A stress-neurobiológia fontos kihívása annak megértése, hogy egyes embereknél miért alakulnak ki stresszel kapcsolatos mentális betegségek, míg másoknál nem. A genetikai háttér mellett a stressz-indukálta szorongás és a depresszió kialakulásában a gyulladás fontos rizikó faktor. Hipotézisünk szerint, a stressz felborítja a gazdaszervezet és a bél mikrobiom közötti immunregulációs homeosztatikus kapcsolatot, ami gyulladáshoz vezet mind a központi idegrendszerben, mind a periférián, ezzel elősegítve a kedélybetegségek kialakulását. Kísérleteinkben krónikus változó stressz sorozatot alkalmazva szeretnénk fel tárni a stressz-indukálta immun- és magatartási eltéréseket patkányokon. Azt bizonyítandó, hogy ezek az eltérések összefüggésben vannak a bél mikroflórájával, a patkányokat stressz közben rifaximinnel, egy gasztrointestinális rendszerben ható (nem szisztémás hatású) antibiotikummal kezeljük. Laktobacillusokat izolálunk a rifaximinnel kezelt, stresszelt állatok bélflórájából és ezek probiotikus potenciálját, a már ismert probiotikus hatású, E coli Nissle 1917 törzzsel együtt vizsgáljuk, hogy képesek-e anti-inflammatorikus hatást kiváltani és egyben csökkenteni a krónilus stressz-okozta szorongást és depressziót? A regulatoros T (Treg) sejtek depléciójával fogjuk ellenőrizni, hogy a probiotikumok anti-inflammatorikus hatását ezek a sejtek közvetítik-e. Végül, human kísérletben depressziós pácienseken fogjuk validálni az E coli Nissle 191 tartalmazó probiotikus készítmény (Mutaflor) hatásosságát.

Mi a kutatás alapkérdése?
Ebben a részben írja le röviden, hogy mi a kutatás segítségével megválaszolni kívánt probléma, mi a kutatás kiinduló hipotézise, milyen kérdéseket válaszolnak meg a kísérletek.

A kísérletek általános célja, hogy felfedezzük azokat a mechanizmusokat, melyekkel egyes probiotikus baktérium törzsek helyreállítják a gazdaszervezet és a mikrobiom közt megromlott homeosztatikus kapcsolatot, antiinflammtorikus körülményket teremtve, melyek eredményeként aktív megküzdési stratégiák kerülnek előtérbe, csökken a szorongás és a depresszió mértéke. Azok közül a mechanizmusok közül, melyek komplex módon összekötik a mikrobiom-bél agy tengelyt, tervezett kísérleteinkben a mikrobiomnak a veleszületett és a szerzett immunitásra gyakorolt hatására fókuszálunk. Ezen belül azt vizsgáljauk, hogy hogyan hat a krónikus váltakozó stressz a mikrobiom összetételére, hogyan változik meg ezáltal az immunreguláció, hogyan alakul ki krónikus gyulladás és hogyan változik a megküzdési stratégia, alakul ki szorongás és depresszió. Másik célunk, hogy speciális probiotikumokat vezessünk be, melyek helyreállítva a bélflóra és a gazda közti normális immunregulációt megelőzzék a krónikus stress okozta pszichés elváltozások kialakulását és segítsék a stress ellenállóképességet. Végül egy olyan, forgalomban lévő probiotikus készítmény hatását (E coli Nissle 1917, Mutaflor), fogjuk vizsgálni depressziós betegeken, amelynek anti-inflammatorikus hatása gyulladásos bélbetegségekben már bizonyított.

Mi a kutatás jelentősége?

A bél mikrobiom és a központi idegrendszer közti komplex kapcsolat vizsgálata a neurobiológia egy új fejezete. Az utóbbi években egyre több adat utal arra, hogy a bél mikroflórája hormonális, idegi és immunkapcsolatokon keresztül képes befolyásolni az agy működését. Ezen a szövevényes rendszeren belül, vizsgálatainkban a mikrobiom immunmodulációs funkciójára fókuszálunk, mivel a gazdaszervezet és a mikrobiom közt megbomlott kapcsolat kiindulópontja lehet olyan gyulladásos folyamatoknak, melyek súlyosbítják egyes pszichiátriai kórképek (szorongás, depresszió, pánik) kialakulását. Mindazonáltal immunregulációs/antiinflammatorikus hatású baktériumokkal való kezelés nagy potenciát rejt magában a központi idegrendszeri betegségek gyógyításában. Ezért a jelen tervezetben két probiotikus baktériumtörzs hatását fogjuk tesztelni, hogy antiinflammatorikus hatásuk révén képesek-e kivédeni a stressz-indukálta gyulladásos folyamatokat és az ezzel kapcsolatos magatartási eltéréseket. Tervezetünk további sajátossága, hogy megállapítsuk az E. coli Nissle 1917 –t tartalmazó probitikus készítmény hatékonyságát depressziós betegeken.

A kutatás összefoglalója, célkitűzései laikusok számára

Napjainkban világossá vált, hogy a bélcsatornában élő baktérium közösség, a mikrobiom hatással van szervezetünk minden működésére, ideértve a központi idegrendszert, sőt a magatartást is. Modern világunkban a megváltozott étkezési szokások és az állandó stressz miatt a mikrobiom összetétele megváltozik és a szervezet ellenálló képességét befolyásoló mikroorganizmusok visszaszorulása, valamint a stressz következtében felboruló egyensúly miatt előtérbe kerülnek olyan gyulladásos kórképek, amelyek rizikó faktorai egyes pszichiátriai betegségeknek (szorongás, depresszió, poszt-traumás stressz betegség PTSD, pánik). Kutatásaink célja olyan probiotikus baktériumok felfedezése, melyek gyulladáscsökkentő hatásuk révén, megelőzik egyes, stressz-indukálta idegrendszeri betegségek kialakulását és kiegészítő terápiaként használhatók ezek gyógyításában.

A major challenge in stress biology is to understand neurobiological background of individual differences in stress reactivity. Some individuals are vulnerable to various stress-related psychopathologies, while others became resilient. In addition to genetic predisposition, inflammation is an important risk factor to develop anxiety and depression in response to chronic, psychogenic stress. It is hypothesized that stress disrupts the homeostatic immune-regulatory relation between the host and microbiome, which results in inflammatory changes at the periphery and in the central nervous system, facilitating the development of mood disorders. Using chronic variable stress in rats, first, we will uncover stress-induced changes in central and peripheral inflammation and behavior. Next, we will use rifaximine, a gut targeted antibiotic to test if stress-induced behavioral and immune changes are critically dependent on the microbiome activity. Lactobacilli will be isolated from rifaximine treated, stressed animals and tested for psychobiotic potential together with a known Gram negative probiotic strain E. coli Nissle 1917. We intend to investigate if these bacteria shift the immune balance towards anti-inflammation and confer resilience to stress-related anxiety and depressive behavior. Depletion of regulatory T cells (Treg) will reveal the dependence of probiotic induced anti-inflammatory changes on this cell population. Finally, preclinical results will be translated to human studies in which the efficacy of probiotic E. coli Nissle 1917 (Mutaflor) will be tested in depressive patients and in a human experimental stress model.

What is the major research question?

The overall aim of this project is to reveal the mechanisms with which probiotic bacteria restore disrupted homeostatic regulation between gut microbiome and the host by conferring anti-inflammatory immune-regulatory changes, which result in pro-active coping, stress resilience, anxiolysis and anti-depressive effects. To tease apart the various mechanisms at play in the complex communication network in the microbiome-gut-brain axis, in this proposal, we will focus on the innate and adaptive immune system as a target at the host-microbiome interaction and as the major risk factor in anxiety and depression.
More specifically, we will characterize how chronic variable stress (CVS) affects rodent microbiome/ how these changes result in compromised innate and adaptive immune regulation, low grade inflammation, neuroinflammation and changes in stress coping, anxiety, and depressive behavior.
Our further aim is to isolate and characterize select probiotic bacteria, which are able to prevent chronic stress-induced hormonal-, immune and behavioral pathology and to confer stress resiliency.
In addition, a probiotic strain (E coli Nissle 1917, Mutaflor), which have been shown to have anti-inflammatory effect in the gut, will also be tested in human acute stress scenario and in patients with depressive symptoms.

Understanding the complex interaction between the microbiome-gut and the central nervous system is an emerging field in neuroscience. Accumulating data now indicate that the gut microbiota communicates with the CNS through neural, hormonal and immune pathways and thereby influences brain function and behavior. Among these possibilities our proposal focuses on the immunoregulatory potential of the gut microorganisms because the disrupted homeostatic regulation between the microbiome and the host results in inflammation, which is an important risk factor for stress-related neuropsychiatric diseases such as anxiety, depression and panic. Providing bacteria with immunregulatory/anti-inflammatory potential to the gut microbiome holds a great promise for the treatment of inflammation-related disorders, including those at the CNS. To this end, in the framework of this proposal we will compare in rats, the ability of two probiotic strains to prevent stress-induced inflammatory changes and related behavioral alterations. The other specificity of our proposal is the intention to confirm the utility of probiotic strain E. coli Nissle 1917 in depressed human patients.

It has recently become evident that gut microbiota can influence all aspects of physiology, including gut–brain communication, brain function and even behavior. The composition of the microbiome in the modern societies has changed due to dietary factors and increasing stress burden. Lack of exposure to immunregulatory microorganisms and stress-induced disrupted communication between the microbiome and host results in chronic inflammation, which is a significant risk factor for many diseases including mental diseases, such as anxiety, depression, post-traumatic stress disorder (PTSD). Our aim in this proposal to find probiotic bacteria with which to prevent stress-induced pathology and support a strategy for adjunct bacterio-therapy for metal well being.

A strressz ellenállóképesség (reziliencia) a stresszkutatás fontos és megoldatlan kérdése. Hipotézisünk az volt, hogy krónikus stresszben a dysbiotikus bél mikrobiom felborítja a gazdaszervezet immunregulációs homeosztatikus szabályozását és olyan szisztémás gyulladáshoz vezet, aminek következménye az idegi gyulladás (neuroinflammation) , ami különböző kedélybetegségek kialakulásához vezethet. Kutatásaink alátámasztották, hogy krónikus stressz hatására a bél mikrobiom összetétele megváltozik, ahogyan a bél áteresztőképessége is, ami bakteriális terhelést okoz a szervezetben. Ezzel párhuzamosan a krónikus stressz hatására szorongásos, anti-szociális magatartás elemek figyelhetők meg az állatokon. A bélflóra rendezése antibiotikum kezelés hatására ugyan kivédi a gasztrointesztinális és az immunrendszerben detektálható stressz-indukálta kóros változásokat, de a krónikus stressz hatására kialakult szorongást nem védte ki. Munkánk során ugyanakkor azonosítottunk egy Lactobacillus törzset, aminek stresszcsökkentő, szorongáscsökkentő, pro-szociális és pro-reprodukciós hatását fedeztük fel.
Results in English
Stress resilience is an important although unresolved issue in stress research. We hypothesized that chronic stress shifts the gut microbiome towards a dysbiotic stage and results in adverse effects in host immunoregulation which translates into neuroinflammation and related psychopathologies. In the framework of this grant we have revealed stress-induced changes in the composition of the gut microbiome, which has been accompanied with leaky gut and systemic bacterial load. These gastrointestinal symptoms appeared in parallel with stress-induced anxiety and anti-social behavioral changes. Restoration of gut microbiome by gut acting antibiotic rifaximine prevented stress-induced gastrointestinal and immune alterations, however was not able to decrease stress-induced adverse behavioral changes. During these experiments we have identified a Lactobacillus strain refered to as TV45, with potential anxiolytic, pro-social and pro-reproductive effects.
