A DEOEC Bőrklinikán 1963 óta folyó, 1991-től OTKA támogatással történő fotodermatológiai kutatómunkában terveink a következők.
1/ Genetikai vizsgálatok: génanalízis bevezetése (UROD-mutációk keresése, polimorfizmus vizsgálata) a porphyria cutanea tarda (PCT) familiáris típusában; további részletes genetikai vizsgálatok egy PCT-s családban, amelyben eddig nem identifikált UROD mutációt detektáltunk; HHC gén mutációk detektálása és a HCV fertőzöttség szűrése az Intézet teljes PCT beteganyagában; 23 tagú erythropoeticus protoporphyriás (EPP) beteganyagunk genetikai feltérképezésének befejezése.
2/ Az ABCG2 transzport fehérje eddig még nem vizsgált lehetséges szerepének tanulmányozása EPP-ben és egyes fototoxikus reakciók patogenezisében.
3/ A fotoallergiás hátterű polymorph fény-exanthemában az UV-fény által károsodott immunszuppressziót okozó faktorok tanulmányozása (Langerhans sejtek, citokinek, receptorok) in vivo és in vitro.
4/ Kérdőíves epidemiológiai vizsgálatok az UV-sugárterhelés felmérésére egészséges populációban és bőrtumoros betegekben.
A kutatási feltételek a résztvevők munkahelyén adottak. A komplex célkitűzések megvalósítását az teszi lehetővé, hogy a résztvevő kutatók több szakterületet és kutatóintézetet képviselnek.
A várható eredmények segítséget nyújtanak a napfény okozta mind nagyobb számú dermatosis és bőrkárosodás patomechanizmusának jobb megismeréséhez, a genetikai fogékonyság megítéléséhez és az eredményes prevencióhoz.

Research work on photodermatology supported by OTKA grants since 1991 has been performed at the Department of Dermatology in Debrecen since 1963. To continue the investigations our plans are as follows:
1/ Genetic study: Establishment of genetic analysis (detection of UROD mutations, polymorphism) in the familial form of porphyria cutanea tarda (PCT); further genetic study of a family with PCT in which we detected an UROD gene mutation unidentified till now; detection of HHC gene mutations and HCV seropositivity in the total PCT patient material in Debrecen; to finish the genetic mapping of our patient material with erythropoietic protoporphyria (EPP).
2/ To investigate the possible role of ABCG2 transport protein not studied till now in EPP and in the pathogenesis of some phototoxic reactions.
3/ In vivo and in vitro study of some factors (Langerhans cells, several citokines and receptors) inducing an immunosuppression damaged by UV light in polymorphic light eruption of photoallergic origin.
4/ Epidemiologic investigations by means of a questionary to estimate UV-loading of the healthy population and patients with skin tumours.
The infrastructure and the conditions of the research are available at the departments of the participants. Achieving the complex goals is facilitated by the multidisciplinary background of the applicants.
The results yield better understanding of the pathomechanism of sunlight induced skin diseases and cutaneous damage, estimation of the genetic susceptibility and the successful prophylaxis.

Munkacsoportunk évtizedek óta foglalkozik az ultraibolya fény élettani és patofiziológiai hatásaival klinikai és molekuláris szinten. Jelen pályázat futamideje alatt több photodermatologiai témájú összefoglaló közleményt és egy monográphiát publikáltunk. 3 testvéren extrém ritka hepatoerythropoeticus porphyriát diagnosztizáltunk, és a hátterében lévő compound heterozigóta mutációkat is identifikáltuk. A sporadikus porphyria cutanea tardában gyakoribb haemochromatosis mutáció és környezeti provokáló tényezők előfordulási gyakoriságáról bővítettük ismereteinket. UVB besugárzás után néhány ABC transzporter fehérje in situ, purinerg receptorok és inflammasoma alkotó elem mRNS és fehérje szintű in vitro expresszió-változását mutattuk ki keratinocita tenyészeteken. Komplettáltuk a keratinocita differenciációs teljes genom expressziós vizsgálatokat. Az irodalomban elsőként igazoltuk, hogy in vitro szintetizált módosított erszényes fotoliáz mRNS bejuttatható volt keratinocita tenyészetekbe és funkcionálisan aktív fehérje szintetizálódott róla. Tissue microarray analízis felhasználásával macrofágok, dendritikus sejtek és egy cistein tartalmú fehérje prognosztikai szerepét vetettük föl melanoma malignumban. Epidemiológiai vizsgálatunk során rámutattunk arra, hogy továbbra is fontos feladat a fényvédelem propagálása, mert 13 és 16 éves iskolás gyermekeken az anyajegyek száma magas, a gyermekek közel 40%-a napégésről, 3,6%-uk szolárium használatról számol be.

Our workgroup has investigated the physiological and pathophysiological effects of ultraviolet radiation at clinical and molecular levels for decades. Over the period of the application several photodermatological articles and one monograph were published. We diagnosed the extremely rare hepatoerythropoietic porphyria in a family, and identified compound heterozygous mutations in the background. We expanded our knowledge about the frequency of haemochromatosis mutation and environmental provoking factors in sporadic porphyria cutanea tarda. We demonstrated alterations in the expression of some ABC transport proteins in situ, and in the expression of purinerg receptors and inflammasome elements at mRNA and protein levels in vitro after UVB radiation in keratinocytes. We completed the whole-genom expression analysis related to keratinocyte differentiation. We proved for the first time that the in vitro synthesized modified marsupial photolyase mRNA can be delivered into keratinocytes, and that the translated protein is functionally active. We suggested the prognostic role of macrophages, dendritic cells and one cysteine-containing protein in melanoma malignum using tissue microarray analysis. During epidemiological studies we pointed out that propagation of light protection is an important task in the future, because the number of birthmarks is high among 13 and 16 year-old young people, and 40% of youngsters reported about sunburn and 3,6% of them use solarium.
