Pályázatunkban három pillérre építve vizsgáljuk a gyermekkori krónikus vesebetegségek etiológiáját és szövődményeinek kialakulását.

(a) Elemezzük a gyermekkori veseelégtelenség egyik leggyakoribb okaként megjelenő vesicoureteralis reflux/húgyúti infekció során kialakuló vesehegesedés genetikai hátterét. Kutatásunk során olyan polimorfizmusok szerepét vizsgáljuk, amelyeknek humán megfigyelések ill. az állatkísérletes adatok alapján szerepük lehet a fibrosis kialakulásában vagy megelőzésében (VEGF, TGF-beta, iNOS, eNOS). Célunk olyan adatbázis létrehozása, amely a betegellátásban segítséget nyújthat a fokozott kockázatú VUR betegek kiszűrésére.

(b) Vizsgálatunkban felmérjük a krónikus vesebetegek korai és késői cardiovascularis szövődményeit. IgA nephropathiában és gyermekkori nephrosis syndromában a korai vascularis szövődmények vizsgálatát tervezzük érfalmerevség meghatározással. Célunk e két betegcsoport keresztmetszeti vizsgálata és hosszútávú követése. Emellett folytatjuk a előző pályázatunkbanl megkezdett késői vaszkuláris szövődmények vizsgálatát uraemiás és transzplantált gyermekekben.

(c) Állatkísérletes modellen vizsgáljuk a vese ischemiás/reperfusiós károsodása során szintetizálódó VEGF szerepét, amelynek feltehetően protektív hatása van a patkányban, kísérleti körülmények között létrehozott akut veseelégtelenségben. Ez a modell mélyebb betekintést enged a veseelégtelenség molekuláris folyamataiba, elemzésével lehetővé válik a lehetséges protektív tényezők leírására
Summary
In the present investigation the etiology and consequences of chronic renal diseases are studied on three different levels.

(a) Evaluation of the genetic background of the most common cause of end stage renal disease in childhood: single nucleotide polymorphisms associated with renal scarring caused by urinary tract infection and/or vesico-ureteral reflux. Polymorphisms supposed to play role in the renal scarring according observational studies and animal models of the disease (VEGF, TGF-beta, iNOS, eNOS SNPs) are evaluated. Our aim is to create a database from the polymorphisms and to describe genetic covariations that affect the clinical course of renal scarring and predict unfavorable outcome.

(b) Determination of cardiovascular disorders in chronic renal disease in childhood: early detection of arterial stiffness with the non-invasive measurement of pulse wave velocity in children with IgA nephropathy and nephrotic syndrome.A cross-sectional and a follow-up study of the vascular changes in the above-mentioned pediatric populations is planned. In addition, based on our previous results on arterial stiffness in uremic and transplanted patients, we will continue with the uremic and the transplanted patients’ follow-up study.

(c) Our aim is to analyze the renal synthesis of VEGF, a candidate factor supporting postischemic recovery of the kidney in a rat model of ischemia/reperfusion injury induced acute renal failure. The use of this rat model enables a better understanding of the molecular events of acute renal damage and may elucidate the protective mechanisms involved.

Kutatásunk klinikai és állatkísérletes szakaszból állt. 1. klinikai szakasz. 1.1. Megállapítottuk, hogy olyan vesetranszplantált gyermekekben, akiknél a veseelégtenség anatómiai malformáció talaján alakult ki a HSPA1B(1267)G allél előfordulása szignifikánsan gyakoribb, ami felveti a HSP72 fehérje esetleges kóroki szerepét a vesekárosodás kialakulásában 1.2. A krónikus vesebetegek korai érfali változásának jellemzésére a pulzushullám terjedési sebesség (PWV) alkalmas. Egy 1008 fős gyermek adatbázis alapján megalkottuk a PWV gyermekkori kor és magasságfüggő percentilis táblázatait. Elvégeztük továbbá három, a PWV mérésére alkalmas készülék keresztvalidációját. Megállapítottuk, hogy a készülékek adatai egymással jó közelítéssel összehasonlíthatóak. Megállapítottuk, hogy veseátültetést követően is magasabb PWV az urémiás terhelés következménye. Az érfal állapotát a mészlerakódást fokozó és gátló folyamatok egyensúlya határozza meg. A második évtől a diszlipidémia már fokozottabb szerepet játszik a folyamatban 2. Állatkísérletben vizsgáltuk a dehydroepiandrosteron (DHEA) és az erythropoietin (EPO) előkezelés hatását ischemia-reperfuziós modellen. Megállapítottuk, hogy DHEA kezelés a veseszövet IL-1beta, IL-6 és VEGF szintézisének változását eredményezte. EPO-val történő előkezelés szignifikáns protektív hatással járt a szövetkárosodás, és a vesefunkció tekintetében, melynek kialakulásában SGK-1 játszik szerepet.

The study was performed on a clinical and experimental level. 1.Clinical studies. 1.1 Our findings on heat shock protein polymorphisms in kidney transplanted children suggest an association between the carrier status of HSPA1B (1267)G with urinary tract malformations, leading to end-stage renal disease 1.2 Carotid-femoral pulse wave velocity (PWV) is an established method to characterize aortic stiffness. Our study, involving 1008 children, is the first to provide age and height specific pediatric reference values for PWV. Our work cross-validating three devices suitable to measure PWV in children showed a good concordance between the instruments. This allows to extrapolate the results between previously established normal PWV values and forthcoming studies using these devices to assess children at long-term risk of cardiovascular disease. Uremic burden (elevated CaxP product, calcitriol treatment before transplantation) and disturbed bone metabolism had a major impact on PWV after transplantation. Two years following transplantation the presence of disturbed lipid metabolism gains importance in determining PWV 2.The objective of our experimental study was to examine the renoprotective effects of dehydroepiandosteron (DHEA) and erythropoietin (EPO) in renal ischemic/reperfusion (I/R) injury in rat model. We found that DHEA pretreatment influences renal IL-1beta, IL-6 and VEGF synthesis. EPO pretreatment protects from renal I/R injury through SGK1 mediated mechanism
