A trombus mesterséges lízise (trombolízise) jelenleg a leghatékonyabb terápia akut iszkémiás stroke esetén. A trombus oldására szolgáló rekombináns szöveti plazminogén aktivátor (rt-PA) kezelés a stroke tüneteinek megjelenésétől számított három órán belül alkalmazható sikerrel. Bár a rt-PA klinikai alkalmazása biztonságos és hatékony az esetek többségében, a terápia legfőbb mellékhatása az akár végzetes kimenetelű intracerebrális vérzés, amely a betegek mintegy 7-8 %-ban fordul elő. Másfelől, az esetek egy részében a terápia hatástalan, a vérrögöt nem sikerül oldani és így a klinikai javulás elmarad. Jelenleg keveset tudunk arról, hogy mért alakul ki vérzéses szövődmény egyeseknél és mért lesz hatástalan a terápia másoknál. A komplikációk előre nem megjósolhatók és kialakulásuk okait általában utólag sem lehet magyarázni. Feltételezhetjük, hogy a terápia kimenetele ill. a vérzéses szövődmények kialakulása összefüggésben lehet a lizálandó alvadék szerkezetével és a fibrinolízis ellen védelmet nyújtó fehérjék szintjével. A legfőbb fibrinolízis inhibitorok, melyek potenciális szerepe felmerül ebben a folyamatban, a véralvadás XIII-as faktora (FXIII), trombin aktiválta fibrinolízis inhibitor és az α2 plazmin inhibitor.
A pályázat célkitűzése hármas:
1/Megvizsgálni, hogy hogyan befolyásolják a FXIII ill. a fibrinolitikus rendszer egyes komponensei a trombolízis sikerét (az elzáródott ér megnyitását)
2/Összefüggést keresni ezen a komponensek szintje és a vérzéses komplikációk között
3/Megvizsgálni, hogy hogyan befolyásolják a FXIII ill. a fibrinolitikus rendszer ismert genetikai polimorfizmusai a trombolitikus terápia kimenetelét

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

Az akut iszkémiás stroke trombolitikus kezelése során a rossz kimenetelért két fő ok tehető felelőssé: a terápia hatástalansága (az elzáródott ér megnyitásának sikertelensége) ill. a vérzéses transzformáció. Mindkét, alapvetően ellentétes irányú véralvadási zavar hátterében jelenlegi ismereteink szerint alapvetően ugyanazok a tényezők állnak (pl. idős kor, súlyosabb neurológiai deficit, stb.), ami alapján arra következtethetünk, hogy számos egyéb, eddig felderítetlen faktor határozza meg a beteg sorsát. A fibrinolízis regulátorainak szerepével kapcsolatban kevés és többnyire ellentmondásos adat áll rendelkezésre. Szinte alig van közlés a véralvadás különböző paramétereinek a vérzéses szövődményekkel való kapcsolatáról. Hátrány, hogy a legtöbb tanulmány relatíve kevés beteg bevonásával készült. A pályázatban szeretnénk részleteiben megvizsgálni a fibrinolízis inhibitorainak szerepét a trombolitikus terápia kimenetelében 130 iszkémiás stroke miatt trombolízisen átesett betegben. A betegektől három alkalommal vennénk vérmintát: közvetlenül a trombolitikus terápia előtt, a terápia után azonnal ill. 24 órával a lízist követően. A mintákból a XIII-as faktor, α2 plazmin inhibitor, trombin aktiválta fibrinolízis inhibitor szintek és főbb polimorfizmusaik meghatározását végeznénk el. A betegek klinikai adatait és a képalkotó vizsgálatok eredményeit a trombolízis előtt, a trombolízis után egy nappal, majd 3 hónappal rögzítenénk. Összevetnénk a stroke rövid ill. hosszú távú klinikai kimenetelét a fibrinolízis regulátorok szintjével ill. a szintek terápia során bekövetkező változásával, összefüggést keresnénk az egyes polimorfizmusok jelenléte és a trombolízis kimenetele között.

Mi a kutatás jelentősége?
Röviden írja le, milyen új perspektívát nyitnak az alapkutatásban az elért eredmények, milyen társadalmi hasznosíthatóságnak teremtik meg a tudományos alapját. Mutassa be, hogy a megpályázott kutatási területen lévő hazai és a nemzetközi versenytársaihoz képest melyek az egyediségei és erősségei a pályázatának!

A rekombináns szöveti plazminogén aktivátorral történő trombolízis forradalmasította az iszkémiás stroke kezelését: egy valóban hatásos terápiás alternatíva lett egy olyan betegség esetén, ahol korábban az orvosi beavatkozások legfeljebb a rehabilitációban és a másodlagos szövődmények elkerülésében merültek ki. Ennek ellenére, több okból kifolyólag, a kezelés nem elérhető minden kórházban. Kisebb kórházakban a kezelést részben a vérzéses komplikációk miatti félelem miatt nem alkalmazzák, holott a terápiára alkalmas betegeket szigorú kritériumok alapján választják ki a szövődmények minimalizása érdekében. Az esetek egy részében viszont az elzárt ér megnyitása nem sikerül és így a klinikai javulás elmarad. A pályázat eredményeként reményeink szerint tisztább képet kaphatunk arról, hogy milyen hemosztázis tényezők befolyásolják a trombolízis kimenetelét, mely a jövőben biztonságosabb és hatékonyabb terápiás gyakorlatot eredményezhet. Hazánkban évente mintegy 45.000 akut iszkémiás stroke eset van. Nyugat-Európában a stroke esetek mintegy 10-30 százalékában történik meg a trombolízis, míg Magyarországon ez az arány még kisebb. Az alvadék struktúráját is befolyásoló főbb fibrinolízis regulátorok tanulmányozása olyan információkat szolgáltathat, mely akár hozzájárulhat ahhoz, hogy a jövőben ki lehessen szelektálni olyan betegeket, akik esetén az időablak kiterjeszthető és így talán még több beteg esetén lesz alkalmazható majd a terápia. A pályázatban végzett klinikai vizsgálat révén reményeink szerint olyan ismeretekre is fény derülhet, melyek kapcsán lehetőség nyílhat az olyan betegek identifikálására még a terápia adása előtt, akikben nagy eséllyel alakulhat ki vérzéses szövődmény. Napjainkban a főbb fibrinolízis regulátorok (elsősorban a trombin aktiválta fibrinolízis inhibitor (TAFI), a faktor XIII (FXIII) és az α2 plazmin inhibitor) farmakológiai inhibitorainak kutatása intenzíven zajlik. A TAFI és a FXIII esetén az inhibitorokat a trombolízis kiegészítő terápiájaként állatkísérletekben már tesztelték és a jövőbeli humán alkalmazásuk ígéretes lehetőségnek tűnik. A fibrinolízis regulátorok trombolízisben betöltött szerepének jobb megismerése segíthet az új inhibitorok terápiás értékének becslésében.

A kutatás összefoglalója, célkitűzései laikusok számára
Ebben a fejezetben írja le a kutatás fő célkitűzéseit alapműveltséggel rendelkező laikusok számára. Ez az összefoglaló a döntéshozók, a média, illetve az érdeklődők tájékoztatása szempontjából különösen fontos az NKFI Hivatal számára.

Az iszkémiás stroke, népies nevén szélütés az agy vérellátási zavarát jelenti, mely akkor alakul ki, amikor egy vérrög elzárja az egyik agyi ütőeret. Ennek eredményeképpen az adott agyi régió működése sérül, amely bénultsághoz, a beszéd vagy beszédértés zavarához vagy látászavarhoz vezethet. A vérrög feloldását trombolízisnek nevezzük, mely jelenleg a leghatékonyabb terápia az agy vérellátásának javítására és a így a tünetek javítására vagy akár megszüntetésére. A trombolízis a tünetek jelentkezésétől számított három órán belül alkalmazható sikerrel. Bár a terápia bizonyítottan hatásos és biztonságos az esetek többségében, a betegek egy részében nem sikerül a vérrögöt feloldani és így a kezelés hatástalan lesz, másoknál pedig agyi vérzéses szövődmény alakulhat ki. A legtöbb esetben ezeket a komplikációkat nem lehet előre megjósolni és a kialakulásuk oka felderítetlen marad. A véralvadási rendszernek számos olyan eleme van, amely a vérrögök szerkezetét és természetes felbomlását szabályozza, ezeket összefoglalóan fibrinolízis regulátoroknak nevezzük. A kutatási projektben ezen regulátorok szerepét kívánjuk vizsgálni a trombolitikus terápia kimenetelében. Terveink szerint 130 iszkémiás stroke-on átesett betegtől vennénk vérmintát a trombolízis előtt, a trombolízis után azonnal és 24 órával a trombolízist követően. Meghatároznánk a fibrinolízis regulátorok szintjét és genetikai variációit és az eredményeket összevetnénk a terápia sikerével. A pályázat eredményeként reményeink szerint tisztább képet kaphatunk arról, hogy milyen tényezők befolyásolják a trombolízis kimenetelét, mely a jövőben biztonságosabb és hatékonyabb terápiás gyakorlatot eredményezhet.

Thrombolysis is the most effective pharmacological approach in acute ischemic stroke to restore or improve functional capacity. Recombinant tissue plasminogen activator (rt-PA) can be administered for the lysis of thrombi within the first 3 hours after the onset of stroke symptoms. Although clinical application of rt-PA has proven to be safe and effective in most cases, the main risk of treatment is potentially fatal intracerebral hemorrhage, which occurs in approximately 7-8% of patients. On the other hand, treatment is not efficient in all cases and in some patients there is a lack of neurological recovery. Little is known why in some patients thrombolytic therapy is less efficient and which are the factors promoting intracranial hemorrhage in others. In most cases, these complications cannot be foreseen at the initiation of therapy. As these patients have higher risk of death or life-long disability, it would be important to identify the underlying mechanisms. It is plausible that effectiveness as well as adverse effects of the thrombolytic agent are likely to depend on factors influencing fibrinolysis and clot structure. Key fibrinolytic regulators of interest in this respect include blood coagulation factor XIII, thrombin activatable fibrinolysis inhibitor and α2 plasmin inhibitor.
The proposal has three major aims:
1/ To find out how FXIII and other components of the fibrinolytic system could influence the success of thrombolysis (the opening of the closed vessel)
2/ To find out how these components influence bleeding complications
3/ To investigate whether known polymorphisms of FXIII and the fibrinolytic system have an influence on the outcome of thrombolysis

What is the major research question?

Adverse outcomes associated with the thrombolytic treatment of acute ischemic stroke are mainly attributable to two major causes: the failure to open the closed vessel (inefficacy of therapy) and to hemorrhagic complications. Albeit opposite mechanisms, known predictors of worse functional outcome due to inefficacy are substantially the same as those of symptomatic intracranial hemorrhage (e.g. older age, worse neurological deficits, etc.), which suggests that there must be a number of other factors influencing clinical outcomes. Data in the literature regarding the role of fibrinolysis regulators in the outcome of thrombolytic therapy is limited in many aspects and reports are often contradictory. Almost no data is available on hemostasis factors associated with bleeding risk during thrombolysis. Moreover, most studies include relatively small number of patients. The proposal is planned to extensively investigate the role of fibrinolytic inhibitors in the outcome of thrombolysis in 130 ischemic stroke patients who all underwent thrombolytic therapy. Blood samples will be drawn on three occasions: before the initation of therapy, after the administration of therapy and 24 hours after thrombolysis. The following investigations will be carried out: levels of factor XIII, α2 plasmin inhibitor, thrombin activatable fibrinolysis inhibitor and determination of their major polymorphisms. Clinical data of patients and results of imaging tests will be registered before thrombolysis, after therapy and 3 months after the event. Results of the biochemical and genetic tests would be correlated with the severity and with short- and long-term functional outcome of the disease.

The clinical application of thrombolysis by recombinant tissue plasminogen activator revolutionized ischemic stroke care by offering an effective treatment for a disease in which previously medical effort could mainly be focused on rehabilitation and avoidance of secondary complications. However, for multiple reasons, treatment is not available in every hospital. Limited availability in inexperienced hospitals is partly due to fears of bleeding complications, despite the fact that those eligible for therapy are carefully selected to minimize bleeding risk. On the other hand, in a subset of patients thrombolytic therapy is inefficient due to the failure of the recanalization of the closed vessel. As a result of the proposal we hope that the role of hemostatic factors which influence thrombolysis outcome would be better clarified, which perhaps could lead to more efficient and safer thrombolysis practice in the future. Approximately 45 000 persons suffer ischemic stroke per year in Hungary. In Western countries, 10-30% of stroke events are treated by thrombolysis, while in Hungary it is even less. It is our hope that gaining knowledge on the factors influencing side effects and clinical outcomes could help to extend the availability of this potent therapeutic option. Studying the main regulators of fibrinolysis, which influence clot structure might provide information that could help to identify future patients in whom therapeutic time window could be extended and perhaps more patients could benefit from this effective therapy. The proposed study may also identify factors associated with bleeding complications, which might help to identify those with elevated risk. Today, pharmacologic inhibitors of the key regulators of fibrinolysis (including thrombin activatable fibrinolysis inhibitor (TAFI), factor XIII (FXIII) and α2 plasmin inhibitor) are in the focus of extensive research. In case of TAFI and FXIII, inhibitors as supplemental therapy to thrombolysis are tested in animal models and their application in human practice could be a promising approach. Clarifying the effect of fibrinolysis regulators in thrombolysis could help to better estimate the therapeutic values of their inhibitors.

Ischemic stroke occurs when an artery is blocked in the brain by a blood clot and this leads to the disturbance in the blood supply. As a result, the affected area of the brain cannot function, leading to inability to move limbs, to understand or formulate speech or to see the whole visual field. Dissolvement of the blood clot, called thrombolysis is the most effective treatment to restore or improve these functions. The thrombolytic drug can be administered within the first 3 hours after the onset of symptoms. Although this clot-busting drug is proven to be effective and safe in most cases, in some patients it is less efficient because the blood clot is not dissolved and in others side effects, such as bleeding in the brain occurs. In most cases, these complications cannot be foreseen at the initiation of therapy and their occurrence remains unexplained. The blood clotting system has a number of elements, called fibrinolysis inhibitors, which regulate the structure of blood clots and their natural break-up. The proposal is aimed to investigate the role of these factors in the outcome of thrombolysis. We are planning to investigate the blood samples of 130 ischemic stroke patients. Blood samples will be drawn on three occasions: before and after the administration of therapy and 24 hours after thrombolysis. The levels of fibrinolysis inhibitors and their genetic variations will be measured and compared with the success rate of thrombolytic therapy. As a result of the study we hope that the role of factors which could influence thrombolysis outcome would be better clarified, which perhaps could lead to more efficient and safer thrombolysis practice in the future.

A rekombináns szöveti plazminogén aktivátorral (rtPA) végzett trombolízis az egyik leghatékonyabb farmakológiai terápia iszkémiás stroke esetén. A lízis azonban sokszor hatástalan, másoknál pedig vérzéses komplikációk lépnek fel. Feltételeztük, hogy a terápia kimenetele összefüggésben lehet a fibrinolízis ellen ható fehérjék szintjével. A vizsgálatba 132 konszekutív, akut iszkémiás stroke-on átesett beteg került, akik i.v. trombolítikus terápiában részesültek. A betegektől felvételkor, az rtPA után és 24 órával később vettünk vért; meghatároztuk a főbb fibrinolitikus és néhány egyéb hemosztázis faktor szintjét és gyakori polimorfizmusait. A betegek klinikai adatait a kórházi felvételkor és a 7. napon rögzítettük. Az intrakraniális vérzést az ECASS II szerint osztályoztuk. A rövidtávú kimenetelt a neurológiai státusz (NIHSS) 7. napi változása alapján, a 90 napos kimenetelt a modifikált Rankin skála (mRS) szerint ítéltük meg. Eredményeink azt mutatták, hogy 24 órával a lízis után mért alacsony XIII-as faktor szint a rövid távú (<14 nap) mortalitás független prediktora. A felvételkor mért alacsony endogén trombin potenciál (ETP) a hosszú távú (<90 nap) mortalitás független prediktorának bizonyult, és szignifikáns összefüggést mutatott a terápia-asszociált szimptomatikus intracerebrális hemorrhagia (sICH) kialakulásával. A lízist követően mért magas VIII-as faktor és von Willebrand faktor szintek a 90. napi rossz funkcionális kimenet (mRS>3) független prediktorai.
Results in English
Thrombolysis with recombinant tissue plasminogen activator (rtPA) is one of the most effective pharmacological therapies for ischemic stroke. However, lysis is often ineffective while in others bleeding complications occur. We hypothesized that the outcome of therapy may be related to levels of proteins regulating fibrinolysis. The study included 132 consecutive acute ischemic stroke patients who all underwent i.v. thrombolytic therapy. Blood samples were taken on admission, immediately after rtPA and 24 h later; major fibrinolytic and some other hemostasis factor levels and their common polymorphisms were determined. Clinical data of patients were recorded at on admission and on day 7. Intracranial bleeding was classified according to ECASS II. Short-term outcomes were assessed according to the change in neurological status (NIHSS) by day 7, long-term outcome was judged by the modified Rankin Scale (mRS) on day 90. Our results showed that a low factor XIII 24h after lysis is an independent predictor of short-term mortality (<14 days). A low endogenous thrombin potential (ETP) at the time of admission proved to be an independent predictor of long-term mortality (<90 days) and showed a significant correlation with therapy-associated symptomatic intracerebral hemorrhage (sICH). Elevated factor VIII and von Willebrand factor levels after lysis are independent predictors of poor long-term functional outcome (mRS> 3).
