A kutatás összefoglalója, célkitűzései szakemberek számára
Itt írja le a kutatás fő célkitűzéseit a témában jártas szakember számára.

A pályázat koncepciójában nem szokványos; az új generációs módszerek, mint a teljes genom szekvenálás, exom szekvenálás, és az array CHG segítségével próbálja meg tanulmányozni azokat a genomikai betegségeket, melyekben nem csak a kódoló szekvenciák mutációi, hanem magának a genomnak az eltérései is okoznak, esetenként dizmorfológiai érintettséggel is járó fenotípus eltérést, Mendel szerinti öröklésmenet mutató családok analízisével. Ezek a betegségek az elfogadott nomenklatúra szerint többségükben a ritka betegségek körébe tartoznak. Míg gondolkozásunkat ma is dominálja az a dogma, hogy az örökletes eltéréseket gondolatilag automatikusan génekhez kötjük, a Humán Genom Project óta tudjuk, hogy a genom egy százaléka a kódoló rész mindössze, és a genomi sivatag névvel is illetett intergénikus területek funkcióval felruházott elemeinek hibái is társulhatnak pathológiával. Az elmúlt 2-3 év világosan megmutatta a humán pedigrék vizsgálatában rejlő potenciált, ráadásul olyan területen, ahol az in vitro-, sejtes-, és állatmodellek nem, vagy csak körülményesen applikálhatóak. A kutatás komplex DNS szerkvencia- és copy number variation (CNV) analízisre épül, aminek eredménye több esetben remélhetőleg molekuláris genetikai szintű diagnózist is jelenthet egyben a vizsgált egyén számára. A pályázat a betegcsoportok terén erősen támaszkodik a meglévő biobank készletre; ami az országos és egy kiemelt EU biobank konzorcium része (www.biobanks.hu, www.bbmri.eu).

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Az OTKA kutatástámogatások történetében bizonyosan alighanem egyedi a kérdésfelvetés, viszont napjainkban csak alapkutásként lehetséges a megcélzott problémakört megközelíteni: bizonyos ritka betegségek nem kutathatóak a beteg nélkül; nem, vagy körülményesen modellezhetőek (sejtes vagy komplexebb rendszereken keresztül), hiszen a genetikai hátterük sem ismert. Az új generációs módszereket nem fogadta be sehol a világon a diagnosztika a rutin ellátás módszereként. Tudnunk kell, hogy az új generációs szekvenáslás éz az array CGH rengeteg területen jól kezelhető és értelmezhető eredményt ad, de világosan látszik, hogy sok adat, szekvencia, szignatúra, kópiaszám variáns, stb., egyenlőre kezelhetetlen információt hordoz. Ennek megfelelően a klinikai vonatkozású kutatásoknak kell egy úttörő szerepet vállalniuk a területen, beleértve a kapott információk IT értékelésének problémakörét is, ami szintén nem lesz egyszerű, vezető folyóiratok már foglalkoznak a patient recontacting problematikájával még akkor is, ha a nyert adatok eredetileg kutatás során keletkeztek. A megcélzott betegségkör választásának logikája abban rejlik, hogy az eddigi adatok szerint a nem csak pont mutációs eredetű, hanem genomi átrendeződés előfordulása még inkább gyakoribb morfológiai eltérést mutató betegekben. Ilyen betegek egyébként relatíve nagyobb számban fordulnak elő a VRONY rendszerben, ezeket az eseteket mélyebben nem vizsgálja senki.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

Az évized végére a biomedicinával foglalkozó tudományos világ világosan felismerte, hogy köszönhetően az új generációs szekvenálás, továbbá az array alapú módszerek robbanásszrű terjedésének, eddig soha nem látott potenciál rendelhető a korábban meglehetősen nehezen kezelhető, emiatt kissé mostohán is kezelt és limitált információértékűnek tartott ritka betegség csoportba tartozó genetikai esetek genomikai feldolgozásához. Már kétségbevonhatatlanul megmutatkozott, a legmagasabb impaktú folyóiratokban megjelentetett közleményeket eredményezve, hogy az ismeretlen genetikájú családfák mellett, akár egyes esetek vizsgálata kapcsán is, az elmúlt egy-két évben annotálásra kerültek további új gén-functiók, genomi elem motívumok, melyek a humán modell, a beteg ember vizsgálatának köszönhetőek, és ez a folyamat még most lépett csak a fellendőlő fázisba. Más szavakkal, az ismeretelen hátterű fenotípus - új gén vagy genomi struktúra - új functió tengely kiemelt fókuszba került köszönhetően a ritka beteg vizsgálatában rejlő potenciális információtartalomnak. Rendkívül felértékelődnek a már meglévő biobank állományokban tárolt különleges genetikai esetek, szerencsés esetet feltételezve, a korábbi kutatási időtartamokhoz viszonyítva, ez a metodikai megközelítés eddig nem látott űtemű tudományos felismeréseket garantálnak, ráadásul az eredmények sok esetben azonnal hasznosultnak tekinthetők, ha arra gondolunk, hogy az egyben beteg emberek diagnózishoz jutását is eredményezték.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

Az orvos-biológiai kutatások története során eddig soha nem állt olyan közel egymáshoz a kutatás és a diagnosztika, mit napjainkban. Az elmúlt 2-3 évben bizonyos nagyon modern és új DNS vizsgáló módszerek valóban robbanászszerű fejlődésen estek át, a műszerek relatíve terjedni kezdtek, és a rajtuk végzett vizsgálatok költségei a Moore törvényt követve elérték azt a költségszintet, amikor ezek relatíve szélesebb körű alkalmazásáról ésszerű gondolkozni. A ritka betegségek körébe tratozó, ritka előfordulású betegségek mindig is voltak, mindig is komoly terhet jeletettek a beteg, a beteg családja, és a társadalom számára is, komoly diagnosztikus és terápiást kíhívást és problémát jelentettek, és jelentenek ma is. Az említett technikák új fejezetet nyitnak ezen begetcsoport számára, amely kutatásból a beteg és a tudomány is profitál: a beteg diagnózishoz juthat, a tudomány új géneket, új mechaniznusokat ismer meg, olyanokat, amiket eddig a meglévő kutató módszerek birtokában nem tudott megismerni. Profitál a társadalom is, hiszen a ritka betegségek kutatásának és későbbi diagnosztikájának, ellátásának mederbe terelése mindenki érdeke. Ezt, az alapkutatés szintjéről látszik ésszerűnek felépíteni. A Pécsi Tudományegyetem 2009-ben elnyert egy 6.2 Mrd Ft összértékű infrastrukturális fejlesztési pályázatot; a Science please! névvel nevezett project beindítását 2012 júliusára tervezik, a jelen pályázat a project részét képezné.

Summary of the research and its aims for experts
Describe the major aims of the research for experts.

The concept of the current application is not usual: with the new generation methods, like the next generation full genome seqeuncing, exome sequencing, and array CGH we want to study those genomic diseases, in which not only the point mutations of the coding sequences, but the alterations of some non-coding genomic seqeunces are associated with not normal phenotypes, ocassionally with accomapining dysmorphology, using analysis ofpedigrees with Mendelian inheritance. Wast majority of these disases belnog to the Rarse-diseases group. Albeit our mind is still under the influance of the dogma which autimatically associates the diseases with coding genes, however, thanks to the Human Genome Project it is already known that the coding seqeunce is only in the 1% range of the genome, and errors of the so called genomic desert, containing functional elements in the intergenic sequences, can also associate with human pathology. The past 2-3 years clearly revealed the impact of the analysis of the human pedigrees with unknown genetic ethiology, not sepaking about that some of these fields are not or hardly can be studied with approaches of in vitro-. cellular-, or animal-models. The research focuses on complex DNA seqeunce and copy number variation (CNV) analysis, and the research result can reveal a molecular genetic diagnosis for the patinet, either. In the study we will use the existing biobank resources, that are parts of the national and an EU based international consortium (www.biobanks.hu, www.bbmri.eu).

What is the major research question?
Describe here briefly the problem to be solved by the research, the starting hypothesis, and the questions addressed by the experiments.

In the history of the OTKA funded research projects such approach is likely very unique might be, however, nowadays the problems subjected in the current application can be regarded still as basic or translational research but not as routine, insurance funded examinations, and certain rare-diseases cannot investigated without the patinets, since they cannot be modeled (by cellualar or more complex systems), because of their genetic background is still not known. The new generatioin sequencing techniques still do not belong to the routine diagnostic methods. It is clear on the one side, that next generation sequencing, and the array CHG provide often useful and straightforwarding results, but it is also clear on the other side, that lots of data, seqeunces, signatures, copy number variants, still remains unsolved with their information content. Therefore, these clinical translation research areas have a pioneering role, including the IT interpretation problematics; the leading medical journals already had introduced the term of the "patient recontacting", even if the data were originally obtained in basic or applied research projects. The concept of the current application comes, in part, from the observation that in patinets having abnormal major or minor morphogenetic clinical varinats, their diseases are associated in less extent with point mutations, but mainly with genomic rearrangements. Relative huge amount of such petients can be found in the VRONY registry; these patinets are not really examined by molecular methods.

What is the significance of the research?
Describe the new perspectives opened by the results achieved, including the scientific basics of potential societal applications. Please describe the unique strengths of your proposal in comparison to your domestic and international competitors in the given field.

Until the end of the previous decade, the biomedicine science had already revealed the exceptional impact of the reseach of the formerly non-favoured rare-disease tasks, as the usefulness of the advantages of the next generation seqeuncing and the spead of the array based methods became supportive in the research of orphan diseases. It is now unequivocally known, and clearly supported by the publications appeared in the highest ranked scientific journals, that besides the analysis of the pedigrees with unknown genetic origin, even the analysis of single entries can result in succesful annotation of new genome functions, verification of new functional genomic elements, that could be identified exclusively by the use of the human model, by the analyisis of the rare-disease affected individuals, and this entire process just entered into the hyped period. In other words, the phenotypes with unknown genetic origin are now in the high prioroty focus due to their scientific impact in the new gene or genomic element - new function research axis. The scientific potencial of biobanks strongly increased, and the biobanks will harmonizing their value and maximizing their use comparing the previous decades, warranting future great progress due to the nex generation technical achievements. Moreover, lots of the resulting findings will have to be regarded as immediate implementation of the results, as lots of the patinets likely will get diagnosis in association with these research efforts.

Summary and aims of the research for the public
Describe here the major aims of the research for an audience with average background information. This summary is especially important for NRDI Office in order to inform decision-makers, media, and others.

During the bio-medical research of the rare-diseases there has no been such direct relationship between the research and diagnostics as it can be observed today. In the past 2-3 years an explosion-like gain could be observed for certain novel DNA examining methods, due to the Moore's-law associated extreme fall of the consumable's costs we entered into a novel period when their extended use already became a daily reality. Diseases belonging to the rare-disease definition presented a great clinical, reseach, and therapeutical challenge all the time. The new generation techniques opened new perspectives for this disease-group, leading to a period when the reseach has a direct merit for the patinets as well: the patinets can get molecular biology diagnosis, and chanche for preconception genetic support, while the science will discover new phisology and biochemistry mechanisms, including those that could have not been discovered just with the already existing tools and methods. The procedure is advantageous also for the contemporary and future society, since appropriate handling of the rare-disease research is a pan-national necessity and political, economical goal as well. The construction of such a system looks necessary to built-up from the basics. At the University of Pécs implementation of a project of 6.2 billion Forints buget is on the way since 2009; the project with the acronym "Science Please!" will be launcehed in July of 2012. The current project would, in part, affiliate to the Science Please! project.

kutatási eredmények (magyarul)
A pályázat során el sikerült érni azokat a célokat, melyek a terveztben szerepeltek, sőt, sikerült több természetes kiterjesztést is sikeresen kivitelezni. Több új gén ill. variáns került annotálásra, több új beteg fenotípus került felismerésre, több új átrendeződést sikerült karakterizálni. Számos funkcionális sajátosságokkal bíró genetikai variáns előfordulási gyakoriságában populációgenetikai különbségeket találtuk. A monogénes ill. a genomikai betegségek terén új fenotípusokat ismertünk meg, melyek tudományos értelemben funkcionális annotálásnak felelnek meg; a klinikum, azaz a beteg vonatkozásában, ez diagnózist jelentett. A terveknek megfelelően sikerrel alkalmaztuk ill. gyarapítottuk biobank készleteinket, számos új nemzetközi kollaborációba sikerült bekapcsolódni közvetlenül vagy indirekten a jelen pályázathoz csatlakozóan. A pályázat során 29 eredeti közleményünk jelet meg, 8 további közlés alatt áll; az eredményeket több nemzetközi konferencián sikerült bemutatnunk; a témavezető két nemzetközi konferencia meghívást is kapott plenáris előadás tartásra. Összesen 5 jelölt szerzett PhD minősítést a pályázat eredményeinek köszönhetően, 2 további hallgató anyaga már bírálók elött van.

In the project all goals were succesfully implemented, in some cases certain natural extension also were achieved. We annotated new genes and genetic variants, we describen new phenotypic variants, including new genomic rearrangments. In a spectrum of functional genetic variants we found population genetic differences. In the genetic and genomic disease groups we desribed new pehotypes, that correspond to novel gene annotations in scientific context, while in the clinics they meant diagnosis for the patients, and patient's families. According to the origainl plans we succesully utilized and gained our biobank collections; we could establish new domestic and international scientific collaborations. We got 29 International, peer reviewed research papers, 8 works are under publication; we also had numerous conference presentations, the group leader had 3 invitations to present plenary lectures in International meetings. A total of 5 candidates got PhD degree, 2 theses are submitted.
