Az Escherichia coli által okozott húgyúti fertőzések az iparilag fejlett országokban a leggyakoribb bakteriális fertőzések közé tartoznak, súlyos egészségügyi és gazdasági kihatásokkal járnak. Ennek megfelelően az uropathogenitás szolgálatában álló virulencia faktorok azonosítására és jellemzésére széleskörű vizsgálatok irányulnak. Ezen faktorok jelenlétének, baktériumtörzseken belüli konstellációjának megismerése a virulencia tényezőkre irányuló etiológiai diagnózist segítheti elő, egyben új célmolekulákat tárhat fel terápiás beavatkozások és a fajlagos prevenció számára.
A céloknak megfelelően nagyszámú uropathogén E. coli izolátum virulencia faktor készleténeknek elemzését kívánjuk elvégezni az eddigi vizsgálatoknál jóval szélesebb faktor palettára kiterjedően. További új megközelítés, hogy a legtöbb virulencia faktorral rendelkező törzsekből egy-egy virulencia faktorban hiányos mutáns sorozatot állítunk elő, majd ezekkel végzünk egérfertőzést. A vad törzs virulenciájával összehasonlítva így felmérhetjük az egyes virulencia faktorok szerepét a törzs teljes virulenciájában.
Az uropathogén E. coli emellett lehetőséget kínál a bakteriális virulencia egyik kulcsfontosságú evolúciós lépésének kísérletes modellezésére. A pathogenitási szigeten elhelyezkedő haemolysin operonnak a Proteus archetípusból E. coli-ba történő horizontális géntanszferét kívánjuk végrehajtani először in vitro, majd in vivo rendszerben. Mivel a húgyúti fertőzések forrása leggyakrabban a beteg bélflórája, az in vivo transzferkísérletet egerek bélcsatornájában végezzük. Emellett az E. coli speciesen belül egy pathogenitási sziget in vivo átvitelét modellezzük hasonló kísérleti rendszerben. A szigeten kódolt virulencia géneknek a rekombinánsokban való expresszióját aszcendáló húgyúti egérfertőzéses modellben vizsgáljuk. Az átvitel végrehajtása egy pathogentiási sziget in vivo transzferének első példája lenne, ezért további pathogenitási szigetek átvitelének módszertani modelljéül szolgálhatna.
Summary
Urinary tract infections caused by Escherichia coli are among the most frequent bacterial manifestations in the industrialized countries presenting with severe health and economic burden. Consequently, identification and characterization of urovirulence associated factors represent priority areas for research interest. Extended knowledge on the presence and strain-related constellation of virulence factors might promote a pathogenicity oriented etiological diagnosis, and might explore new target molecules for antimicrobial intervention and specific prevention.
For this purpose we investigate the virulence factor constellation of a large number of isolates extending the studies to a significantly broader range of factors than ever studied. As a new approach individual virulence genes of selected isolates with the broadest virulence factor palette will be knocked out, and their virulence will be assessed in mouse models. In this way the share of these genes in the total virulence can be assessed in a uniform genetic background.
Furthermore, uropathogenic E. coli provides a possibility to experimentally prove a postulated step of virulence evolution. We perform model experiments to mobilize the pathogenicity island associated haemolysin operon form the archetype Proteus into E. coli. First in vitro transfer will be modelled, and than in vivo transmission experiments will be done. As urinary tract pathogens originate most commonly from the patient’s intestinal flora the in vivo transfer experiment will be performed in the mouse intestine. Iside species transfer of the pathogenicity island fom E. coli to E. coli will also be modelled in vivo. Expression of the island-coded virulence genes in the recombinants will be studied by ascendent urinary tract infection in mouse. Fulfilment of the transfer would be the first example for the in vivo transmission of a pathogenicity island, and could serve as a methodological model for further island transfers.

Az Escherichia coli fő húgyúti virulencia faktorainak vizsgálatával bizonyítottuk, hogy az alfa hemolizin a toxicitásért felelős legjelentősebb pathogenetikai tényező, emellet a tok és fimbriák a kolonizációban jászanak fontos szerepet. A haemolysin operont tartalmazó pathogenitási szigetet in vitro és egerek bélscatornájában in vivo is át tudtuk vinni a genus más képviselőjébe. Emellett a hemolizin operon kicserélhetőségét bizonyítottuk Escherichia és Proteus törzsek között szintén in vivo környezetben is. A hemolizáló rekombinánsok virulenciája a recipienshez képest szignfikánsan magasabb. Hibridizációs kísérletekkel az E. coli és Proteus penneri haemolizin operonjainak homológiáját bizonyítottuk. Gyakorlati jelentőségű megfigyelés, hogy a rezisztens hemolizáló törzs antibiotikum kezelés esetén a bélcsatornában elszaporodva életet veszélyeztető fertőzés forrása lehet. A hemolzin és fimbria operonok szabályozásában jelenős mechanizmusokat tártunk fel.
Results in English
In mouse experiments we have proved that alpha-haemolysin with its toxic effect is the main urinary virulence factor of E. coli. Besides fimbrial adhesins and the capsule are important in the first colonisation step of the infection. The pathogenicity island containing the haemolysin operon could be transferred from the wild type strain to non-hemolytic E. coli strains in the mouse intestine. Furthermore, the haemolysin operon is exchangable in vivo between E. coli and Proteus strains. The haemolytic recombinants present with significantly increased virulence in various mouse models. With hybridisation experiments we could prove homology between the haemolysin operons of E. coli and Proteus penneri. The observation that antibiotic resistant haemlysin producing E. coli in the microbiota may elicit severe infection in the antibiotic treated patients may be of paractical significance. Regulatory machineries governing the expression of haemolysin and fimbria operons were also elucidated.
