Az újszülöttkori agyi hipoxiát/iszkémiát követő idegrendszeri károsodások kialakulásában direkt neuronális mechanizmusok, valamint indirekt vaszkuláris-endothelialis és glia folyamatok is fontos szerepet játszanak. Hipotézisünk szerint a mitokondriumok belső membránjában található ATP-szenzitív káliumioncsatornák (mitoKATP) farmakológiai aktivátorai által közvetített neuroprotekció direkt neuronális hatások mellett legalább részlegesen a cerebrovaszkuláris endothelium/simaizom funkciójának védelmén keresztül valósul meg. A mitoKATP nyitók prototipusának tekinthető diazoxid kémiai prekondicionáló hatású, de más támadásponton keresztül a mitokondriális szuperoxidtermelést is fokozza, amely szintén prekondicionáló lehet önmagában is. A diazoxid mellett ezért egy másik mitoKATP aktivátort (BMS191095) is vizsgálni fogunk, mert izolált malac mitokondiumokban a BMS nem fokozta a szuperoxidanion termelést.
Tervezett vizsgálatainkban tovább szeretnénk jellemezni a mitoKATP aktiváció endothelialis/neuronális citoprotektív hatásának fiziológiai és sejtszintű mechanizmusait újszülött malacban. Ezért azt a hipotézist kívánjuk tesztelni, hogy a mitoKATP csatorna nyitásával kiváltott azonnali kémiai prekondicionálás független-e a mitokondriális ROS képződéstől az újszülött agy sejtjeiben in vivo. A prekondicionálás eredményességét különböző iszkémia-szenzitív neuronális-vaszkuláris reakciók megtartottságával, ill. a vér-agy gát integritásának fennmaradásával fogjuk tesztelni. A válasz specificitását különböző mitoKATP illetve plazmamembrán KATP (sKATP) szelektív antagonisták felhasználásával vizsgáljuk. A véráramlási változásokat intravitális mikroszkópia, laser-Doppler, és laser-speckle (folt interferencia) módszerekkel szeretnénk analizálni, ami lehetőséget nyújt a pia mater és az intraparenchymalis arteriolák szelektív izsgálatára. A vér-agy gát károsodását/integritását az Evans-kék plazmaextravazáció módszerével mutatjuk ki. A diazoxid ill. a BMS által kiváltott szuperoxidanion termelést in vivo hydroethidin-ethidium fluoreszcenciával kívánjuk kimutatni. Tervezett kísérleteink eredményei hozzájárulhatnak racionális neuroprotektív stratégia kidolgozásához, amely mérsékelheti a perinatális hipoxia/iszkémia neurológiai következményeit.

Following neonatal cerebral hipoxic/ischemic stress both direct neuronal and indirect vascular and glial mechanisms contribute to the consequent neuronal damage. We hypothesize that the neuroprotection afforded by the pharmacological activators of ATP-sensitive potassium channels located in the inner mitochondrial membrane (mitoKATP) is in part due to the protection of cerebrovascular cells.
The prototype mitoKATP opener diazoxide induces chemical preconditioning, but also induces mitochondrial superoxide production that may be involved in its preconditioning effect. Thus, we plan to use another mitoKATP activator (BMS191095) too, since BMS did not induce superoxide production in mitochondria isolated from piglet brain.
We wish to further investigate the protective mechanism of mitoKATP activation in piglets. We will test the hypothesis if the acute preconditioning employed by mitoKATP opening is not related to mitochondrial superoxide production in cells of the neonatal brain. The protective effect of preconditioning will be evaluated based on the preservation of ischemia-sensitive neuronal-vascular mechanisms, and the retained integrity of the blood-brain barrier. The specificity of the drugs on mitoKATP will be proven by using mitoKATP-selective and plasmalemmal surface KATP (sKATP)-selective antagonists. Cerebrovascular changes will be assessed with intravital microscopy, laser-Doppler and laser-speckle flowmetry enabling us to selectively determine the involvement of pial and intraparenchymal arterioles in the responses. Blood-brain barrier integrity will be determined with the Evans-Blue dye extravasation technique. Diazoxide and/or BMS induced superoxide production will be assessed in vivo by the hydroethidin-ethidium fluorescent technique. Our studies may emanate in results that can contribute to the development of rational neuroprotective strategy to diminish the neurological dysfunction after perinatal hypoxic/ischemic stress.

Az újszülöttkori agyi hipoxiát/iszkémiát követő idegrendszeri károsodások kialakulásában direkt neuronális mechanizmusok, valamint az ún. neurovaszkuláris egység diszfunkciója is fontos szerepet játszanak. Vizsgálatainkban a mitokondriumok belső membránjában található ATP-szenzitív káliumioncsatornák (mitoKATP) direkt farmakológiai aktivátorai által közvetített ún. farmakológiai prekondicionálást vizsgáltuk, ill. azt, hogy más farmakológiai támadáspontú vegyületek hasonló prekondicionáló hatása kapcsolatba hozható-e a mitoKATP vel. Ezeknek a vizsgálatoknak legfontosabb eredménye az, hogy míg a szelektív mitoKATP nyitók nem okoznak feltétlenül mitokondriális eredetű reaktív oxigénmetabolit (ROS) termelést, más hatékony farmakonok gyakran nem specifikus mitokondriális támadáspontokon, a mitokondriális ROS termeltetésén keresztül hozzák létre a prekondicionált fenotipust. In vivo vizsgálatainkban a neurovaszkuláris egység diszfunkcióját hipoxia/iszkémia érzékeny cerebrovaszkuláris érreakciók segítségével határoztuk meg. Kísérleti eredményeink egyrészt új információkkal gazdagították az érfunkciók károsodásának patomechanizmusát, másrészt sikerült új támadáspontokat azonosítani, melyek klinikai transzlációs kutatások alapját képezhetik, melyek az újszülöttkori hipoxiás/iszkémiás enkefalopátia kezelésében segíthetnek.

Following neonatal cerebral hipoxic/ischemic stress direct neuronal injury and the dysfunction of the so-called neurovascular unit both contribute to the developing encephalopathy. The pharmacological preconditioning induced by the activators of the mitochondrial ATP-sensitive potassium channels (mitoKATP) was investigated and whether other drugs with different primary actions would also trigger preconditioning through the involvement of mitoKATP. The most important message emanating from these studies was that though selective mitoKATP openers do not elevate mitochondrial reactive oxygen species (ROS) production, other effectively preconditioning drugs often create the preconditioned phenotype through non-specific actions on mitochondria mediated by ROS. lin the inner In our in vivo studies, neurovascular dysfunction was assessed with hypoxia/ischemia sensitive cerebrovascular responses. Our results provided new insights into the mechanism of altered cerebrovascular reactivity after hypoxic/ischemic stress, and they also identified new targets which after further translational studies may yield new therapies to treat hypoxic/ischemic encephalopathy in the newborn.
