A hosszú szénláncú, többszörösen telítetlen zsírsavakkal való perinatális ellátottság a gyermekek fejlődésével és egészségük megőrzésével való összefüggését három területen kívánjuk vizsgálni. 1. DNS kivonását tervezzük azokból a női tejmintából (n = 769), melyeknek a zsírsavösszetételét korábban már vizsgáltuk, majd nemzetközi együttműködésben tervezzük a zsírsav deszaturáz enzimek (FADS1 FADS2) genetikai polimorfizmusának a meghatározását. Tervezzük továbbá az ugyanabból a klinika vizsgálatból származó anyai és csecsemő plazmaminták zsírsavösszetételének a meghatározását. A FADS1 FADS2 polimorfizmus és a női tej és a plazmalipidek zsírsavösszetételére vonatkozó adatok együttes értékelése hozzájárulhat a zsírsavak metabolizmusa jobb megismeréséhez. 2. Az Egészségügyi Világszervezet a szoptatás javasolt időtartamát 6 hónapra kiterjesztő javaslata alapján tervezzük a laktáció késői időszakának (6-12 hó) megfelelő női tej zsírsavösszetételének vizsgálatát. 3. A köldökzsinórvérben mért magas transz izomér zsírsavértékek a 18 hónapos csecsemők rosszabb neurológiai fejlődésével észlelt összefüggése alapján tervezzük zsírsav-analitikai profilunk a korábban nem mért transz izomerekkel, mindenekelőtt a konjugált linolsavval történő kiegészítését. A fenti vizsgálatok sikeres megvalósítása megteremtheti elméleti alapját annak, nyomon kövessük azoknak a gyermekeknek a fejlődését, akiknek az anyja a várandósság során hosszú szénláncú, többszörösen telítetlen zsírsavakat tartalmazó étrendi kiegészítő kezelést kapott egy az EU 5. keretprogram támogatásával 2002-2003-ban megvalósított vizsgálatunkban.
Summary
We plan to investigate the role of perinatal long-chain polyunsaturated fatty acid (LCPUFA) supply in the development of children and in the maintenance of their health within three main research objectives. 1. We plan to extract DNA from the human milk samples (n = 769) that we analysed previously and to determine (in international collaboration) the gene cluster polymorphism of fatty acid (FA) desaturase enzymes (FADS1 FADS2). We also plan to measure FA composition in maternal and infantile plasma samples originating form the same study. Complex evaluation of FADS1 FADS2 polymorphism with FA compositional data of human milk and blood plasma may contribute to our better understanding of FA metabolism. 2. The decision of the World Health Organisation to enhance the recommended duration of breast-feeding to 6 month renders it imperative to investigate the hitherto somewhat neglected question of FA composition of human milk at and after the 6th month of lactation. 3. On the basis of the first human report on the association of high trans isomeric FA values in cord blood to poorer neurological outcome in childhood, we plan to improve our analytical methods and extend the scope of the trans FAs quantified in plasma samples also to conjugated trans linoleic acids. Successful realisation of these research objectives will provide the theoretical background for following up between the years of 2009 and 2012 the development of those children whose mothers received dietary supplementation of long-chain polyunsaturated fatty acids in one of our previous studies carried out with the support of the 5th Framework of the EU in 2002-2003.

Igazoltuk, hogy női tej minták maradékai alkalmasak DNS kivonására, aminek az analízisével megállapíthattuk, hogy szignifikáns összefüggés áll fenn a zsírsavak szintézisében szerepet játszó deszaturáz enzimek polimorfizmusa és az arachidonsav-értékek, valamint az enzimaktivitást leíró biokémiai jellemző között [Lattka és mtsai, Am J Clin Nutr 2009]. Női tej mintákat a tejelválasztás 6. hetében és 6. hónapjában vizsgálva megállapítottuk, hogy 18 szénatomot tartalmazó transz izomér telítetlen zsírsavak értékei szignifikáns fordított korrelációban vannak a legfontosabb hosszú szénláncú, többszörösen telítetlen zsírsavak értékeivel [Szabó és mtsai, J Pediatr Gastroenteorl Nutr 2010]. Hasonló fordított összefüggést találtunk a vörösvértest membránlipidek foszfatidilkolin és foszfatidiletanolamin frakciójában várandós anyákban a terhesség 20. és 30. hetében és a szüléskor egyaránt [Decsi és mtsai, Ann Nutr Metab 2011]. Az irodalom 51 közleményének rendszerezett áttekintése során azt találtuk, hogy amíg a telített és egyszeresen telítetlen zsírsavak értékei a férfiakban magasabbak, addig a nőkben a hosszú szénláncú, többszörösen telítetlen zsírsavak szignifikánsan magasabb értékei állapíthatóak meg [Lohner és mtsai, Ann Nutr Metab 2013].

We demonstrated that the remnants of the milk samples analysed contain enough cellular material to allow the extraction of DNA, and performed DNA extraction. We observed significant associations of fatty acid desaturase genotypes with arachidonic acid concentrations and the surrogate parameter of delta-5-destaurase activity [Lattka et al, Am J Clin Nutr 2009]. We investigated human milk samples obtained at the 6th week and 6th month of lactation and found that the availability of 18-carbon trans isomeric fatty acids was inversely associated to the availability of several long-chain polyunsaturated fatty acids in human milk at the 6th month of lactation [Szabó et al., J Pediatr Gastroenterol Nutr 2010]. We reported fatty acid compositional data of erythrocyte phosphatidylcholine and phosphatidylethanolamine lipids in women at the 20th and 30th week of gestation, at delivery and in their newborns and found that the sum of trans fatty acids significantly inversely correlated to docosahexaenoic acid in phosphatidylcholine lipids [Decsi et al, Ann Nutr Metab 2011]. Our systematic review of the literature showed on the basis of 51 publications reviewed that the contributions of saturated and monounsaturated fatty acids were higher, whereas those of AA and DHA were lower in men than in women; however, sex explained only ≈ 2% of the variability of plasma phospholipid DHA values [Lohner et al, Ann Nutr Metab 2013]. These data highlighted the importance of planning a subgroup analysis by sex in perinatal LCPUFA supplementation trials.
