegek vizsgálata. A nem differenciált arthritises esetek kb. egyharmadában később rheumatoid arthritis alakul ki. A rheumatoid arthritisre való hajlamot mutató betegek korai azonosítása lehetővé teszi a terápia korai megkezdését.
4. Annak vizsgálata, hogy a fenti két kórkép esetében az új, extracelluláris vezikulákkal kapcsolatos vizsgálati eredményeknek és a hagyományos diagnosztikai adatoknak (genetikai, szerológiai, stb.) az integrálása mennyiben teszi lehetővé a jelenleginél korábbi és pontosabb diagnózis felállítását.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Mind a magas halálozási arányú pancreas carcinoma, mind a rheumatoid arthritis korai diagnosztikája alapvető fontosságú.
A pályázat során megválaszolandó kérdések a következők:
1. Mennyiben szolgáltathat a vérplazma eredetű extracelluláris vezikulákra jellemző felszíni glikozilációs mintázat (glycosignature) diagnosztikus információt i) pancreas carcinomában és ii) rheumatoid arthritisben?
2. Mennyiben jelent a vérplazma eredetű extracelluláris vezikulákra jellemző felszíni interakciós molekulapartnerek azonosítása diagnosztikus információt i) pancreas carcinomában és ii) rheumatoid arthritisben?
3. Mennyiben szolgáltathat a vérplazma eredetű extracelluláris vezikulák nukleinsav tartalma (DNS és RNS) diagnosztikus információt i) pancreas carcinomában és ii) rheumatoid arthritisben?
4. Mennyiben képezhetik a fenti vizsgálatokból származó, extracelluláris vezikulákra vonatkozó adatok más, hagyományos diagnosztikai adatokkal integrálva komplex, multiparaméter alapú, rendszerszintű diagnosztikai elemzések alapját?

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

A tervezett vizsgálatok a közelmúltban felfedezett extracelluláris vezikulákkal (exoszómákkal, mikrovezikulákkal és apoptotikus vezikulákkal) kapcsolatos vizsgálatokon alapulnak. Az extracelluláris vezikulák foszfolipid kettősréteggel határolt képletek, melyek hozzávetőlegesen a vírusokkal és a baktériumokkal megegyező mérettartományba esnek. A létezésüket a közelmúltig figyelmen kívül hagyta tudományos közösség, egyszerű sejttörmeléknek tekintették őket. Napjainkra az extracelluláris vezikula területet robbanásszerű fejlődés jellemzi. Az elmúlt 15 év vizsgálatai rámutattak, hogy az extracelluláris vezikulák sejtek általi kibocsátása aktív, szabályozott folyamat. Napjainkban az extracelluláris vezikulákra mint potenciális új generációs biomarkerekre tekintünk. Kimutatásuk a keringésből minimálisan invazív tesztként („folyékony biopsziaként”) forradalmasíthatja számos megbetegedés diagnosztikáját.
A jelen pályázat során két kiemelkedő jelentőségű diagnosztikus problémát jelentő kórképet (a pancreas carcinomát és a nem-differenciált arthritist) tervezzük vizsgálni extracelluláris vezikula alapú módszerekkel, melyek esetében a korai diagnózis és az ennek megfelelő terápia alapvetően befolyásolja a betegség kimenetelét.
Sikeres megvalósulás esetén a kidolgozott módszerek a fenti kórképeken messze túlmutatva számos más tumor és gyulladásos megbetegedés korai diagnózisát lehetővé tevő teszt alapjául szolgálhatnak.
Bár a tervezett vizsgálatok egy része speciális műszerigényű (pl. grating coupled interferometry, tömegspektrometria), azonban amennyiben sikerül jelentős betegség-specifikus extracelluláris vezikula kötődést mutató fehérjéket azonosítani, úgy ezen egyedi fehérjék felhasználásával a továbbiakban egyszerű és alacsony költségigényű módszerek kifejlesztésére nyílhat lehetőség (pl. gyorsteszt vagy enzyme linked lectin assay (ELLA).

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

Az extracelluláris vezikulák létezése és jelentősége a közelmúltig ismeretlen maradt a kutatók számára. Hozzávetőlegesen másfél évtizede kezdtek gyűlni adatok arra vonatkozóan, hogy a fenti, vírusokkal és baktériumokkal megegyező méretű, sejthártyával körülvett hólyagocskákat szervezetünk valamennyi sejtje aktívan termeli. A pályázat az extracelluláris vezikulák lehetséges orvosbiológiai alkalmazását tervezi vizsgálni. A hasnyálmirigyrák korai felismerésére tervez extracelluláris vezikulák vizsgálatán alapuló tesztet kifejleszteni. E mellett az un. „nem-differenciált ízületi gyulladásban” szenvedő betegek közül azon személyek korai felismerését lehetővé tevő teszt kifejlesztését célozza, akik esetében a folyamat a későbbiekben rheumatoid arthritisszé alakul. Sikeres megvalósítás esetén a kidolgozott módszerek a fenti két, kiemelkedő jelentőségű megbetegedéseken túl számos más daganatos és gyulladásos kórkép korai felismerésének elvi lehetőségét teremthetik meg. A vezikula alapú, új generációs vizsgálati eredmények önmagukban, vagy a hagyományos diagnosztikai módszerek eredményeivel kombinálva, lehetővé tehetik a betegségek jelenleginél korábbi felismerését, és ez alapvetően befolyásolhatja az alkalmazott kezelést és a betegség kimenetelét. Miközben az extracelluláris vezikulák diagnosztikus biomarkerként való felhasználhatósága intenzív nemzetközi kutatás tárgyát képezi, az ismert projektek kizárólag a vezikulák RNS (elsősorban miRNS) tartalmának vizsgálatán alapulnak. A jelen pályázat munkatervében az RNS vizsgálatokon túlmenően három, eredeti koncepción alapuló, korábban nem vizsgált vezikula alapú diagnosztikus teszt kidolgozásának a lehetőségét vizsgáljuk

Summary of the research and its aims for experts
Describe the major aims of the research for experts.

The proposal aims at investigating the biomedical applications of the recently discovered extracellular vesicles. The team of the PI has an almost 10 year long work experience in the extracellular vesicle field that serves as a basis of the proposed study. The proposal will foccus on a tumor and an inflammatory disease (both of which pose significant diagnostic problem even these days). In these two clinical conditions the question of early diagnosis remains to be solved, and early diagnosis would have a strong impact on therapeutic decisions and clinical outcome of these diseases.

Goals of the proposal:
1. Development of novel, orextracellular vesicle based diagnostic tests (focusing on previously uninvestigated molecular features)
2. Studies on patients with pancreatical cancer using the above novel, extracellular vesicle based tests
3. Studies on patients with undifferentiated arthritis to identify the individuals (28% of all cases) who will later develop rheumatoid arthritis using the novel extracellular vesicle-based tests
4. Assessing the diagnostic sensitivity and specificity of integrated data obtained using the novel extracellular vesicle based tests and those obtained using conventional diagnostic tests (genetic , serological, etc.).

What is the major research question?

Early diagnostics is of critical importance both in the case of pncreatical cancer and rheumatoid arthritis.
The questions that will be addressed in the proposed study are as follows:
1. Do glycosignatures on blood plasma extracellular vesicles provide diagnostic information i) in pancreatical cancer and ii) in rheumatoid arthritis?
2. Do surface molecule interaction partners of blood plasma extracellular vesicles provide diagnostic information i) in pancreatical cancer and ii)in rheumatoid arthritis?
3. Does nucleic acid cargo (both DNA and RNA) of extracellular vesicles provide diagnostic information i) in pancreatical cancer and ii) in rheumatoid arthritis?
4. May integration of EV-based data with conventional diagnostic parameters lead to earlier and more sensitive multiparameter diagnosis i) in pancreatical cancer and ii) in rheumatoid arthritis than currently available tests?

What is the significance of the research?
Describe the new perspectives opened by the results achieved, including the scientific basics of potential societal applications. Please describe the unique strengths of your proposal in comparison to your domestic and international competitors in the given field.

The proposed project is built on the recent discovery of extracellular vesicles (exosomes, microvesicles and apoptotic vesicles), phospholipid bilayer bound structures in the size ranges of bacteria and viruses. Until recently, extracellular vesicles were neglected and were referred to as “debris”. An explosion like development in the field in the past 15 years has lead to the understanding that extracellular vesicles are released by all cells of the body in an active, regulated fashion, and are currently considered as potential next generation biomarkers. Their detection from the circulation is currently considered as minimally invasive tests, „liquid biopsies” that may revolutionize diagnostics of several diseases. Here we selected two outstanding diagnostic problems (early diagnostics or pancreatic cancer and undifferentiated arthritis) in which an earlier precise diagnosis may lead to therapeutic decision with substantial impact on the clinical outcomes. If successful, this project may provide diagnostic tests for a wide range of tumors and inflammatory diseases far beyond d the two examples investigated in the project.
Importantly, even though during this developmental phase several analytical procedures (such as the use of grating coupled interferometry or mass spectrometry) require special equipment, once some specific and strong EV binder molecules are identified, such selected lectins or nucleic acids may serve as a basis to develop simple and cost effective tests (such as lateral flow tests or lectin-based enzyme linked lectin assay (ELLA).

Summary and aims of the research for the public
Describe here the major aims of the research for an audience with average background information. This summary is especially important for NRDI Office in order to inform decision-makers, media, and others.

The existence and significance of extracellular vesicles has remained hidden until recently for the scientific community. Approximately 15 years ago data started to accumulate rapidly, suggesting that these membrane-surrounded small „packages” (falling into the size ranges of viruses and bacteria) are produced actively bay all cells of the body. The current proposal aims at investigating the biomedical applications of extracellular vesicles. An extracellular vesicle based test is planned to be developed for early diagnosis of pancreatical cancer. Furthermore the PI and colleagues aim at investigating the possibility of identifying those individuals from among those suffering in undifferentiated arthritis, who will later develop rheumatoid arthritis. If successful, the project may galvanize the development of diagnostic tests for a wide range of tumors and inflammatory diseases. The novel, extracellular vesicle-based data alone or in combination with those obtained by conventional tests, may enable early recognition of the investigated diseases. This in turn, may substantially impact therapeutic decisions and clinical outcomes. Possible exploitation of extracellular vesicles as diagnostic biomarkers is currently under intense investigation worldwide; these studies are almost exclusively focus on RNA (in particular, miRNA) cargo of the vesicles. Importantly, beyond the intensely investigated RNA tests, here we propose three different original (previously uninvestigated) concepts for the development of novel extracellular vesicle based diagnostics.

A lezárult OTKA támogatás ideje alatt a Kutatócsoport a munkatervnek megfelelően a sejtek által termelt extracelluláris vezikulák (EV-k) biomedicinális vonatkozásait vizsgálta a rheumatoid arthritisre (mint gyulladásos autoimmune kórképre) és a pancreas carcinomára fókuszálva (mely utóbbiban nem rendelkezünk korai keringő biomarkerrel). A munkaterven túlmutató módon alapvető módszertani fejlesztésekre is sor került az EV izolálás és detektálás területén (differenciál detergens lízis bevezetése és vizsgálata, EV izolálási módszerek összehasonlítása, radioaktív EV jelölés és SPV lipid assay optimalizálása). Igazoltuk az LDL és a DNS asszociációját az EV felszínekkel. Rheumatoid arthritises monociták esetében a túloxidált membránrészletek EV formájában való kibocsátását igazoltuk, rheumatoid arthritises vérplazma eredetű EV-k esetében kimutattuk, hogy gátolják az osteoclastok képződését. Rheumatoid arthritis-specifikus protein koronát igazoltunk az EV-k körül rheumatoid arthritises vérplazmában történő EV inkubációt követően. Pancreas carcinomában potenciális EV-asszociált biomarker molekula jelölteket azonosítottunk. Colorectális carcinomában pedig egy új EV biogenesis útvonalat (az MVB-szerű kis EV klaszterek en bloc szekrecióját) sikerült igazolnunk.

During the funded period of this OTKA grant, the Research Group investigated the biomedical relevance of extracellular vesicles (EVs) in rheumatoid arthritis (an autoimmune inflammatory disease) and pancreatic cancer (a tumor with no early circulating biomarker) according to the research plan. Beyond the studies outlined in the research plan, the funded project yielded in several methodological developments in the field of EV isolation and detection (differential detergent lysis, comparison of methods for extracellular vesicle isolation, radiolabelling of extracellular vesicles and optimization of the SPV lipid assay). We confirmed the association of LDL and DNA with the surface of EVs. In rheumatoid arthritis, we observed that monocytes release their over-oxidized membrane segments by EV secretion. Furthermore, we found that rheumatoid arthritis-derived extracellular vesicles inhibit osteoclastogenesis. We confirmed the formation of a rheumatoid arthritis-related protein corona around EVs incubated in rheumatoid arthritis blood plasma. Furthermore, using mass spectrometry, in pancreatic cancer we identified numerous potential EV biomarker-candidate molecules. Finally in colorectal cancer we identified a novel EV biogenesis route (en bloc secretion of MVB-like small EV clusters).
