A diabétesszel összefüggő vaszkuláris rendellenességek és ezek következményeként kialakuló neuropátiák elsődleges oka az, hogy hyperglikaemia hatására fokozódik a szervezetben a reaktív szabadgyökök termelődése, s ugyanakkor csökken az endogén antioxidáns védelem hatékonysága.
A közelmúltban bizonyítottuk, hogy streptozotocin-indukált diabéteszes patkányokban a bélperisztaltika szabályozásáért felelős myentericus nitrerg neuronok, valamint ezek oxigén- és tápanyagellátásában fontos szerepet játszó mesenterialis kapillárisok a bélcsatorna különböző szakaszaiban különböző mértékben sérülnek, és az azonnali inzulinkezelésre is eltérően reagálnak. Ez alapján feltételezzük, hogy a neuronális mikrokörnyezetnek meghatározó szerepe van a diabétesszel összefüggő neuropátiák patogenezisében. Ezért vizsgálataink elsődleges célja, hogy megismerjük azokat a molekuláris mechanizmusokat, melyek meghatározzák e bélszakasz-specifikus mikrokörnyezetet.
E pályázat keretében tervezett kísérleteinkkel tisztázni szeretnénk, hogy a caveolinok hogyan szabályozzák az erek fiziológiás működéséhez szükséges nitrogén-monoxid szintjét, a lizil-oxidáz expressziója hogyan befolyásolja az erek permeabilitását, és az endogén antioxidáns rendszer milyen szerepet játszik a diabéteszes változások kivédésében. Ezen kívül tervezzük az egyes bélszakaszok mikroflórájának metagenomikai analízisét is. Biztosak vagyunk abban, hogy vizsgálataink fényt deríthetnek a bélfal diabéteszhez köthető regionális változásainak okaira, s ezzel eredményeink olyan új diagnosztikai és terápiás utakat jelölhetnek ki, melyek csökkenthetik a gasztrointesztinális rendszert érintő diabéteszes szövődmények kialakulásának kockázatát.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Eddigi eredményeink alapján fogalmazódott meg bennünk az a kérdés, hogy a különböző bélszakaszokban levő mesenterialis kapillárisok, hasonlóan a myentericus neuronokhoz, miért reagálnak eltérően a diabéteszes állapotra, még akkor is, ha a vércukorszintet inzulinkezeléssel jól kontrolláljuk.
Mivel a bélcsatorna egyes szakaszainak eltérő anatómiai felépítése és működése szigorú genetikai és molekuláris szabályozás alatt áll, úgy gondoljuk, hogy magában a bélfalban lehetnek olyan régió-specifikus molekuláris különbségek, amelyek meghatározzák az entericus neuronok és a bélfal kapillárisainak diabéteszt kísérő regionális sérülését. Ennek igazolására kontroll, streptozotocinnal indukált diabéteszes és inzulin-kezelt diabéteszes patkányok bélcsatornájának különböző régióiban kvantitatív immunhisztokémiai, RNS- és fehérjeszintű molekuláris biológiai, valamint metagenomikai módszerek segítségével a következő kérdésekre keressük a válaszokat:
1/Hogyan szabályozzák a caveolin fehérjék az endothelialis eredetű nitrogén-monoxid termelődését, s vannak-e ebben bélszakasz-specifikus különbségek?
2/Megfigyelhetők-e regionális változások a két legfontosabb endogén antioxidáns enzimrendszer, a hemoxigenázok és a metallothioneinek termelődésében?
3/Milyen mintázatot mutat az extracelluláris mátrix molekulák keresztkötésében, s így a kapilláris permeabilitás szabályozásában fontos szerepet játszó lizil-oxidáz enzim expressziója a bélcsatorna oro-anális tengelye mentén?
4/Milyen bélszakasz-specifikus különbségeket mutat a bél baktériumflórájának összetétele, illetve a mikrobiom funkcionális analízise az egyes kísérleti állatcsoportokban?

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

A diabétesz legfőbb kockázati tényezőjének számító kardiovaszkuláris betegségek mellett, a cukorbetegek több mint 75%-ánál gyakran jelentkeznek különböző gasztrointesztinális motilitási rendellenességek, mint a dysphagia, reflux, a gyomor ürülésének késése, hányás, hasi fájdalom, székrekedés vagy hasmenés. Bár a diabéteszes gasztroenteropátia patomechanizmusa rendkívül összetett folyamat, ezek a klinikai tünetek arra utalnak, hogy a diabétesz súlyosan károsítja a bélidegrendszert.
Bár a diabéteszes betegek gasztrointesztinális panaszainak kezelése mindennapi megoldandó feladatot jelent a klinikusok számára, e panaszok hátterében álló entericus neuronok diabétesszel összefüggő sérüléséről és e sérülések lehetséges kiváltó okairól irodalmi adat alig áll rendelkezésünkre. Ezért kezdtük el vizsgálni néhány évvel ezelőtt a bélidegrendszer myentericus nitrerg neuronjainak károsodását streptozotocinnal indukált diabéteszes állatmodellben. Eddigi eredményeink alapján feltételezzük, hogy a kísérletes körülmények között kiváltott krónikus diabéteszes patkányok különböző bélszakaszaiban a kapillárisok permeabilitását szabályozó intracelluláris és extracelluláris enzimfehérjék termelődésében, az antioxidáns védekező rendszer működésében és a baktériumflóra összetételében és funkciójában olyan regionális különbségeket találunk, melyek magyarázatot adhatnak az entericus neuronok és a bélfal mikrokeringésének bélszakasz-specifikus változásának okára. Az egészséges bélflóra megváltozása nemcsak előre jelezheti a különböző anyagcsere-betegségek kialakulását, de a baktérium-összetételben tapasztalt eltérések mértéke a betegség lefolyásának állapotát is jellemezheti.
A jelen pályázati periódusban tervezett vizsgálatainkkal olyan molekuláris mechanizmusokat ismerhetünk meg, amely ismeretek birtokában lehetővé válik a betegség korai felismerése, amikor a hyperglikaemia még sem az erekben, sem az idegsejtekben nem okozott vissza nem fordítható súlyos elváltozásokat. Munkánk távolabbi célja az, hogy kísérleteink eredményeivel elméleti alapot nyújtsunk új diagnosztikai és alternatív terápiás eljárások kidolgozásához, valamint újabb probiotikumok és a napi klinikai gyakorlatban alkalmazható célzott gyógyszermolekulák fejlesztéséhez.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

A diabétesz egy tartósan magas vércukorszinttel jellemezhető anyagcsere-betegség, melyben igen magas a különböző keringési betegségek kialakulásának kockázata. Az érrendszeri szövődmények fő oka az, hogy a magas vércukorszint miatt fokozódik a káros szabadgyökök termelődése, ugyanakkor csökken az ellenük való védelem hatékonysága.
A diabéteszes betegeknél gyakoriak a bélmotilitási zavarok miatt kialakuló emésztőszervi panaszok, de ezek oka nem ismert. Kutatócsoportunk egy kísérletesen indukált diabéteszes állatmodellben évek óta vizsgálja a bélperisztaltika szabályozásában meghatározó szerepet játszó nitrerg neuronokat. Eredményeink szerint a bélcsatorna különböző szakaszaiban levő nitrerg neuronok diabétesz hatására különböző mértékben sérülnek, sőt, a vércukorszint szabályozásához használt inzulinra is eltérően válaszolnak. Ez arra utal, hogy magában a bélfalban lehetnek olyan molekuláris különbségek, melyek biztosítják, vagy épp megakadályozzák a neuronok egészséges működését.
Ezért tervezett vizsgálataink elsődleges célja, hogy részletesen tanulmányozzuk a különböző bélszakaszokban levő nitrerg neuronok közvetlen környezetét. Vizsgáljuk a neuronok közelében futó erek áteresztőképességét szabályozó molekulákat, a bélfal saját antioxidáns enzimeinek mennyiségét és működését, valamint azt, hogy az egyes bélszakaszokban élő baktériumok és anyagcseretermékeik hogyan befolyásolják az adott bélszakasz oxigénnel vagy épp oxigén szabadgyökökkel való ellátottságát.
Úgy gondoljuk, hogy kísérleti eredményeink magyarázatot adnak a bélfal diabéteszt kísérő regionális változásainak okaira, s így tudományos alapot szolgáltathatnak új diagnosztikai és terápiás utak kidolgozásához.

Growing amount of evidence has indicated that increase of the hyperglycaemia-induced oxidative stress and decreased effectiveness of the endogenous antioxidant protection play the major role in the initiation of diabetes-related vascular and neuronal damage.
Using a streptozotocin-induced diabetic rat model we recently demonstrated that nitrergic myenteric neurons, which are key regulators of peristalsis, and also the mesenteric capillaries supplying these neurons, display different susceptibilities to diabetic damage and also to insulin treatment in the different gut segments. Based on these results we suggested the importance of the molecular difference in the neuronal microenvironment in the pathogenesis of diabetic enteric neuropathy.
Therefore, the main focus of our ongoing researches is to reveal such molecular mechanisms which are critical in resulting a region-specific pathological microenvironment for enteric neurons. Within the frame of this proposal, we aim to clear that how the endothelial-derived nitric oxide is regulated by caveolins, how the lysyl-oxidase expression can affect the permeability of capillaries, and which role have the endogenous antioxidants to prevent the diabetes-related damages. Besides investigating the molecular differences itself in the gut wall, we are planning the metagenomic analysis of the microbiome in the different intestinal regions of streptozotocin-induced diabetic rats.
We are confident that our results will provide deeper insight into the pathomechanism of the diabetes-related regional damage, and contribute to outline of new diagnostic and therapeutic avenues to reduce the risk of gastrointestinal diabetic complications.

What is the major research question?

Our previous results raise the question; why the mesenteric capillaries and also the myenteric neurons in the different intestinal segments are affected differentially by the diabetic condition even under good glycaemic control?
It is well established that the anatomical, functional and pathological regionality of the gastrointestinal tract develops under strict genetic and molecular control. Therefore, we are planning to investigate those regional molecular differences itself in the gut wall, which are thought to be responsible for the diabetes-related region-specific damage of the enteric neurons and microvessels. Using quantitative immunohistochemistry, molecular and metagenomic methods on protein, DNA and RNA level in tissue samples and intestinal contents of different gut segments of control, streptozotocin-induced diabetic and insulin-treated diabetic rats to find the answers for the following questions:
1/ How the production of endothelial-derived nitric oxide is regulated by caveolins and are there any regional differences in it?
2/ Is there any gut region-specific changes in the production and function of the most important endogenous antioxidants like heme oxygenases and methallothioneines?
3/ How the expression of lysyl-oxidase, a cross-linker of extracellular matrix molecules is changing and regulates the permeability of microvessels in the oro-anal axis of the gut?
4/ Which regional differences can be observed in the composition of the gut microflora and in the function of the microbiome in the gut of different experimental animals?

What is the significance of the research?

Diabetes has long been recognized as the major risk factor of cardiovascular complications; however, gastrointestinal symptoms such as dysphagia, reflux, gastroparesis, vomiting, abdominal pain, diarrhoea or constipation are also common among more than 75% of diabetic patients. The pathomechanism of diabetic gastroenteropathy is highly complex, but these symptoms are often caused by gastrointestinal motility abnormalities related to enteric neuropathy. The treatment of these gastrointestinal symptoms of diabetic patients is everyday tasks to be solved for the clinicians; however, there is hardly any data of the possible causes of diabetes-related enteric neuropathy.
Therefore, using a streptozotocin-induced diabetic rat model we have investigated the involvement of those nitrergic myenteric neurons in diabetes-related gastrointestinal complications which have key role in regulating gut motility.
Based on our already published results we presume that the molecular differences in the expression of intracellular and extracellular proteins regulating capillary permeability, in the behaviour of endogenous antioxidant system and in the composition and function of the microbiome in different intestinal segments can reveal the cause of the gut region-specific neuronal and vascular damage in experimentally induced diabetic rats. The alteration of intestinal microbiota can not only predict the development of metabolic diseases such as diabetes, furthermore, the divergences from the core microflora may define the status of disease.
We expect that the investigations planned within the frame of this proposal will provide deeper insight into the molecular mechanisms of diabetic gastrointestinal complications thereby will allow the detection of disease before developing hyperglycaemia-induced irreversible vascular and neuronal damage and will outline of new diagnostic and alternative therapeutic methods with new probiotics or drugs in the very early stages of hyperglycaemic insults.

Diabetes accompanied with chronic hyperglycaemia is a well-established risk factor for the development of cardiovascular diseases. The major cause of these vascular complications is the increased generation of reactive oxygen species, however, decreased protection against them.
The gastrointestinal symptoms common among diabetic patients are often caused by intestinal motility abnormalities related to enteric neuropathy. Therefore, using an experimentally induced diabetic rat model the nitrergic myenteric neurons, which have a key role in regulation of gut motility, have been investigated for years in our laboratory. According to our previous results, the nitrergic neurons located in different gut segments display different susceptibilities to diabetic damage and they also exhibit different levels of responsiveness to insulin replacement. These indicate the importance of the molecular difference itself in the gut wall allowing neurons in one intestinal region to function normally, while causing them to die in another.
Therefore, our primary aim is to investigate the most important players in the microenvironment of nitrergic neurons located in different gut segments. We plan to investigate such molecules that regulate the permeability of capillaries in the vicinity of myenteric neurons, the production and function of endogenous antioxidants, and the role of intestinal bacteria and its metabolic effects in oxygen supply or free radicals production in the different gut segments of diabetic rats.
We expect that our results may explain the molecular background of the diabetes-related regional changes and open new directions in the diagnosis and the therapies of diabetes.

Jelen kutatási projektben kísérleteink elsődleges célja az volt, hogy megismerjük azokat a bélszakasz-függő molekuláris különbségeket, melyek hozzájárulnak a diabétesszel összefüggő régió-specifikus nitrerg myentericus neuropátia kialakulásához. Metagenomikai vizsgálataink rávilágítottak arra, hogy a krónikus hiperglikémia következtében a béllumen-asszociált mikrobióta összetételének változása az ileumban, míg a nyálkahártyához kötődő mikrobióta megváltozása a duodenumban a legjelentősebb. Kísérleteinkkel bizonyítottuk a reaktív oxigén származékok bélszakasz-függő felhalmozódását, valamint hogy a fokozódó oxidatív stressz következtében az endogén antioxidáns rendszer is regionálisan aktiválódik. Korábbi eredményeinkkel egyetértésben bizonyítottuk az endogén hemoxigenáz rendszer régió-specifikus indukcióját, valamint azt is, hogy a hemoxigenázokkal kolokalizáló nitrerg myentericus neuronok aránya a diabéteszes állatokban fokozódik a különböző bélszakaszokban. Ezen kívül leírtuk, hogy a nitrerg submucosus neuronok denzitása szintén bélszakasz-függő módon változik a diabéteszes és az inzulin-kezelt patkányokban illetve, hogy a hemoxigenáz 1- és 2-immunreaktív neuronok jelenléte a vastagbélben a legmeghatározóbb.
Results in English
In the present project, we investigated those regional molecular differences, which are thought to be responsible for the gut region-specific damage of the nitrergic myenteric neurons of type 1 diabetic rat model. In conclusion our metagenomic studies, chronic hyperglycaemia had the most prominent effect on the lumen-associated microbiota in the ileum (Klebsiella invasion), whereas in the case of mucosa-associated microbiota the major changes were encountered in the duodenum (Mycoplasma invasion). We demonstrated evidence of gut region-specific accumulation of reactive oxygen species and also that enhanced oxidative stress leads to regionally distinct activation of endogenous antioxidants in the different intestinal segments of diabetic rats. In support of our earlier finding that the susceptibility of nitrergic neurons to diabetes is strictly regional, our study provides evidence of gut segment-specific diabetes-related induction of the endogenous heme oxygenase (HO) system and also intestinal region-dependent enhanced co-localization of HO1 and HO2 with neuronal nitric oxide synthase in myenteric neurons. We demonstrated at the first time that the neurochemical character of the nitrergic submucous neurons exhibits gut region-dependent changes in diabetic and insulin-treated diabetic rats. In addition, we prove that HO1- and HO2-immunoreactive submucous neurons are present in small amounts in the small intestine, but in high abundance in the colon of controls and diabetics.
