Itt írja le a kutatás fő célkitűzéseit a témában jártas szakember számára.

A rosszindulatú daganatos betegek szisztémás terápiája során kérdés, hogy van-e olyan gén, amelynek az expressziója előre jelezheti a terápiával szembeni érzékenységet vagy rezisztenciát. A kutatás során az emlőrák kezelésében alkalmazott fő szerekkel szembeni rezisztencia-biomakerek azonosítását lehetővé tevő adatbázis és elemző rendszer létrehozása a cél. Az első lépésben az elérhető adatbázisok alapján összeszedjük azokat a mintákat, ahol korábbi vizsgálatok alapján génexpressziós, kezelési és klinikai adatok is rendelkezésre állnak. A második lépésben sejtkultúrás modell rendszerben meghatározzuk néhány kiválasztott szerrel szemben érzékeny és rezisztens sejt expressziós profilját. A sejtkultúrás adatok a klinikai adatok validálását teszik lehetővé. A kutatás harmadik, befejező lépése az adatok statisztikai feldolgozása valamint a klinikai és expressziós adatok integrálása egy olyan környezetbe, ahol az elemzést automatizált módon lehet elvégezni.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

A kutatás abból a feltételezésből indul ki, hogy a korábbi, terápiás biomarkerek azonosítását célul kitűző kutatások az alacsony mintaszám miatt nem voltak sikeresek. Ezért jelen projektben 15 évre visszamenőleg fogjuk valamennyi elérhető adatot összegyűjteni. Elképzeléseink alapján ez a tartomány már elegendően nagy mintaszámot fog eredményezni, hogy valóban megbízhatóan tudjunk új biomarkereket azonosítani. Az azonosított biomarkereket később a betegek osztályozása során lehet alkalmazni.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

A hasonló kutatások során általában mindenki azt adja meg, hogy pld. "azonosítani fogjuk az antraciklin-rezisztencia új biomarkerét". Itt nem ez a cél. A klinikai adatok integrálásával egy olyan adatbázist hozunk létre, ahol az összes gén expressziója vizsgálható a legfontosabb kezelések fényében. A felhasznált sejtkultúrás adatok a korrelációk alapszintű validálását teszik lehetővé. Azonban a kutatás fő eredménye a létrehozott elemző rendszer lesz, ahol minden egyes kutató már saját maga (!) tudja az összes elérhető gén közül az általa vizsgált gén (!) hatékonyságát ellenőrizni. Ezután az egyes kutatókra fog hárulni a feladat, hogy az ok-okozati összefüggéseket (pld. génkiütés vagy génbevitel alkalmazásával) igazolják. Ilyen rendszer jelenleg egyáltalán nem érhető el.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

A rosszindulatú daganatos megbetegedések gyógyszeres kezelése során általában több elérhető szer is felhasználható egy-egy konkrét beteg gyógyítása során. Azonban ma - néhány ritka kivételtől eltekintve - nem áll rendelkezésünkre olyan teszt, amivel a várható rezisztenciát előre tudnánk jelezni. Amennyiben a tumor rezisztens egy adott szerrel szemben, akkor az azzal való kezelés hatástalan lesz, a beteg életminősége romlik, és a kezelés költsége feleslegesen terheli az egészségbiztosítást. A kutatás során kísérletes és meglevő klinikai adatok összekapcsolásával létrehozunk egy adatbázist, amivel az emlőtumor esetén a terápiás választ előrejelző géneket lehet azonosítani. A létrehozott rendszer emlőrák-kutatók számára lehetővé fogja tenni olyan új biomarkerek azonosítását, amelyek a terápiás válasz előrejelzésére lesznek használhatóak. A későbbiekben ezeket a biomarkereket a klinikusok a konkrét betegek esetén is tudják alkalmazni.

Summary of the research and its aims for experts
Describe the major aims of the research for experts.

A major open issue in the systemic therapy of cancer patients is the identification of genes capable to predict response or resistance to a given treatment. Our primary goal is to establish a database and an analysis tool to identify and validate biomarkers of resistance in breast cancer. The first step will be the collection and integration of databases containing gene expression, treatment, and follow-up data. Then, data from a cell culture model will be utilized to establish expression profiles related to the resistance against a selected set of agents. In this, sensitive and resistant cell lines will be compared, these data will be used to validate the clinical results. The third step includes the integrated analysis of all data in one setting. Finally, we will combine the entire database into a platform to enable the analysis of any gene in an automatized environment.

What is the major research question?
Describe here briefly the problem to be solved by the research, the starting hypothesis, and the questions addressed by the experiments.

We hypothesize that low sample number was the major reason behind the failure of previous biomarker candidates proposed to predict therapy response. In the first step of the current project, we will collect available data published in the last 15 years. Using this time range we will have a sample number vastly superior to previous studies and this will enable to discover robust biomarkers. The new biomarkers can be used in the future in experimental research and in in patient classification.

What is the significance of the research?
Describe the new perspectives opened by the results achieved, including the scientific basics of potential societal applications. Please describe the unique strengths of your proposal in comparison to your domestic and international competitors in the given field.

Generally, most research projects target the identification of a specific biomarker, like "a gene related to anthracycline-resistance". We have a different objective. By integrating available large clinical datasets we will enable the investigation of any selected gene in correlation to the most common treatment options. We will perform analysis of cell culture results to provide a basic validation for the correlations. The major results of the research will be the analysis tool where each researcher can investigate the efficiency of any selected gene. Then, the researchers can decide how validate the causative relations (like using gene editing or gene silencing). No similar analysis tool is available currently.

Summary and aims of the research for the public
Describe here the major aims of the research for an audience with average background information. This summary is especially important for NRDI Office in order to inform decision-makers, media, and others.

Multiple agents are available in the treatment of breast cancer patients. The selection of the most effective among these would be necessary in order to provide the best available treatment for a given patient. However, with the exception of a few examples, tests capable to predict resistance are not available for most of the treatment protocols. In case the tumor cells are resistant for a given agent, then the treatment will have no effect, the patient's quality of life will deteriorate, and the health care will have to carry the unnecessary costs. In our research, we will establish a database by linking experimental and clinical data. This database will enable to identify new genes capable to predict therapy response in breast cancer. The established analysis tool will enable breast-cancer researchers to identifiy and validate new biomarker candidates capable to spot therapy resistant tumors. In the future, these biomarkers can be used by pathologists and clinicians to improve the treatment of cancer patients as well.

A kutatás célja a rosszindulatú tumoros betegségek kezelése során használt gyógyszeres kezeléssel szembeni rezisztencia biomarkereinek azonosítása volt. A vizsgálatokat elsősorban emlőtumor esetén végeztük el, de más tumortípusokkal is foglalkoztunk. A legfontosabb eredmény a rocplot.org online platform felállítása volt, amely rendszer lehetővé teszi, hogy nagy beteganyag esetén a terápiás választ valós időben össze tudjuk kötni az egyes tumorok transzkriptom-szintű génexpressziós mintázatával. Az összehasonlítás során egy receiver operator characteristics elemzés készül, amely által egy folyamatos változót (génexpresszió) össze tudunk kapcsolni klinikai kimenettel (rezisztens vagy szenzitív tumor). A rendszer két fő tumortípusra készült el a projekt alatt (emlő és ovárium), amelyek közlésre is kerültek. Jelenleg folyamatban van a platform kiterjesztése agytumorra és colon tumorra. További experimentális vizsgálatokban a legjobb gének eltérő expresszióját igazoltuk klinikai mintákban illetve sejtkultúrás modell rendszerekben. A kutatás eredményeit 15 vezető szerzős és 10 társszerzős publikációban közöltük le, ezek részletes ismertetését lásd a beszámolóban.

The major goal of our research project was the identification of biomarkers capable to predict therapy response against chemotherapy agents in solid tumors. The primary focus of the study was on breast cancer, but other tumor types were also investigated. The most important result is the establishment of the rocplot.org online platform. This system enables the real-time linking of gene expression changes with therapy response using data from a large patients cohort. During the analysis, receiver operator characteristics is computed, this enables the assessment of a continuous variable (gene expression) in relation to therapy outcome (either responder or nonresponder). We have established the platform for breast and ovarian cancer, and further extension for colon cancer and for brain tumors is currently en route. Additional experimental studies were executed to validate the gene expression alterations in clinical samples and in cell culture model systems. Results of the research were published in 15 lead-author publications and in 10 co-authored publications - a detailed summary of these projects in presented in the uploaded final report.
