Nem-kissejtes tüdőrákszövetben változó intenzitású lymphocyta infiltráció tapasztalható. Saját adataink szerint a vérben változatos mértékben felszaporodnak a natural killer (NK), az NKT és az invariábilis NKT (iNKT) lymphocyták amellett, hogy regulatorikus T-sejtek (Treg) is nagyobb számban fordulnak elő. A plazmában magas koncentrációban jelen van interferon-gamma és a tolerogén hatású interleukin-10. A tumor által keltett immunválasz tetten érhető, de egyelőre nem található összefüggés az immunválasz és a túlélés között. Irodalmi adatok szerint az NK, az NKT és főként az iNKT lymphocyták tumorellenes hatása talán elsősorban korai stádiumú és vad típusú p53-at kifejező adenokarcinómában lehet klinikailag is jelentős. Más adatok szerint a Treg sejtek megjelenését a Kras mutáció segíti elő. Négyszáz, tüdő adenokarcinóma miatt operált beteget (IA-IIIA) magába foglaló vizsgálatunkban ezért összehasonlítjuk a p53 és Kras vad típusú és mutáns, de azonos stádiumú adenokarcinómát kísérő NK és Treg választ a tumorban és a perifériás vérben. Összehasonlítjuk p53 és Kras szerint azonos genotípusú, de korai (IA) és előrehaladott (IIB/IIIA) adenokarcinómában kialakuló NK és Treg választ. Reszekált tumorszövetet és perifériás vérmintát vizsgálunk. Az antitumor immunválasz intenzitását az NK és Treg sejtek tumorszöveti, valamint vérben keringő aránya alapján számoljuk. Meghatározzuk az NK sejtek interferon-gamma és perforin szintézisét. Az immunválasz jellegét és erősségét összevetjük a progressziómentes és teljes túléléssel. Eredményeink gazdagíthatják a tumor immunológiai ismereteket és a pulmo-onkológiai prognosztika eszköztárát.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Számos közlemény és saját eredményünk is arra utal, hogy a tüdő nem-kissejtes daganatai együtt járnak a természetes ölősejtek (NK, NKT, iNKT lymphocyták) proliferációjával és ezek a sejtek - változó mértékben - a tumorokat is infiltrálják. Eddig azonban nem sikerült igazolni a veleszületett immunválasz fékező hatását a kedvezőtlen klinikai lefolyásra. Más tumorokban szerzett klinikai és kísérletes adatok szerint a tumorellenes NK válasz hatásosabb, ha a tumor vad típusú (nem mutáns) p53-at fejez ki és korai stádiumú. A tolerogén hatású regulatorikus T-sejt válasz pedig akkor gyenge, ha a sejtek Kras vad típusúak maradnak. A kutatás kérdése ezért az, hogy korai stádiumú (IA) és vad genotípusú (vad p53 és vad Kras) adenokarcinómában erősebb-e az eliminatív immunválasz, illetve gyengébb-e a Treg infiltráció, mint p53 és Kras mutáns, valamint előrehaladott stádiumú (IIB és IIIA) és kettős mutáns adenokarcinóma esetében? A kérdést abban a hisztológiai alcsoportban (adenokarcinóma) vizsgáljuk, melyben - irodalmi adatok szerint - a legvalószínűbb a tumorellenes immunválasz klinikailag is igazolható, kedvező hatása (a progressziómentes és teljes túlélés). A vizsgálatba 400 operábilis adenokarcinómás beteget vonunk be.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

A kutatás jelentőségét az adja, hogy egy régi kérdésre új módon keresi a választ. Ugyanis jelentős ismeretanyag halmozódott már fel a tüdőrákban is zajló veleszületett immunológiai válaszreakcióról, de ennek egyetlen paramétere sem használható a napi pulmo-onkológiai gyakorlatban sem a prognózis meghatározásában, sem a gyógyszeres terápiában (immunterápia). Ezért a tüdőrák ellenes természetes ölősejtes (NK, NKT, iNKT), valamint regulatorikus T-sejtes immunválaszt a betegek egy hisztológiai alapon definiált alcsoportjában (adenokarcinóma) aszerint vizsgáljuk, hogy az adenokarcinóma milyen genotípusú (p53 és Kras), valamint a betegség milyen onkológiai stádiumban van. Azért célszerű a tüdőrák ellenes immunválasz tanulmányozása ilyen módon, mert általában az adenokarcinómák, a korai stádiumú tumorok, valamint a p53 és Kras vad típusú tumorok erősebb NK és gyengébb Treg választ váltanak ki. Más kutatás eddig nem vetette össze ezen karakterek szerint a tüdőrák ellenes NK és Treg választ, valamint ezek hatását a betegség progressziómentes és teljes túlélésére.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

A tüdőrák a vezető daganatos halálok Magyarországon és nemzetközi viszonylatban is. Kezelése műtét, kemoterápia, vagy sugárkezelés. Utóbbiak csak ideig-óráig tartóztatják fel a tüdőrák előrehaladását. Az operált tüdőrákos betegek többségében is visszatér a tüdőrák és ilyenkor ismét csak a kemoterápia és a sugárkezelés marad az orvosok kezében, mint daganatellenes "fegyver". A tervezett kutatás arra keres választ, hogy a szervezet immunrendszere felimeri-e a tüdőrák egy fajtáját, az adenokarcinómát, ez a immunológiai felismerés és az azt követő tumorellenes immunológiai válasz milyen körülmények között erős, vagy gyenge? Azt keressük, hogy a tumor bizonyos genetikai adottságai és előrehaladott, vagy korai stádiuma befolyásolja-e, hogy a tumort támadó, vagy éppen az azt elfogadó (toleráló) immunválasz erős-e? Négy év alatt megvizsgáljuk mintegy 400, operált adenokarcinómás beteg tumorát szövettani módszerrel, valamint áramlás-sejtanalitikai technikával. A laboratóriumi eredményeket összevetjük a vizsgált betegek kórlefolyásával. A kutatás a Semmelweis Egyetem klinikái, intézetei és az Országos Onkológiai Intézet egyes osztályainak együttműködésében valósul meg. A várható eredmények gazdagítják a tüdőrákra vonatkozó tumorimmunológiai tudást és gazdagíthatják a kórlefolyást előre felmérő módszereket (prognosztika).

Summary of the research and its aims for experts

Lymphocytes variably infiltrate non-small cell lung cancers (NSCLC). We have observed a great degree of accumulation of NK, NKT, iNKT and regulatory T (Treg) lymphocytes in the peripheral blood of late-stage NSCLC patients. Plasma of these patients contained IFN-gamma and TNF-alpha, but also IL-10, which induces tolerance of the tumor. No relationship could be uncovered between the immune response and the survival. There is accumulating evidence in other tumors that the clinical impact of the accumulation of the various NK subsets may be significant if the tumor is in an early stage and contains wild-type, not mutant p53. Other data point to mutation of Kras which support the accumulation of Treg-s in adenocarcinoma. Based on these, we aim to compare the intratumor and peripheral blood accumulation of NK and Treg lymphocytes in stage IA versus stage IIB/IIIA as well as in same stage but wild-type versus mutant p53/Kras pulmonary adenocarcinomas. The intesity of antitumor immune response is calculated by the ratio NK/Treg in tumor and peripheral blood. The IFN-gamma, perforin and granzyme B content of NK cells is determined. The function of NK and Treg cells isolated from freshly resected tumor is studied by flow cytometry. Number and function of NK and Treg cells is collated with progression-free and overall survival. Our expected results will enrich current knowledge on immunology of lung adenocarcinoma and support prognostics in pulmo-oncology.

What is the major research question?

Results of other laboratories and our group point to variable degree accumulation of various natural killer (NK, NKT, iNKT) and regulatory T- (Treg) lymphocytes in non-small cell lung cancer (NSCLC). The important issue of a favourable effect of immune response on the clinical course of NSCLC has remained unsettled. Evidence accumulates, that early oncological stage and wild-type p53 predict stronger antitumor NK response. Other data raise the possibility that the response of Treg-s is regulated by the genotype of Kras. Therefore, the research question of this proposal is as follows: first, is the tumor eliminative NK response stronger and the tolerogenic Treg response weaker in early (IA) than later (IIB/IIIA) stage, resectable pulmonary adenocarcinoma; second, is the NK response stronger in p53 wild-type than mutant (same stage) p53 adenocarcinoma; third, is the Treg response weaker in Kras wild-type than mutatnt cases? These immunological mechanisms are to be studied in adenocarcinoma, because genotype and stage influence the immune response as well as the clinical course (progression-free and overall survival) in this particular subtype of lung cancer. We will study 400 patients who underwent or will undergo resection of lung adenocarcinoma.

What is the significance of the research?

The significance of this research is in finding a new way of answering an old question. There are some data about the innate immune response activated by lung cancer, but none of these can be used in prognostics or therapy of this disease. We study the innate immune response in early versus later stage (but resectable) adenocarcinoma and in different genotype subgroups of them (wild-type and mutant p53 and Kras). We planned this project based on preliminary data which indicate that early stage tumors are more immunogenic, than in later stages. Also, wild-type 53 adenocarcinomas can induce stronger NK response, while wild-type Kras adenocarcinomas may induce weaker Treg accumulation in the tumor. The effect of tumor factors like p53 or Kras genotype and stage on antitumor immune response has not yet been elucidated.

Lung cancer is the leading malignant cause of early mortality in Hungary and world-wide. Treatment is surgery, chemotherapy or radiotherapy or the variable combination thereof, but the relentless progression of lung cancer can rarely be prevented. This research aims to study new mechanisms which determine weather the immune system recognizes adenocarcinoma of the lung. We plan to find out if certain genotype of the tumor or the oncological stage (early or advanced disease) are important factors which regulate the power of the antitumor immune response. Within the 4 years of the planned project we intend to study 400 cases of operable lung adenocarcinoma. Cancer tissue will be studied by microscopy and various functional assays which all enable the researcher the measurement of the immunological response against cancer. The laboratory results will be evaluated together with the clinical course of the patients. The work is going to be performed at several departments of Semmelweis University Medical School and the National Oncological Institute. The expected results will add new scientific information to pulmo-oncology and tumor immunology. These results may strengthen the precision of prognosis making in patients suffering from lung adenocarcinoma.

Az I-IIIA. stádiumú tüdő adenokarcinómák (n=121) természetes ölősejtes (NK) beszűrtségének nagyobb mértéke a teljes túlélés (5 és 10 év) megnyúlásával járt. A tumorok NK infiltrációja a stádiumtól (I-II-IIIA) nem függött. Az NK infiltráció kisebb mértékű volt férfiakban mint nőkben, valamint Kras mutáns, mint vad típusúakban. A tumorok Treg infiltrációja szintén a túlélés megnyúlásával párosult, mely váratlan eredmény. A Treg infiltráció a nemtől nem függött, ahogy a stádiumtól, de a Kras genotípustól sem. A tervezettnek megfelelően megvizsgáltuk a tumorok p53 mutációs státuszát is: a 121 mintában összesen 64 féle mutációt találtunk, ezek közül 40 olyat, mely aminosav cseréhez vezetett. A 40 aminosav csere közül 27 volt olyan, mely a p53 funkció teljes megszűnéséhez vezetett (disruptive mutation). A p53 eredmények klinikai adatokkal való összevetése még hátra van. Ugyanígy elvégzendő a kettős (Kras és p53) mutációs terheltség klinikai jelentőségének vizsgálata. A publikáció készül. Elkészült egy kézirat (publikálásra ismételten benyújtva - Clinical Lung Cancer), mely egy eredetileg nem tervezett vizsgálat anyagát tartalmazza. Ebben elsőként igazoltuk, hogy előrehaladott stádiumú (IIIB-IV.) nem-kissejtes tüdőrákban (n=175) hosszabb a progresszió mentes túlélés, ha társbetegségként krónikus obstruktív tüdőbetegség (COPD) is fennáll. A COPD e váratlan kedvező hatása összefüggött a tumoros gyulladás és a tumor által keltett immunszuppresszió enyhítésével.

More intensive infiltration by NK cells of stage I-IIIA lung adenocarcinoma (n=121) was associated with longer overall survival (5 and 10 years). NK infiltration of tumors was not dependent from stage (I-II-IIIA). NK infiltration was less intense in men than women and in Kras mutants than Kras wild-type patients. More intense Treg infiltration was also leading to longer overall survival, which was an unexpected result. Infiltration by Treg-s was not dependent from oncologic stage, gender or Kras genotype. Among the 121 adenocarcinoma tissue samples 64 different mutations were identified. Among these, 40 mutations led to exchange of amino acids within the p53 molecule. Among the latter, 27 amino acid changes could be identified as leading to disruption of the p53 molecule resulting in complete loss of function. The clinical impact of p53 mutations (alone or combined with Kras mutation) have not been analysed, yet. Manuscript is in preparation. One manuscript has been submitted for publication (Clinical Lung Cancer). In this originally not planned study we were the first to show that concomitant chronic obstructive pulmonary disease (COPD) prolonged the progression-free survival of stage IIIB-IV non-small cell lung cancer patients (n=175) during first line chemotherapy. This unexpected favourable effect of COPD was related with amelioration of cancer-induced inflammation and immunosuppression.
